Abstract:Reported here are the cases of two HIV-positive patients with skin lesions suggestive of leprosy, based on clinical and pathological analysis, which worsened during the few weeks following initiation of highly active antiretroviral therapy. The lesions improved after a few weeks of multidrug therapy for leprosy. Mycobacterium leprae was confirmed by polymerase chain reaction analysis of blood in case 1 and of a biopsy sample in case 2. Neither Mycobacterium avium complex nucleic acid, which is usually associated with immune restoration syndrome, nor mycobacterial cutaneous manifestations were detected in either case(AU).

Abstract:A importancia mundial da farmacovigilancia vem aumentando, mesmo nos paises subdesenvolvidos. Neste contexto, a sindrome de hipersensibilidade a dapsona (SHD), um efeito colateral raro e potencialmente grave, deve ser considerada, sem, entretanto, comprometer a seguranca da droga e sua ampla utilizacao. Objetivando padronizar os criterios diagnosticos da sindrome, realizou-se uma revisao sistematica da literatura analisando trabalhos, publicados de quinze paises endemicos entre 1956 e 2001. Cento e oito casos foram reportados neste periodo, destes 96,2% apos 1980, com 13,3% de obitos. Neste grupo, 59% apresentaram a sindrome completa - febre, rash, linfadenopatia e hepatite, enquanto 41% expressaram forma incompleta. A taxa de mortalidade do grupo que preenche os criterios de SHD completa foi 9,6% e nao houve associacao estatistica com injuria hepatica. Todavia ressalta-se pouca qualidade dos dados coletados, o que reforca a necessidade de relatos mais precisos em futuras publicacoes. (AU).

Abstract:Asthma and eczema (atopic dermatitis) are characterized by a number of unexplained phenomena: the familial aggregation of disease, the initiation of disease by apparently trivial exposure to allergens, the preferential transmission of disease from affected mothers and the large increase in prevalence of disease in Westernized societies in the last century. A number of genes and chromosomal regions have been identified that consistently show linkage to asthma and its related phenotypes. Known loci modify the strength of the atopic response, nonspecific inflammation, the ability to respond to particular allergens and nonspecific airway reactivity. Eczema has been shown to be due to a different set of genetic loci that are shared with other skin diseases such as psoriasis and leprosy. Genetic and genomic studies both provide evidence that epithelial surfaces are active in the induction of allergic disease. (AU).

The reaction included fever, malaise, dermatitis, jaundice with hepatic dysfunction, lymphadenopathy, and hemolytic anemia. Abstract:Sera from C3H and C57BL mice infected with Mycobacterium lepraemurium (MLM) and from human leprosy patients were examined for antibodies against MLM by a crossed immunoelectrophoresis (CIE) technique. Altogether antibodies against six or seven MLM antigens were found in the mouse sera. After a small inoculum of MLM, C3H mice produced more antibodies than C57BL mice. After a large inoculum both strains produced about the same amount of antibodies but showed qualitative differences in their response patterns. A serum pool from patients with lepromatous leprosy contained antibodies against six MLM antigens; five of these were identical with the antigens against which antibodies were found in infected mice. C57BL mice that had lost their delayed-type hypersensitivity to MLM during the course of a disseminated infection showed a fairly strong antibody response to three antigens and a weak response to three others. Since almost the same spectrum of antibodies, although in lower titres, could be demonstrated in C57BL mice with intact delayed-type hypersensitivity to the bacilli, these antibodies did not seem to interfere with the expression of cell-mediated immunity.(AU).

Abstract:This study illustrates the consequences of smallpox revaccination in 45 lepromatous, 28 tuberculoid, and 47 normal individuals. Results obtained with leprosy were associated with a relative anergy against the vaccinia virus, the anergy being minimal in the tuberculoid leprosy but marked in the cases with lepromatous leprosy.(AU).

Abstract:Lepromatous leprosy (LL) patients whose bacillary load has decreased to almost undetectable levels by long-term chemotherapy failed to develop delayed-type hypersensitivity (DTH) to Mycobacterium leprae antigen following immunization with killed armadillo-derived m. leprae. When these LL patients were immunized with killed M. leprae in a mixture with live BCG, only DTH to purified protein derivative (PPD) was induced. These results are further evidence that immunological unresponsiveness to the leprosy antigen of patients with lepromatous leprosy is antigen-specific and non-reversible.(AU).

Abstract:Guinea-pigs were immunized with Mycobacterium leprae in saline and with autoclaved preparations of Mycobacterium w., ICRC bacillus, Mycobacterium phlei and Mycobacterium vaccae. A group of animals were also immunized with live Mycobacterium w. All animals were challenged after one month of M. leprae and other mycobacteria. Induration produced in response in guine-pigs to challenge with homologous mycobacterial preparations and M. leprae. In most cases, the early reaction was higher with homologous antigens as compared to M. leprae antigens were however of comparable order especially in animals immunized with Mycobacterium w, and ICRC bacillus. Animals immunized with M. leprae gave low late reactions with preparations from Mycobacterium phlei and Mycobacterium vaccae.(AU).

Abstract:Guinea pigs immunized with intact or disrupted armadillo-grown human Mycobacterium leprae administered in aqueous or oil vehicles were tested with various dilutions of M. leprae suspended in saline, water-soluble M. leprae extract, purified protein derivative, and a water-soluble extract of normal armadillo tissue. The results demonstrated the following. (i) Under no conditions was any skin test reactivity found to normal armadillo tissue extract. (ii) Positive sensitization to both M. leprae and its water-soluble extract was achieved by sensitizing guinea pigs with M. leprae suspended in Hanks solution or saline. Autoclaved M. leprae in Hanks solution or saline inoculated intradermally was an effective immunogen. Oil suspensions or emulsions were effective at sensitization, but appeared to be no better and, in general, slightly weaker, than simple inoculation in aqueous suspension. (iii) Living BCG failed to reveal a significant adjuvant effect on sensitization to M. leprae. However, cord factor appeared to potentiate slightly the sensitization to M. leprae in aqueous suspension. (iv) The minimum dose required for sensitization with M. leprae in aqueous suspension was 55 micrograms of purified bacilli. (v) Animals inoculated with M. leprae in saline or with M. leprae together with BCG showed positive skin test reactivity to the first skin test application made fully 1 year after the initial sensitization. The efficacy of autoclaved, irradiated M. leprae in aqueous, oil-free medium suggests a relatively safe approach to human vaccination studies.(AU).

Abstract:Antigens prepared from five mycobacteria viz. M. leprae M. vaccae, M. phlei, M.W and ICRC bacillus were used for assessing cross reactivity between different antigens on sixteen patients with different forms of leprosy. ICRC bacillus and M.W gave the best correlation with M. leprae.(AU).
