Resumo: Extracts from forty-three plant species were selected on account of reported traditional uses for the treatment of TB and/or leprosy. These were assayed for antimycobacterial activities. A simple in vitro screening assay was employed using two model species of mycobacteria, M. aurum and M. smegmatis. Crude methanolic extracts from three of the plants, C. mukul, P. corylifolia and S. canadensis, were found to have significant antimycobacterial activity against M. aurum only (MIC=62.5 microg/ml). Bioassay guided fractionation led to the isolation of two known benzophenanthridine alkaloids, sanguinarine (1) and chelerythrine (2), from the roots S. canadensis and the known phenolic meroterpene, bakuchiol (3) from the seeds of P. corylifolia. The fractionation of the resin of C. mukul lead to a decrease in antimycobacterial activity and hence further work was not pursued. Compound (2) was the most active against M. aurum and M. smegmatis (IC(50)=7.30 microg/ml [19.02 microM] and 29.0 microg/ml [75.56 microM], respectively). M. aurum was the most susceptible organism to all three compounds. No significant difference in antimycobacterial activity was observed when the two alkaloids were tested for activity in media of differing pH values. The activities of the pure compounds against M. aurum were comparable with those against M. bovis BCG with compound (2) being the most active (M. bovis BCG, IC(50)=14.3 microg/ml [37.3 microM]). These results support the use of these plants in traditional medicine. (AU). Resumo: Estudo descritivo, objetivando avaliar o programa de controle da hanseníase do Município de Buriticupu, Maranhão. Foram analisados 214 prontuários de pacientes portadores das diversas formas clínicas de hanseníase, atendidos na unidade de saúde da Universidade Federal do Maranhão, localizada no referido município. A população de estudo foi constituída de 110 casos de hanseníase das formas paucibacilares (PB) e 104 das formas multibacilares (MB) registrados no período de janeiro de 1991 a dezembro de 1995. As informações referentes à forma clínica, número de contatos registrados, examinados e avaliados, grau de incapacidade no início e final do tratamento e situação de registro, foram coletadas num formulário específico. A análise dos resultados foi baseada nos parâmetros dos indicadores operacionais definidos pelo Ministério da Saúde. Houve um discreto predomínio da forma PB da doença. Observou-se baixos percentuais em relação aos pacientes com grau de incapacidade física avaliada no início e final do tratamento, a alta por cura e contatos examinados. O percentual de abandono foi elevado. O programa foi considerado "precário" em todos os indicadores utilizados para o estudo. (AU). Resumo: A series of drugs that had been found active against Mycobacterium leprae in mice by the continous method of drug administration was tested by the kinetic method. Vadrine and vyomivin were found inactive. Cephaloridine, streptovaricin, and rifampin gave bactericidal-type results. In a second experiment, rifampin was found to have distinct bactericidal effect when given for only 2 days. The plasma levels of rifampin that were associated with bactericidal effect in mice were in the range reported for man receiving acceptable dosages of rifampin. Cephaloridine and, especially, rifampin merit further investigation in clinical trials in leprosy patients, either as single drugs or in combination with other active drugs. The combination of rifampin and dapsone (DDS) or acedapsone (DADDS) appear to provide the advantages of tboth drugs.(AU). Resumo: The effectiveness of a BCG vaccination programme in protecting against leprosy was assessed by case-control and cohort analyses of data from the Lepra Evaluation Project in Karonga District, Northern Malawi. Results indicate that BCG provides at least 50% protection against leprosy in this population and that protection is independent of age, sex, schooling status, or location within the project area. Agreement between these findings and those from a controlled trial in Uganda indicates that BCG is sufficiently effective against leprosy in East and Central Africa to be considered an important element of leprosy control in that region.(AU). Resumo: To assess methods for detecting peripheral neuropathy, 28 previously untreated patients with lepromatous Hansen's disease underwent upper extremity manual muscle testing, sensory testing by using monofilaments, and electrophysiological nerve conduction studies (motor and sensory) at their initial examination. All but three patients demonstrated some abnormality identified by at least one of the testing procedures. Sensory testing with monofilaments located the greatest number of abnormalities found in 24 of the 28 patients. Next, electrophysiological testing demonstrated neuropathy in 21 of the 28 patients tested; 20 of these patients had sensory abnormalities and 20 had motor irregularities. The least sensitive method was manual muscle testing, which detected abnormalities in only 12 patients. Furthermore, sensory testing with monofilaments revealed peripheral neuropathy in 5 patients whose electrophysiological studies showed normal sensory patterns, and electrophysiological testing detected abnormalities in 1 patient whose sensory monofilament examination was normal. The results of this study support the usefulness of all three testing modalities.(AU). Resumo: Assessment of bacteraemia has been made at weekly intervals in 36 lepromatous leprosy patients who were put on different antileprosy drug under four regimens, viz., DDS alone, DDS in combination with rifampicin (DDS + RIF), clofazimine (DDS + CLF) and thiacetazone (DDS + TCT). In general, with the continuation of treatment the bacillary load in the blood decreased considerably while bacteriological index (BI) of the skin remained constant during the study. No significant difference was noted in M. leprae clearance from blood between the groups treated with DDS alone and groups treated in combination with CLF and TCT. However, DDS + RIF treatment was most efficient in clearing acid-fast bacilli (AFB) from blood as compared to those noted with other drug regimens.(AU). Resumo: O reconhecimento do grande número de pacientes com lesões oculares e a inexistência de um controle oftalmológico sistematizado no Hospital Lauro de Souza Lima motivaram a dinamização do serviço de oftalmologia com a participação da enfermagem. Estabeleceram-se os seguintes objetivos: - oferecer atendimento permanente e sistematizado de prevenção e tratamento oftalmológico; - desenvolver e aplicar técnicas simples de prevenção e tratamento; - ampliar os conhecimentos específicos da equipe de enfermeiras; - oferecer campo de treinamento profissional. O elemento básico para o sucesso da unidade foi a disposição das enfermeiras em assumir o trabalho, a cooperação e a disponibilidade do médico oftalmologista e o estabelecimento de rotinas de trabalho. A rotina para avaliação dos comprometimentos oculares tem duas etapas: 1) Entrevista com o paciente para identificação dos sintomas subjetivos dos comprometimentos oculares; 2) Exame ocular: área superciliar e ciliar; vias lacrimais; pálpebras; músculos orbiculares; conjuntiva; episclera; esclera; córnea; pupila; cristalino; pressão intra-ocular; acuidade visual. Na rotina descrita, utiliza-se um mínimo de recursos que permite, à enfermagem, identificar os principais problemas oftalmológicos do paciente nos quais pode intervir com ações preventivas e curativas.(AU).
