Abstract:Many diseases cause modification of plasma protein fractions which amy be studied by electrophoretic analysis, The clinician receives valuable information enabling him to confirm diagnoses and guide treatment in cases of hemoconcentration, malnutrition and certain staets of disturbed protein equilibrium such as myeloma, nephosis, cirrhosis, kala azar and lymphogranuloma venereum. Hyperglobulinemias have been observed in various cases of leprosy by several authours 1,2,3,4, including Seibert and Nelson5, who were the first to analyse the serum of three leprosy patients by electrophoretic methods, findings increased gamma and alpha globulin fractions, without including clinical data or the classificationi of their cases. In order to make electrophoretic analyses available to the clinical leprologist, the authours undertook to study leprosy under its various clinical forms by combining clinical findings with electrophoretic data. The present paper brings the results of their first 24 cases. Leprosy patients who had not undergone previous antileprotic treatment were examined on arrival at the Departamento de Profilaxia da Lepra where they appeared for registration. Biopsies were performed for histopathological examination and venous blood was collected for electrophoretic analysis. The Mitsuda reaction could not be followed in most of the cases who bacame interned in distant leper colonies. The cases were divided into 3 groups, the first comprising the lepromatous and the other two the incharacteristics and tuberculoid forms respectively. Electrophoretic examinations revealed an increased gamma globulin fraction in the first group, with practically normal patterns in the other two groups. There was but one exception where a lepromatous case showed no gamma globulin increase. In the inchacateristic group there were 2 cases of slight gamma globulin increased and 2 others with a tendency towards a rising gamma fraction, all of which gave biopsy findings of lepromato...(AU)^ien.

Abstract:To assess methods for detecting peripheral neuropathy, 28 previously untreated patients with lepromatous Hansen's disease underwent upper extremity manual muscle testing, sensory testing by using monofilaments, and electrophysiological nerve conduction studies (motor and sensory) at their initial examination. All but three patients demonstrated some abnormality identified by at least one of the testing procedures. Sensory testing with monofilaments located the greatest number of abnormalities found in 24 of the 28 patients. Next, electrophysiological testing demonstrated neuropathy in 21 of the 28 patients tested; 20 of these patients had sensory abnormalities and 20 had motor irregularities. The least sensitive method was manual muscle testing, which detected abnormalities in only 12 patients. Furthermore, sensory testing with monofilaments revealed peripheral neuropathy in 5 patients whose electrophysiological studies showed normal sensory patterns, and electrophysiological testing detected abnormalities in 1 patient whose sensory monofilament examination was normal. The results of this study support the usefulness of all three testing modalities.(AU).
