Title:Lot quality assurance sampling (LQAS) for monitoring a leprosy elimination Program.

Abstract:In a statistical sense, prevalences of leprosy in different geographical areas can be called very low or rare. Conventional survey methods to monitor leprosy control programs, therefore, need large sample sizes, are expensive, and are time-consuming. Further, with the lowering of prevalence to the near-desired target level, 1 case per 10,000 population at national or subnational levels, the program administrator's concern will be shifted to smaller areas, e.g., districts, for assessment and, if needed, for necessary interventions. In this paper, Lot Quality Assurance Sampling (LQAS), a quality control tool in industry, is proposed to identify districts/regions having a prevalence of leprosy at or above a certain target level, e.g., 1 in 10,000. This technique can also be considered for identifying districts/regions at or below the target level of 1 per 10,000, i.e., areas where the elimination level is attained. For simulating various situations and strategies, a hypothetical computerized population of 10 million persons was created. This population mimics the actual population in terms of the empirical information on rural/urban distributions and the distribution of households by size for the state of Tamil Nadu, India. Various levels with respect to leprosy prevalence are created using this population. The distribution of the number of cases in the population was expected to follow the Poisson process, and this was also confirmed by examination. Sample sizes and corresponding critical values were computed using Poisson approximation. Initially, villages/towns are selected from the population and from each selected village/town households are selected using systematic sampling. Households instead of individuals are used as sampling units. This sampling procedure was simulated 1000 times in the computer from the base population. The results in four different prevalence situations meet the required limits of Type I error of 5% and 90% Power. It is concluded that after validation under field conditions, this method can be considered for a rapid assessment of the leprosy situation. (AU)^ien.

Title:Dermal extracellular matrix in cutaneous leprosy lesions.

Abstract:Thirty-eight biopsies of cutaneous lesions from leprosy patients [borderline tuberculoid (BT) 14, borderline lepromatous (BL) 18, lepromatous (LL) 6] were processed for staining of some extracellular matrix (ECM) components (collagen, proteoglycans, elastic fibers and fibronectin). Specific histological staining and the indirect immunofluorescence method with antibodies to collagen and fibronectin were utilized. The ECM of the normal dermis was strikingly modified in the inflammatory infiltrate. By Gomori's reticulin and anti-fibronectin immunostaining, replacement of the dense interlaced collagen fibers with a reticular mesh was observed in the infiltrate. The immunoreactivity obtained with anti-type I and anti-type III collagens showed positive fibrils and a lumpy pattern in the lepromatous and tuberculoid lesions with a higher amount in the lepromatous lesions. The lack of clear-cut boundaries between the normal dermis and the inflammatory infiltrate in the lepromatous (BL, LL) lesions was correlated with the blurred limits of the clinical lesions of this pole of the leprosy spectrum. Absence of elastic fibers in the infiltrate was a constant finding, and fuchsin-positive microfibrils were found in some infiltrates. The clear zone of lepromatous lesions was devoid of oxytalan fibers. Elaunin fiber rings around sweat gland acini were present even when the leprosy infiltrate was seen enveloping them. The original ECM is replaced by a newly assembled one, which is suited for the dynamic nature of the inflammatory process. The trophic effects of the ECM upon the cutaneous epithelial structures are modified so that atrophy and late degeneration ensues. These ECM modifications contribute, therefore, to the biological alterations of the skin functions in leprosy. (AU)^ien.

Abstract:A hanseníase é uma doença infecto-contagiosa de evolução crônica que, em decorrência do acometimento do sistema nervoso periférico, ocasiona perda de sensibilidade e paralisias musculares que se não diagnosticadas e tratadas adequadamente podem evoluir para incapacidades físicas permanentes. O presente estudo visa avaliar a implicação dessas incapacidades na realização de atividades diárias de indivíduos afetados por essa doença por meio da aplicação da Escala SALSA e da Avaliação do Grau de Incapacidade. Foram entrevistados 70 pacientes afetados pela hanseníase que frequentam o Ambulatório de Dermatologia do Instituto Lauro de Souza Lima - Bauru, SP encaminhados para o setor de Terapia Ocupacional desse Instituto. Os resultados obtidos demonstraram que a hanseníase interfere na capacidade de realização de atividades diárias dos indivíduos acometidos, independentes do grau de incapacidades que apresentam. Abstract:Para descrever a satisfação de indivíduos atingidos pela hanseníase com neurite grave, submetidos à neurolise de troncos periféricos, 70 indivíduos foram entrevistados no Estado de Rondônia. Foram utilizados questionários estruturados com perguntas fechadas para avaliar os aspectos sócio-demográficos e clínicos. Além disso, foi avaliada a opinião e a satisfação em relação à neurolise, utilizando-se perguntas abertas. Quarenta e cinco (64,3%) eram do sexo masculino (idade média: 43 anos; variação: 18-81 anos). A maioria dos indivíduos (77%) apresentava a forma dimorfa da doença. Sessenta (85%) estavam satisfeitos com o resultado da cirurgia. Não houve nenhuma diferença considerável relativa à satisfação, quando estratificada segundo tronco nervoso operado. Os dados oriundos das perguntas abertas foram categorizados segundo temas: dor, perda sensitiva, déficit motor, dificuldades para realizar atividades diárias, trabalho e lazer. A opinião mais importante foi alívio da dor e possibilidade de realização de atividades diárias novamente. A aparência estética da cicatriz foi mencionada como um aspecto negativo por um indivíduo. Os dados evidenciam que a descompressão cirúrgica foi vista de modo positivo pelos indivíduos atingidos pela hanseníase, mas que há aspectos psico-sociais que precisam de atenção. A satisfação dos pacientes deve ser levada em consideração, como também a avaliação objetiva da dor e o déficit sensitivo e motor após neurolise. (AU)^ipt.

Title:Immuno-histopathology in the diagnosis of early leprosy.

Abstract:The present study of 45 early leprosy cases in an endemic area in China indicates: a) Sensitivity of acid-fast bacilli (AFB) detection can be significantly improved by examining approximately 30 serial sections. AFB and/or phenolic glycolipid-I (PGL-I) were mostly detected in the infiltrates in the subepidermal zone, intraneurium, perineurium and around blood vessels. b) PGL-I antigen was positive in 10 clinically suspected, single lesion leprosy cases and AFB positive in 7 patients, AFB and/or PGL-I in nerve in 6 patients. c) Nonspecific chronic inflammation in indeterminate leprosy presented as selective perineural and/or intraneural infiltration with lymphocytes predominating. In the infiltrating mass, fragments of neural tissue were demonstrated with anti-S-100 protein staining. d) Except for 3 cases with unknown numbers of lesions, the present positive immunohistopathological findings are in direct correlation with the number of lesions at first diagnosis, namely: 41.6% (10/24) for single lesion, 66.6% (6/9) for 2 lesions, and 88.8% (8/9) for patients with > or = 3 lesions. e) Typical epithelioid or macrophage granuloma formations were not seen in early leprosy with a single lesion. In testing the immunological inclination of these patients with CD68 or tumor necrosis factor-alpha (TNF-alpha) a positive test is likely to be of prognostic value since TNF-alpha is involved in granuloma formation and nerve damage.
