Title:Epithelioid granuloma in the iris of a lepromatous leprosy patient; an unusual finding.

Abstract:This case report depicts a case of histopathologically confirmed polar lepromatous (LL) leprosy with a bacterial index of 4+. He experienced recurrent episodes of erythema nodosum leprosum (ENL) in the first 5 years after diagnosis. Skin smears became negative after 6 years of dapsone monotherapy and have remained negative since that time. At 23 years after diagnosis, the patient had developed cataracts and underwent intracapsular cataract extractions with broad-based iridectomies. In one of the iris specimens, histopathologic examination revealed a focal granuloma composed of epithelioid cells. Subsequently a lepromin skin test showed a positive Mitsuda reaction with a borderline tuberculoid histopathology. This clearly illustrates the immunological upgrading of a polar lepromatous patient, perceived first in the iris tissue. (AU)^ien.

Abstract:To detect the presence of Mieschre's granuloma in erythema nodosum leprosum, fifty eight specimens biopsied from recent lesions were carefully examined, but in none of them is was found. The author points ou that, after the early positive reports of Portugal and Orbaneja, workers have repeatedly failed to observe the so-called radiated granulomas in erythema nodosum type of leprotic reaction. On the other hand, a review of the literature indicates that there is not yet, even in classical erythema nodosum, a general agreement regarding frequency, histogenesis and meaning of these estructures. The author concludes, therefore, that the ocasional histological findings, quoted above must not be over emphasized as an evidence of the identity between these two conditions. (AU)^ien.

Abstract:El armadillo de 7-bandas, Dasypus hybridus, ha sido utilizado en estudios de lepra experimental en un reducido numero de animales. Con el proposito de conocer la susceptibilidad de esta especie frente al Mycobacterium leprae y de llevar a cabo estudios patologicos con microscopia de luz y microscopia electronica se inoculo cinco armadillos con germenes provenientes de lesiones lepromatosas de dos enfermos sin tratamiento. Se encontro lesiones lepromatosas disemidas en uno solo de los cinco armadillos inoculados las que morfologicamente son similares a las previamente descritas por otros autores en esta misma especie, en D. novemcinctus y D. sabanicola. Se describe aspectos ultraestructurales del granuloma lepromatoso y del M. leprae. Se describe tambiem lesiones degenerativas de fibras miocardicas y de fibras musculares esqueleticas en el armadillo que presento lesiones lepromatosas, asi como en los cuatro armadillos en los que esta no se desarrollaron y se discute la natureza de estas lesiones y de otras de tipo granulomatoso inespecifico en la serosa pleural y peritoneal (AU).

Abstract:We comparaed the granuloma morphology and immune response of hamsters inoculated with Paracoccidioides brasiliensis (Pb) into the cheek pouch, which lacks lymphatic drainage, and the ffotpad, which is rich in lymphatics. Our objective was to better understand the modulation of Pb granuloma in an immunocompetent animal inoculated in an immunologically privileged site. The humoral immune response (ELISA) and cell mediated immunity (footpad test) became positive on days 7 and 14, respectively in animal inoculated into footpad and on days 35 and 60 in animals inoculated into the pouch. Typical epithelioid granulomas were observed at both sites on day 14. The number of fungi gradually decreased from the beginning of the experiment in footpad lesions, but only after day 35 in pouch granulomas, when cell mediated immunity was detectable. The results indicate that typical epithelioid paracoccidioidomycotic granulomas may develop in the absence of a detectable immune response; however, they are incapable of controlling fungal reproduction. Lack of lymphatic drainage delays the appearance of a detectable immune response, but with time fungi escape from the pouch, elicit an immune response and reach other organs. Our results further indicate the importance of the lymphatics in the pathogenesis of paracoccidioidomycosis. (AU).

Abstract:Coccidioidomycosis occurs in arid and semi-arid regions of the New World from the western United States to Argentina. Highly endemic areas are present in the southwest United States. Coccidioides species live in the soil and produce pulmonary infection via airborne arthroconidia. The skin may be involved by dissemination of the infection, or by reactive eruptions, such as a generalized exanthem or erythema nodosum. Interstitial granulomatous dermatitis and Sweet's syndrome have recently been recognized as additional reactive signs of the infection. Coccidioidomycosis is a [quot ]great imitator[quot ] with protean manifestations. Cutaneous findings may be helpful clues in the diagnosis of this increasingly important disease.(AU).

Abstract:A utilizaçäo da bolsa jugal do hamster, foi avaliada pela inoculaçäao de 6,0x10.8 M. leprae/ml no tecido subepitelial de 60 animais, empregando como grupo controle, 12 hamsters inoculados no coxim plantar. Os animais foram sacrificados em 20 e 48 horas e aos 7, 14, 21 e 28 dias pi. A evoluçäo da lesäo de inoculaçäo foi analisada pelo exame histológico em cortes corados pela hematoxilina-eosina e faraco-fite. A avaliaçäo da viabilidade bacilar na bolsa jugal do hamster foi realizada 7, 14, 21 e 28 dias pi pelo teste de recuperaçäo bacilar em camundongos. Os resultados nos permitiram concluir que a resposta inflamatória ao M. leprae na bolsa jugal evoluiu para formaçäo de granulomas macrofágicos semelhantes aos da hanseníase virchoviana humana porém, o teste de recupraçäo bacilar sugeriu que näo houve multiplicaçäo dos bacilos. As lesões do coxim plantar evoluiram para granulomas epiteliódes semelhantes aos da hanseníase dimorfa. (AU) .

Abstract:Com base em um novo conceito morfológico e classificaçäo de inflamaçäo granulomatosa: os granulomas polares, na histogênese do granuloma tuberculóide de teste Mitsuda positivo e na relaçäo entre o grau de diferenciaçäo histogenética e comportamento de tumores, os autores concluiram que o comportamento "benign" ou "maligno" da hanseníase depende do grau de diferenciaçäo tuberculóide das lesöes. Se a lesäo é histologicamente bem diferenciada em direçäo a um granuloma polar tuberculóide (hanseníase tuberculóide), ela terá um comportamento "benigno". Ao contrário, se esta diferenciaçäo for ausente (hanseníase virchoviana) ou pobre (hanseníase interpolar), o comportamento da lesäo será "maligno". (AU).
