Spiro-piperidyl-rifamycins ..- Source: s.l; s.n; Oct. 1980. 6 p. tab. Abstract: The biological properties of spiro-piperidyl-rifamycins, a new class of rifamycin antibiotics, are described. In these derivatives the positions 3 and 4 have been incorporated into an imidazolyl ring bearing a spiro-piperidyl group N substituted with linear and branched aliphatic chains. The in vitro antibacterial activity against Staphylococcus aureus and Mycobacterium tuberculosis increases with the number of the carbon atoms in the linera side chain, whereas the inhibitory effect on Escherichia coli is lowered. The antibacterial activity is only marginally affected by branching of the side chain. In vivo (experimental infections of mice) the optimal therapeutic activity against M. tuberculosis is shown by compounds bearing 3 approximately 5 carbon atoms as a linear or branched side chain; in comparison with rifampicin, the potency of these derivatives is 2 approximately 3 times higher. The finding is in a good agreement with the exceptional tissue tropism, which seems to be a favourable property of this group of derivatives. Abstract: Seventy-one Burnese adult patients with lepromatous leprosy were treated with regimens of rifampicin monotherapy, 450 mg daily for 60 days or relapsed after stopping DDS therapy, 20 were intolerant of DDS, 18 were DDS resistant and 15 had received no previous treatment. Rifampicin produced a 75 por cento reduction in the size of skin nodules in two thirds of the patients and a complete disappearance of nodules in the others. After one month drug treatment the MI fell to zero but the BI remained unchanged. the once weekly regimen was as effective as the daily treatment. Four patients had to be withdrawn due to ENL reactions.. Abstract: O efeito do tratamento local do nariz de paciente do tipo lepromatoso com diferentes formulaçöes de rifampicina em gotas ou "sprays" nasais, foi investigado em um grande número de pacientes. As preparaçöes foram ou aspergidas ou instiladas dentro das narinas depois de sua limpeza por jato com soluçäo salina normal a 37-C. Observou-se que 10 mg/ml de rifampicina eram eficazes na reduçäo do BI e do MI a zero no nariz em sete dias na maioria dos pacientes. Näo foi visto efeito desagradável em qualquer dos paciente. Sugere-se que "sprays"/gotas nasais possam prevenir a transmissäo da hanseníase, uma vez que o nariz é reconhecido ser uma importante porta de saíde de M. leprae. Além disso a deformidade nasal pde ser prevenida quando a rifampicina "sprays"/gotas é usada täo logo o diagnóstico é feito. Acredita-se que o tratamento local juntamente com a terapia sistêmica contribuiriam muito no controle da transmissäo da hanseníase. (AU). Abstract: Sete pacientes consecutivos, de um grupo de 283 portadores de Hanseníase em tratamento com esquema terapêutico multidroga (MDT), foram referidos ao Hospital Evangélico de Londrina e ao Hospital Universitário Regional Norte do Paraná, no período de 1989 a Novembro de 1991, devido a insuficiência renal aguda (I.R.A.). Nestes sete pacientes ocorreram oito episódios de IRA, acompanhados de epigastralgia (8/8), febre (6/8), vômitos (5/8), anúria (4/8), urina escura (4/8), dores ósseas acompanhadas de mialgias (5/8) e icterícia (3/8). Quatro pacientes necessitaram tratamento dialítico, e três foram submetidos à biópsia renal que demonstrou Nefrite Rúbulo Intersticial Aguda. Os autores atribuem estas manifestaçöes ao uso intermitente da rifampicina e sugerem cuidados a serem observados quando se usa esquema multidroga (MDT). (AU). Abstract: Säo apresentados 4 pacientes com resistência a mais de uma das drogas que fazem parte dos esquemas terapêuticos preconizados pela OMS contra a hanseníase. Os autores discutem o modo de aparecimento dessa múltipla resistência e consideram a possibilidade de ocorrer esse fenômeno de maneira inicial, isto é, paciente que se infectam com bacilos já resistentes a mais de uma droga. Lembram o que está acontecendo nos Estados Unidos com a tuberculose, onde aumentou consideravalmente o número de casos com resistência inicial a múltiplos fármacos e admitem a possibilidade da AIDS que é responsável pela maior parte dessa situaçäo, vir a ter papel semelhante na hanseníase. Alertam também para o fato de drogas ainda em fase experimental e que já se acham disponíveis no comércio, começarem a ser utilizadas indevidamente, comprometendo o seu uso no futuro. (AU).
