Resumo: The ocular condition 8,235 Malawi leprosy patients surveyed; 6.3 per cent were found to suffer from eye lesions and only 0,3 per cent. were blind. The patients are classified into lepromatous, broderline, and tuberculoid groups. in lepromatous patients eye lesions occur in the later stages of the sidease, and in tuberculoid patients at an earlier stage. Lepromatous patients are affected mainly by iridocyclitis and superficial puntacte keratitis. Tuberculoid patients mainly from lagophthalmos and corneal anaesthesia due to nerve palsy. The lw incidence of ocular manifestations is considered to be due in part to the wide use of sulphones; this is especially true in the lepromatous group, in which only 6 per cent. had eye lesions while under dapsone treatmet. In tuberculois patients under current treatment eye manifestations were present in 4i per cent. Steroids are highly effective in controlling hypersensitivity reactions of the uvea.(AU). Resumo: Clinical and pathological investigations in a group of 113 patients with leprosy of long duration demonstrated the importance of iris changes as a cause of blindness. In lepromatous leprosy the so called chronic iritis produces iris atrophy with small nonreacting pupils which exaggerate the visual impairment created by developing lens changes and corneal opacities. The cause of this chronic iritis is believed to be neuroparalytic from early involvement of the small nerves of the iris, particularly the autonomic supply. Clinical and pharmacological evidence for this theory is supported by the histological changes observed in 18 specimens of iris removed during the course of cataract surgery, with progressive atrophy of the iris preferentially affecting the dilator muscle and leading to a nonreacting miosed pupil. Further pharmacological and histological studies are to be undertaken on lepromatous patients with iris involvement with a view to establishing possible methods of prevention.(AU). Resumo: 1- As manifestaçoes oculares da hanseníase podem ser causa de cegueira em aproximadamente 250.000 indivíduos no mundo. Portanto, é claro que a hanseníase ocular se constitue em importante fonte de cegueira evitável. Há necessidades claras de açoes coordenadas para prevenir a perda visual nos pacientes com hanseníase. 2- A detecçao precoce e tratamento da hanseníase é de grande importancia para previnir futuras complicaçoes e incapacidades devidas à doença, incluindo a perda visual. Programas nacionais de controle da hanseníase e prevençao de cegueira devem, portanto, ter uma atuaçao ativa na identificaçao e tratamento destes pacientes com risco de cegueira. 3- Há uma forte necessidade de alertar os profissionais que trabalham com oftalmologia para hanseníase ocular e suas consequências. Com este objetivo os treinamentos nas escolas médicas em nível de graduaçao e pós graduaçao necessitam ser enfatizados. 4- Recomenda-se que todas as reunioes nacionais e internacionais de hanseníase devem dar particular atençao para a hanseníase ocular. 5- Profissionais de saúde que atuam com a hanseníase e assistentes oftalmológicos devem ter um papel chave na detecçao e conduçao da morbidade ocular(AU).
