Título:Reanimation of the lagophthalmos using stainless steel weight implantation; a new approach and prospective evaluation.

Resumo:Weight implants have been used for over 30 years in surgical rehabilitation following lagophthalmos. However, there has been a renewed interest among ophthalmologists in recent years. The weight implant technique combined with lateral canthus resuspension was performed on 22 eyes in Thai patients. It is a reliable method for correcting lagophthalmos and ectropion. In previous studies almost pure gold was used in 99.99% of eyelid implants. However, we wanted to use other materials which are not expensive and are readily available. A stainless steel plate was selected. The majority of our surgeries were done with a stainless steel weight implant of 1.368 g. Patients were followed for 7 to 55 months, and no rejections of the implants were observed. This study achieved a > or = 90% success rate, and showed that a stainless steel plate is an alternative to gold for the weight implant technique. (AU)^ien.

Título:Ocular complications in newly diagnosed borderline lepromatous and lepromatous leprosy patients: baseline profile of the Indian cohort

Resumo:Aim: To describe ocular manifestations in newly diagnosed borderline lepromatous (BL) and lepromatous leprosy (LL) patients in India. METHODS: Ocular complications, at enrolment, occurring in all new borderline lepromatous and lepromatous leprosy patients detected by active case finding within the geographically defined leprosy endemic area of the Gudiyattam Taluk in India from 1991 to 1997 who consented to ocular examinations every 6 months, during and 5 years after treatment with multidrug therapy (MDT), were studied. RESULTS: Orbicularis oculi weakness (4.62 per cent), lagophthalmos (4.20 per cent), ectropion (0.42 per cent), trichiasis (0.84 per cent), blocked nasolacrimal ducts (1.68 per cent), pterygium (11.34 per cent), impaired corneal sensation (53 per cent), corneal opacity (10.5 per cent), corneal nerve beading (1.68 per cent), punctate keratitis (1.26 per cent), keratic precipitates (4.62 per cent), iris atrophy (1.68 per cent), and cataract (12.6 per cent) were ocular complications seen in the 301 lepromatous patients at enrolment. 4.6 per cent had blind eyes. Increasing age was associated with ocular complications. 80 per cent of patients were skin smear acid fast bacilli (AFB) positive. The LL/BL ratio was 1:6.4. 71 per cent had some limb deformity. 44 per cent had only leprosy related ocular complications (LROC), 28 per cent had only general ocular complications (GOC) while 14 per cent had both LROC and GOC. Ocular complications were significantly related to leg deformities. Corneal nerve beading was seen most in LL patients (100 per cent) having high bacterial content. Lagophthalmos and muscle weakness were associated with reversal reactions. CONCLUSIONS: Corneal nerve beading occurs in LL patients with high bacillary count. Patients with reversal reaction are more likely to present with orbicularis oculi weakness and lagophthalmos. Leprosy related ocular complications and general ocular complications are significant problems in newly diagnosed lepromatous patients. Elderly, deformed, skin smear positive, lepromatous patients are associated with increased ocular morbidity and form a group that require acceptable and accessible eye care. (AU).

Título:Frequency of ocular complications of leprosy in institutionalized patients in NWFP Pakistan

