Abstract:Preparou-se uma lepromina do tipo bacilar, a uma concentração de 2%/oo, com bacilos de Hansen obtidos do baço e f¡gado do cadaver de um doente lepromatoso. Os estudos feitos nos pacientes lepromatosos, tuberculóides e indiferenciados, demonstraram que esta lepromina provoca reações precoces e tardias que coincidem com as reações clássicas que se observam com a lepromina bacilar preparada com nódulos cutâneos. Esta fonte de germes permitir  preparar lepromina em quentidade muito maior do que normalmente se conseguia e torna possível um melhor abastecimento deste produto aos serviços regionais de controle de doentes, comunicantes e suspeitos, em uma campanha antileprosa que atinja amplas zonas endêmicas. Abstract:The intradermal reaction to lepromin has been studied in a total of 563 persons, cases of leprosy and contacts. Observation of its clinical and histopathologic development has shown the following facts:a- In allergic cases intradermal injection of lepromin induces an "early" reaction which is well defined, both clinically and histologically, beginning a few hours after injection and reaching its maximum between 48 and 72 hours, thereafter decreasing until it disappears completely during the first week. b- This early reaction, which is manifested clinically by an erythematous halo, usually infiltrated, is not of nonspecific character since it has the histologic structure common to allergic reactions and is only seen in those cases which offer resistance to the infection (i.e., neural forms of the disease, particularly the tuberculoid form) and does not occur in cases which are considered anergic, as in the lepromatous type...AU.

Abstract:Em três grupos diferentes de contatos de pacientes de Hanseníase foram observados resultados da intradermorreaçäo ao Antígeno Solúvel (AS). Em área urbana endêmica para hanseníase (taxa de prevalência de 2,87 por 1.000 habitantes); foram estudados três grupos: 1) Contatos intradomiciliares ou conviventes com um caso por mais de um ano. 2) Contatos de vizinhança ou extradomiciliares e 3) Habitantes nas áreas distantes do foco. Trinta e nove por cento das 1569 intradermorreaçöes foram positivas. Demonstrou-se uma associaçäo entre a positividade da intradermorreaçäo e proximidade de fonte de infecçäo, com a idade e o tempo de residencia no mesmo domicílio. Considerou-se o AS um bom instrumento para a identificaçäo de grupos de riscos para a hanseníase, embora os achados näo quantifiquem seu valor preditivo no diagnóstico individual da infecçäo.(AU).

Abstract:Testes cutaneos sao utilizados para o acompanhamento de casos de hanseniase, pois constituem um dos parametros para a classificaçao de forma clinica da doença, prognostico e mesmo para indicaçao e acompanhamento do tratamento. Estes testes foram desenvolvidos no inicio do seculo e utilizam suspensoes de bacilos extraidos de lepromas humanos ou de tatus infectados. Neste artigo, revemos as caracteristicas do teste da lepromina ou de Mitsuda e dos testes alternativos que foram desenvolvidos. Discutimos tambem o potencial da utilizaçao de antigenos recombinantes como uma possibilidade futura para obtençao de testes cutaneos mais especificos e mais padronizaveis em relaçao aqueles obitidos a partir de seres humanos ou de tatus.(AU).

