Resumo:Numerous clinical and serological similarities exist between lepromatous and dimorphous leprosy and diseases od collagen, particularly lupus erythematosus and rheumatoid arthritis. The clinical manifestations include spontaneous skin-ulcerations, ischaemic necrosis, petechial and purpuric eruptions, vesicle and bulla formation, subcutaneous nodules, hepatosplenomegaly, migratory arthralgia and polyarthritis, bizarre skin lesions including butterfly facial rashes, and a general tendency toward exacerbations and remissions. Haematological findings include anaemia and the ocasional appearance of lupus cells.Positive serological findings include rheumatoid factors, circulating thyroglobulin antibodies, false positive serological tests for syphilis, and cold precipitable proteins (a consistent finding). Because of these similarities, the authors believe that leprosy is a disease which should be considered as a model for the study of hyperglobulinaemic states, especially the diseases of collagen. An important consideration is that, unlike these other diseases, the agent causing leprosy is known.

Resumo:From the point of departure indicated in the introduction of this report, further studies have been carried out on cardiolipin, cholesterol and cephalin in an effort to obtain an antigen composition with high sensitivity with sera of various types of leprosy but low sensitivity with other sera. Ordinary crude cephalin was fractioned by Folch's method into phosphatidyl-ethanolamine, diphospho-inositide, and phosphatidyl-serine, and these were combined in various ratios. In order to obtain the optimal mixture-ratio of cardiolipin, cholesterol and cephalin, two were kept constatnt and the other changed serially in quantity. The optimal ratio of the three cephalin fractions (in the other stated above) for high sensitivity with pooled sera from lepromatous cases was found to be 29:10:1. The process was repeated with this cephalin mixture and varying quantities of cholesterol and cardiolipin. The least reactive amounts of these various substances were selected as optimal amounts, as prevention of the development of nonspecific reactions was desired. The optimal combination proved to be 0.2 cc. of o.2 per cent cardiolipin 0.2 cc. of 1 per cent cholesterol, and 0.4 cc. of 1 per cent cephalin fractions (0.29 cc. of phosphatidyl-ethenolamine, 0.1 cc. of diphosphoinositide, and 0.01 cc. of phosphatidyl-serine), plus 0.2 cc. of absolute alcohol. The cardiolipin: cholesterol: cephalin ratio of this antigen is 1:5:10...(AU).

Resumo:Se exponen los diferentes tests serológicos en la lepra, desde la antigua reacción de Rubino hasta los recientes del FLA-ABS, RIA, PHA, ELISA, GLP-1, MLPA, SACT y PCR. Se comparan los resultados de ellos que son positivos práticamente en el 100 por ciento de multibacilares, y sólo en un 50 por ciento de los paucibacilares, aunque el FLA-ABS da más positividad, siendo su mayor indicación en la detección de las formas subclínicas. En este aspecto y en el de predecir la aparición de la enfermedad no parecen ser extremamente útiles. Se considera que el PCR, el último test en estudio, podría ser la solución para la detección precoz de la enfermedad. (AU).