Resumo:BACKGROUND: There is no systemic disease, which so frequently gives rise to disorders of the eye as leprosy does. The study was conducted to determine the prevalence and gravity of ocular complications in institutionalized leprosy patients in NWFP. It is important to provide necessary information to leprosy health workers and general physicians in order to sensitize them to early detection and treatment or referral to appropriate centre. METHODS: A prospective study of ocular complications of leprosy patients was conducted at the leprosy centre of Lady Reading Hospital Peshawar and the Leprosy Hospital Balakot, district Mansehra. The study included a record of the name, age, sex, type, duration of disease and completion of multi-drug therapy (MDT). Classification of the patients was done according to Ridley and Jopling 5-group system. Visual acuity was tested by Snellen chart and those patients having a vision of less than 3/60 were labelled as blind. Ocular adnexa were examined by naked eye and lacrimal sac regurgitation test was done. Slit lamp biomicroscopy was done for anterior segment examination and direct ophthalmoscope was used for fundoscopy. RESULTS: The authors studied 143 patients in the above mentioned leprosy centres. Out of these, 59 had lepromatous leprosy, 39 borderline tuberculoid leprosy, 9 tuberculoid leprosy, 33 borderline lepromatous leprosy, and 33 borderline leprosy. The majority of patients came from the northern districts of NWFP, including Malakand division and district Mansehra. The male to female ratio was 4:1. The age of the patients ranged from 14 to 80 years and the duration of the disease ranged from 1 year to 48 years. Ocular complications were found in 73 per cent of the patients. These complications included loss of eyebrows in 57 patients, loss of eyelashes in 37, corneal changes (including opacity, ulceration, and/or anaesthesia) in 44, iridocyclitis in 31, lagophthalmos in 36, ectropion in 13, and chronic dacryocystitis in 3. Of the total of 15 (11 per cent) patients who went blind from ocular complications, 16 eyes did so due to corneal opacities, 6 eyes due to cataract, 5 eyes due to chronic anterior uveitis and one eye due to corneal ulcer, panophthalmitis and phthisis bulbi each. CONCLUSIONS: A significant number of leprosy patients (73 per cent) have ocular complications. The frequency of ocular complications increases with the increasing age and duration of disease of the patients. (AU).

Título:Ocular complications in incident relapsed borderline lepromatous and lepromatous leprosy patients in South India

Resumo:PURPOSE: To determine the magnitude of ocular complications that present in incident cases of relapsed borderline lepromatous (BL) and lepromatous leprosy (LL) patients. METHOD: From 1991 to 1997, all new BL and LL patients who had relapsed from an earlier disease, detected by active case finding in the geographically defined area of Gudiyattam taluk, were invited for ocular examination after their leprosy status was confirmed clinically and histopathologically. RESULTS: Sixty relapsed lepromatous patients, 45 male and 15 females, were examined. Fifty-two patients had relapsed after receiving only dapsone mono-therapy, 4 after receiving paucibacillary multi-drug therapy (PB-MDT) preceded by dapsone mono-therapy and 4 after only PB-MDT. Three (5 per cent) patients had lagophthalmos, 1 (1.6 per cent) patients each had ectropion and trichiasis, 32 (53 per cent) patients had impaired corneal sensation in both eyes, 2 (3.3 per cent) patients each had corneal opacity (associated with reduced vision), corneal nerve beading, punctate keratitis, keratic precipitates, and iris atrophy, 4 (6.6 per cent) patients had cataract associated with decreased vision, 1 (1.6 per cent) patient had blocked naso-lacrimal duct and 13 (21.7 per cent) patients had pterygium. Seven (12 per cent) patients had a visual acuity of 6/18 or less, 4 (6.7 per cent) patients had 6/60 or less and one patients had vision below 3/60. General ocular complications rather than leprosy-related ocular complications were responsible for reduced vision. Lagophthalmos was associated with increased duration of the disease (P = 0.009), Grade II deformity (P = 0.001), punctate keratitis (P < 0.001) and cataract (P < 0.001). Beaded corneal nerves were associated with lepromatous leprosy (P < 0.001) and high mycobacterial infection (P = 0.05). Patients whose initial disease was categorised as BL and LL had greater impairment of vision (P = 0.037), more iris atrophy (P = 0.013), increased keratic precipitates (P = 0.013) and more corneal nerve beading (P = 0.013), when compared with the group comprising Tuberculoid-tuberculoid (TT), Borderline-tuberculoid (BT) and Intermediate (IND). CONCLUSION: This first report on ocular complications in relapsed lepromatous patients demonstrates that general and leprosy-related ocular complications occur in these patients. However, they are not in excess of those reported in other leprosy groups... (AU).

Resumo:Leprosy in Hawaii dates back to about 1840. The first recorded ocular leprosy is from the 1880s. Robert Louis Stevenson and Jack London both wrote descriptions of the ocular signs of leprosy. Pinkerton in 1927 and Van Poole in 1934 reported large series of patients with ocular leprosy. In 1973 there were about 2,168 cases of leprosy in the 50 states. Most ophthalmologists practice in areas where there are leprosy patients.(AU).