Abstract:Neste estudo propos-se comparar o teste intradermico de Mitsuda e os alelos HLA-DR2/HLA-DR3 e HLA-DQ1 relacionados com as formas clinicas da hanseniase, visando contribuir para o delineamento de nova metodologia no auxilio prognostico desta doenca. Foram estudados 176 pacientes hansenianos (50 HT, 50 HV e 76 HD). A tipificacao do HLA-DR e HLA-DQ foi determinada pela tecnica de PCR/SSP e reacao de Mitsuda pela intradermorreacao com leitura em 28 dias. Na forma HT os resultados demonstraram que a reacao de Mitsuda foi positiva em todos os pacientes, sendo 16 por cento positivo1+ 68 por cento positivo2+ e 16 por cento positivo3+. O diametro de induracao variou entre 4,0 a 18,0mm, com valor medio de 7,5mm. A especificidade HLA-DR2 esteve presente em 28 por cento dos pacientes deste grupo (24 por cento HLA-DRB1*15 e 4 porcento HLA-DRB1*16). A especificidade HLA-DR3 esteve presente 16 por cento dos pacientes (8 por cento HLA-DRB1*17 e 8 por cento HLA-DRB1*18), 2 por cento apresentaram os alelos HLA-DR2 e HLA-DR3 simultaneamente, totalizando 46 por cento da amostra estudada com o marcador. A forma HV apresentou a reacao de Mitsuda negativa em todos os pacientes estudados. A especificidade do HLA-DQ1 esteve presente em 74 por cento dos pacientes (34 por cento HLA-DQB1*05, 28 por cento HLA-DQB1*06 e 12 por cento HLA-DQB1*05 e HLA-DQB1*06). No grupo HD, o padrao de leitura da reacao de Mitsuda resultou em negativo em 48,7 por cento duvidoso 5,2 por cento positivo 1+ 30,3 por cento e positivo 2+ 15,8 por cento. O diametro da iduracao variou entre negativo a 7,5mm com valor medio de 2,4mm...(AU).

Abstract:1. A study been made of the progress of the disease in relation to the results of the lepromin test, in 160 cases of leprosy (109 neural, 46 lepromatous, and 5 of doubtful classification) first tested six years ago. 2. the prognostic value of the test is best illustrated in cases of the neural type. Of the 109 cases of the neural type lepromin test was positive group, improvement was seen in about 84 per cent of the cases. In the disease becoming worse in only 14 per centin the lepromin-negative and doubtful group improvement was seenonly in 53 per cent of the cases the disease becoming worse in 47 per cent. In only one case has the disease changed from the neural to the lepromatous type, and this occurred in a lepromin-negative case. The degree of positivity of the test also influence the prognossis of the 92 lepromin-positive neural cases 20 had strongly positive 49 moderately positive and 23 weak positive reactions (AU).

Abstract:The present system of reading the leprolim test was established arbitrarily, and the findings have been related to the different forms of leprosy, the immunological characteristics of which are still uncertain. From this combination of unknown factors much confusion may arise. For example, of 194 nodular cases 74 gave "positive" reactions that could evidently have no immunological or prognostic significance. Studying the evolution of the reaction in two very distinct groups of cases, nodular and tuberculoid, with undoubtedly opposite conditions of immunity, the author points out the essential distinctive features of the reactions typical of these forms of the disease. In the nodular form the test is negative: there is no reaction, or only a small papular one, due to nonspecific factors, which reaches its maximum before the fourth or fifth day, seldom is larger than 5 mm., and may persist until the fourth week. In the tuberculoid form the true positive reaction occurs: a nodular lesion, often suppurating, usually belated in appearing, of progressive evolution, generally reaching its maximum from the second to the sixth week, seldom less than 5 mm. in diameter in the fourth week. The dimensional criterion of positivity is not sufficient, and the test must be performed by trained observers familiar with the clinical signs. Even so, it is necessary to admit a large group of doubtful reactions, of indefinite aspect and borderline dimensions (4-6 mm. diameter). Recognition of doubtful groups permits not only avoiding error of reporting reactions, but also avoids possible faults due to different antigenic values of different lots of lepromin, the standardization of which still presents a problems. Abstract:A study has been made of the variations in the results of the lepromin test in cases the neural type, demonstrated by repeated testing of the same cases in different circumstances There apper to be at least five possible factores which, acting singly or in combination, may influence the result of the test. These different factors are: variations in clinical activity, variations in bacteriological findings, variations in the time year at which the test is done, deterioration of the lepromin on keeping, and the use of different lots of lepromin Our stdy has shown that the two most important factors are the variations in clinical activity and tima of the yaer at which the test is done.
