Title:High oral prevalence of Candida krusei in leprosy patients in northern Thailand ..-

Abstract:Although Candida albicans is the most common human yeast pathogen, other Candida species such as C. krusei are now recognized as emerging agents, especially in patients with human immunodeficiency virus (HIV) disease. C. krusei is inherently resistant to the widely used triazole antifungal fluconazole and poses therapeutic problems, especially in systemic candidiasis. In a surveillance study of leprosy patients (with arrested or burnt-out disease) in a leprosarium in northern Thailand, we found a rate of oral carriage of C. krusei (36 per cent) significantly (P smaller 0.05) higher than that for a healthy control group (10 per cent). Among the Candida-positive patients, 16 of 35 (46 per cent) carried C. krusei, while C. albicans was the second most common isolate (12 of 35 patients; 34 per cent). The corresponding figures for the control group were 2 of 13 (15 per cent) and 6 of 13 (46 per cent), respectively. Studies of the antifungal resistance of the C. krusei isolates from patients indicated that all except one of the isolates were resistant to fluconazole, two isolates were resistant to ketoconazole, and all isolates were sensitive to amphotericin B. Evaluation of their genetic profiles by randomly amplified polymorphic DNA analysis with three different primers and subsequent analysis of the gel profiles by computerized cluster-derived dendrograms revealed that the C. krusei isolates from patients belonged to 10 disparate clusters, despite the origin from a single locale. These nascent findings indicate an alarmingly high prevalence of a Candida species resistant to a widely used antifungal in a part of the world where HIV disease is endemic. (AU).

Abstract:In recent years leprosy has been increasingly imported into Germany. An Indian leper showed the following features in our speciality: 1. Partly granulomatous and partly ulcerative inflammatory processus of nasal mucosa with adhesions between the tubirnate and the septum; 2. leproma in the nasal vestibule; 3. diffuse lepromas of the pinnae; 4. radiologically proved atrophy of the middle part of the alveolar process of the maxila with loose incisors (Moller-Christensen syndrome); 5. loss of the outer parts of the eyebrows (lucio signs); 6. visible and palpable firm swelling of the great auricular nerve on the right side on the neck.(AU).

Abstract:Sixty-two patients with lepromatous leprosy were examined for changes in the uvula and the soft palate. Ten patients had definite gross pathologic changes in this area. Loss of the uvula and part of the soft palate due to granulomatous nfiltration, leproma formation, ulceration, and fibrosis were found. Absense of these structures was observed in a very marked degree only in patients in whim lepromatous leprosy had been present for many years or who had not undergone any kind of aspecific treatment. The histologic and functional features, such as diddiculty in swallowing, regurgitation, and rhinolalia aperta, with their possible consequences are described.(AU).

Abstract:Thirty patients with lepromatous leprosy of long duration were examined for gingival and periodontal changes. All patients presented with chronic gingivitis and periodontitis of an unspecific type. Etiologic factors affecting the development of gingival and periodontal changes in this disease are: lack of oral hygiene with severe plaque and calculus formation due to the inability of the patients to perform oral hygiene, mouth breathing, specific granulomatous infiltrations, and a possible specific drug action. Specific changes in form of 'facies leprosa' are not pathognomonic of lepromatous leprosy but may be found in 20% of the cases among Thai and Chinese patients. Occurrence and pathogenesis of 'facies leprosa' is described.(AU).

Abstract:Em trinta pacientes hansenianos, 15 pertencentes à variedade tuberculóide reacional do tipo tuberculóide e 15 ao grupo dimorfo, foram realizados exame estomatológico e biópsia da mucosa do palato com o propósito de se avaliar a incidência de lesoes bucais específicas nessas duas formas anátomo-clínica da moléstia. Oito pacientes do grupo dimorfo e 5 da variedade tuberculóide reacional apresentaram lesoes bucais, caracterizadas por granulomas com baciloscopia negativa ou reaçoes inflamatórias nao específicas com baciloscopia positiva. As manifestaçoes clínicas destas lesoes foram discretas, representadas por eritema, pápulas, mais raramente por placas e, em apenas un caso, uma pequena lesao ulcerada. A baciloscopia no palato foi, em geral, discreta e em apenas dois casos dimorfos atingiu +++ (três cruzes). A confrontaçao destes resultados com os dados colhidos na revista de literatura permitiram inferir algumas conclusoes, ou sejam: 1. O comprometimento da mucosa bucal em pacientes tuberculóides reacionais e dimorfos sugere a existência de disseminação hematogênica, a partir das lesões cutâneas, nas fases mais ativas destas duas manifestações da hanseníase. 2. O maior comprometimento bucal no grupo dimorfo, e neste grupo de pacientes em surtos de pseudo-exacerbação, sugere que as lesões mucosas e viscerais se tornem mais freguentes à medida que se encaminha no espectro em direção ao polo virchoviano. 3. A baciloscopia na mucosa bucal é sempre igual ou inferior a baciloscopia cutânea do mesmo doente, o que sugere que esta localização não apresenta condições biológicas ideais para o desenvolvimento bacilar. Este fato, aliado à pequena extensão dos infiltrados específicos e à integridade da mucosa na imensa maioria dos casos, sugere que nos pacientes tuberculóides reacionais e dimorfos a mucosa oral não constitui fonte importante de eliminação bacilar.(AU).

Two kinds of lesions found: active and residual. Specific active lesions were described as maculas, inflitrations, papules, nodules and plaques, which were most frequently found, in decreasing order in uvula, soft and hard palate, and lips. Lesions presenting positive bacilloscopic, even with as apparenthy normal mucosa, were always considered as active. Residual lesions were destruction, atrophy and retraction of uvula, scars and nasopalatine perforation, more frequent in those patients with longer duration of the disease. According to bacilloscopy, oral lesions involute simultaneously with skin lesions. for this study, clinical, bacilloscopic and histopathological examinations were made, besides photographs of the most suggestive cases(AU).