Resumo:The results of a two year survey of eye problems among the patients at the Palo Seco Hospital in the Canal Zone are presented. Only two patients, one classified as having lepromatous leprosy and the other as having the tuberculoid form of the disease, failed to exhibit ocular complications. The high prevalence of leprotic ocular disease (96%) is most probably due to the advanced age of the patients, the lengthy duration of their illness, and the high percentage of patients afflicted by the lepromatous form of the disease.(AU).

Resumo:The ocular condition 8,235 Malawi leprosy patients surveyed; 6.3 per cent were found to suffer from eye lesions and only 0,3 per cent. were blind. The patients are classified into lepromatous, broderline, and tuberculoid groups. in lepromatous patients eye lesions occur in the later stages of the sidease, and in tuberculoid patients at an earlier stage. Lepromatous patients are affected mainly by iridocyclitis and superficial puntacte keratitis. Tuberculoid patients mainly from lagophthalmos and corneal anaesthesia due to nerve palsy. The lw incidence of ocular manifestations is considered to be due in part to the wide use of sulphones; this is especially true in the lepromatous group, in which only 6 per cent. had eye lesions while under dapsone treatmet. In tuberculois patients under current treatment eye manifestations were present in 4i per cent. Steroids are highly effective in controlling hypersensitivity reactions of the uvea.(AU).

Resumo:Fifty-one Montangards in South Vietnam with proven leprosy were studied ophthalmologically. Of these, 39 had ocular involvement. The cornea was involved in 31 of the 39 patients. About the same percentage of ocular complications were seen in cases of lepromatous leprosy as tuberculoid and the mixel type. Five patients were blind.(AU).

Resumo:Ophtalmic findings suggestive of leprosy include loss of eyebrows and eyelashes, beaded corneal nerves, puntacte subepithelial superior limbal corneal opacities, entropion of the upper eyelids, conjunctival hyperemia an the other canthal area, iris pearls and unexplained unilateral or bilateral 7th nerve histories were presented. The average time interval between the initial presenting complaint the establishment of the correct diagnosis was six years. There is no accurate method of determining the real incidence of leprosy in the United States, but it would appear to be higher than suspected. Ophthalmologists should consider leprosy when examining patients with any of the findings just mentioned, particularly if the patient has a sssociated skin or neurologic findings or nasal stuffiness.(AU).

Resumo:In the Green-Pastures-Leprosy-Hospital in Pikhara/Nepal 340 outpatients with leprosy were examined to evaluate the incidence of lagophthalmus and posterior synechiae of the iris. Patients who had already received chemotherapeutical treatment during the first 5 years following the onset of the disease developed fewer cases of lagophthalmus in both types of leprosy than patients who remained untreated for longer than 5 years. A comparable positive influence of early chemotherapy on the incidence of posterior synechiae of the iris could not be proven. The possible causes were discussed...(AU).

Resumo:A survey of 121 leprosy patients in the Netherlands showed eye involvement in 20% of the patients, rising to 27% if madarosis and the adnexae are included. Iris atrophy was the main lesion encountered in BL and LL patients. One typical lepromatous fundus lesion was seen. No bilateral blindness occurred. Two eyes had a visual acuity of 3/60 due to intractable scleritis and acute iritis in LL with ENL. Seven eyes of 5 patients had a visual acuity of 0.2 to 3/60, mainly due to iris atrophy in lepromatous leprosy. Prevalence of blindness in our series was low as compared with the literature.(AU).

Resumo:In this survey of leprosy patients in Nepal, half the patients examined were found to have some ocular abnormality associated with leprosy. However, many did not have marked ocular signs, and might have been passed as normal in the absence of a more detailed examination. This paper in intended to demonstrate the importance of regular examination of the eyes of every leprosy patient.(AU).

Resumo:A total of 466 leprosy patients in Nepal, some advanced cases, were surveyed for ocular lesions. 74.2% were found with ocular features, and 12.7% of the eyes were blind. The patients were classified in tuberculoid, borderline-borderline, and lepromatous groups. Lepromatous leprosy is responsible for major ocular complications and blindness.(AU).

Resumo:The prevalence of ocular lesions in patients with leprosy attending the Port Moresby General Hospital leprosy clinic was 52%. The prevalence of potentially sight threatening lesions in these patients was 12%. Their mean age was 26.3 years, and the mean estimated duration of disease was 7.2 years.(AU).
