Resumo: Secondary amyloidosis is the leading cause of death in lepromatous leprosy in this country. The mortality is directly caused by amyloid involvement of the kidneys, which occurred in 100% of this series. The diagnosis of this complication depends largely on the development of a persistent proteinuria, Congo red absorption, hepatosplenomegaly and, to a lesser degree, altered serum proteins and perhaps liver biopsy in selected cases. the correlation between the Congo red test and the degree of amyloidosis is poor, and unless this test shows 80 to 100% dye absorption in one hour the diagnosis cannot be made from this finding alone. Serum protein and lipoid changes occur in lepromatous leprosy per se, and in a study of the various protein fractions no consistent alteration occurred in cases complicated by amyloidosis. The average duration of life after the onset of proteinuria in amyloidosis was 38.33 months, with a fairly wide range above and below this figure. The development of anemia and a progressive elevation of the nonprotein nitrogen are unfavorable prognostic signs. Resumo: Some of the possible factors involved in the genesis of amyloidosis during the course of lepromatous leprosy have been studied. In a group of 101 patients from the U. S. Public Health Service Hospital at Carville, La., it is estimated that 40 to 50% have concomitant amyloidosis. This high proportion has been previously wll-documented by autopsy studies (2,3) and was suggested by a 31% positive result in 69 gingival biopsies among this group. Congo red results were often positive, particularly when accompanied by persistent albuminuria. The high percentage of amyloidosis at Carville lies in distict contrast to that of 119 patients studied at Guadalajara, Mexico, where amyloid was diagnosed in only 6%. Factors which may play some role in this different include discrepancies in animal fat consumption between the two population groups. Patients at Carville eat fewer calories but twice the percentage of saturated fat in their diet as compared to the Mexican group studied...AU. Resumo: Fourteen hundred and forty five biopsied tissues of various organs including liver, skeletal muscle, kidney, lymphnode, larynx and skin from 1222 patients of leprosy obtained during the last 10 years have been examined histopathologically to study various types of pathological lesions. These patients ranged in age from 7 years to 72 years and the duration of illness varied from less than one year to twenty years. Although the presence of lepromatous granuloma, AFB in varying proportions, and some nonspecific lesions have been observed in them, but amyloidosis was not detected in any of them. This finding is discussed in view of the observations of other authors of different parts of the world in this connection. Consumption of mainly vegetarian diet in our population and that of meat in Western population has been suggested to be the probable cause of the difference of amyloidosis observed in the two groups of people.(AU). Resumo: Sixty consecutive patients with leprosy were investigated for renal involvement. Clinically overt renal disease was present in 4 patients; 3 presented with a nephrotic state and one patient with progressive renal failure. Urinalysis showed daily protein loss ranging from 0.4 to 8.9 g in 8 patients and microscopic haematuria in 4 cases. Elevated levels of blood urea and creatinine were seen only in one patient with diffuse proliferative glomerulonephritis. Of the 36 patients in whom distal tubular functions were evaluated, concentration and/or acidification defects were detected in 9 patients (25%). Renal histology revealed no abnormality in any of these patients. Serum C3 levels were decreased in 5 patients with lepromatous leprosy and 3 patients with borderline leprosy. Histological evidence of renal involvement was detected in 9 patients (15%). Amyloid deposits were seen in 3 (5%) patients of whom 2 had lepromatous leprosy and one had tuberculoid leprosy with chronic trophic ulcers. Mesangial proliferative lesions were seen in 5 (8.3%) and diffuse proliferative lesions (with crescents in more than 70% of glomeruli) in one patient. All of them had lepromatous leprosy. Three of the 5 patients with mesangial proliferative glomerulonephritis had erythema nodosum leprosum at the time of biopsy. Immunofluorescence studies revealed granular deposits of IgA, IgM and C3 in one patient with mesangial proliferation and IgA/IgM with or without C3 in 3 more patients in whom renal histology was normal. Glomerulonephritis associated with leprosy appears to be immune mediated but confirmation requires identification of lepra antigen in the glomerular immune complex deposits.(AU). Resumo: Rectal biopsy in 190 inpatients at a leprosy hospital in the Highlands of Papua New Guinea disclosed 16 patients with secondary amyloidosis. This represented 20% of the patients who had had polar lepromatous leprosy (L.L.) for more than 2 years. Patients with amyloidosis characteristically had either a history of recurrent attacks of erythema nodosum leprosum (E.NH) reactions or chronic trophic ulcers. Levels of the serum component (protein SAA) antigenically related to the amyloid fibril protein AA were monitored, at varying intervals for three months, in lepromatous patients with E.N.L. reaction. The SAA levels rose during E.N.L. reactions in parallel with the neutrophil count. SAA occurred with greatest frequency among patients with LH, while most non-lepromatous patients with detectable SAA had chronic trophic ulcers. The correlation between raised neutrophil count and elevated SAA concentration, observed in this and other studies, suggests that neutrophils are associated with the production of SAA.(AU).
