Resumo:Glomerular disease in the tropics differs considerably from that in temperate climates and between the developed and the developing world. The prevalence and pattern of glomerular disease in the tropics in turn varies widely in different geographic regions and is influenced by environmental, nutritional, and socioeconomic factors. Among the infective agents that have been identified as contributing to an increased prevalence of glomerular disease in the tropics are Plasmodium malaria, Schistsomiasis mansoni, Mycobacterium leprosy, and hepatitis B virus (HBV). A decline in the prevalence can only be expected with the eradication of infections. Accurate knowledge of the prevalence, histologic features, and causes is essential to achieve prevention and management of glomerular diseases in the tropics. (AU).

Resumo:Membranoproliferative glomerulonephritis (MPGN) is a common form of glomerulonephritis and frequently is associated with chronic infections. Leprosy, one of the most common infections worldwide, was found in conjunction with MPGN, type I, in a patient. Serological abnormalities typical of MPGN, improvement in renal function with therapy of acute complications of leprosy, and long-term renal improvement with antileprosy therapy all occurred in this patient. Others have found that MPGN is found in 11% to 43% of leprosy patients undergoing renal biopsy. Serological abnormalities typical of MPGN frequently are found in patients with lepromatous leprosy. The associations of MPGN and leprosy, and the susceptibility of the glomerulonephritis to therapy, should be emphasized.(AU).

Resumo:A renal biopsy from a patient with lepromatous leprosy but no history of erythrema nodosum leprosum (ENL) showed histologic, immunofluorescent, and ultrastructural features typical of immune-complex glomerulonephritis. A literature review found reports of 187 renal biopsies from leprosy patients, with glomerulonephritis in 31% of the cases, and a variety of other renal lesions. This accumulated evidence demonstrates that, contrary to widely held opinion, there is no association between glomerulonephritis and a history of ENL. Furthermore, it suggests that glomerulonephritis in leprosy patients has a similar incidence in lepromatous and non-lepromatous cases. These observations have an important bearing on concepts regarding the pathogenesis of immune complex glomerulonephritis in leprosy and the humoral immune mechanisms in patients with leprosy.(AU).

Título:Renal lesions in leprosy amongst north Indian patients

Resumo:Sixty consecutive patients with leprosy were investigated for renal involvement. Clinically overt renal disease was present in 4 patients; 3 presented with a nephrotic state and one patient with progressive renal failure. Urinalysis showed daily protein loss ranging from 0.4 to 8.9 g in 8 patients and microscopic haematuria in 4 cases. Elevated levels of blood urea and creatinine were seen only in one patient with diffuse proliferative glomerulonephritis. Of the 36 patients in whom distal tubular functions were evaluated, concentration and/or acidification defects were detected in 9 patients (25%). Renal histology revealed no abnormality in any of these patients. Serum C3 levels were decreased in 5 patients with lepromatous leprosy and 3 patients with borderline leprosy. Histological evidence of renal involvement was detected in 9 patients (15%). Amyloid deposits were seen in 3 (5%) patients of whom 2 had lepromatous leprosy and one had tuberculoid leprosy with chronic trophic ulcers. Mesangial proliferative lesions were seen in 5 (8.3%) and diffuse proliferative lesions (with crescents in more than 70% of glomeruli) in one patient. All of them had lepromatous leprosy. Three of the 5 patients with mesangial proliferative glomerulonephritis had erythema nodosum leprosum at the time of biopsy. Immunofluorescence studies revealed granular deposits of IgA, IgM and C3 in one patient with mesangial proliferation and IgA/IgM with or without C3 in 3 more patients in whom renal histology was normal. Glomerulonephritis associated with leprosy appears to be immune mediated but confirmation requires identification of lepra antigen in the glomerular immune complex deposits.(AU).

Resumo:The presence of subepithelial humps seen in the glomeruli on electron microscopy in a case of lepromatous leprosy with erythema nodosum leprosum is described. Negative cultures and normal antibody titers for streptococci suggest a nonstreptococcal etiology for this finding. Futher studies in this immune complex nephritis are required, to establish its relationship to lepromatous leprosy.(AU).

Resumo:Immunofluorescent banding of the dermal-epidermal junction of skin was demonstrated in 3 of 7 patients with lepromatous leprosy by direct immunofluorescence microscopy. The banding was caused specifically by deposition of IgM. Within the glomeruli of one patient, dense, amorphous deposits in subendothelial and intramembranous position were also demonstrated by electron microscopy. These preliminary findings suggest that lepromatous leprosy may be associated with immunologic disurbances of both skin and glomerular basement membranes.(AU).

Resumo:Twenty one patients of lepromatous leprosy were investigated for evidence of renal diseases. Histological lesions were observed in 76 percent cases. Sixty two percent of the cases showed a proliferative type of glomerulonephritis. Amyloidosis was present in 14 percent. The remaining 24 percent had either non-specific changes confined to renal tubules or were normal.(AU).

Resumo:Electron microscopic examination of renal biopsies from 19 patients with leprosy who had edema, proteinuria, or hematuria showed a proliferative glomerulonephritis in 12, amyloidosis in 2, and no lesion in 5. The proliferative glomerulonephritis was of different patterns: diffuse with or without exudation, focal, or mesangial. Subendothelial and/or subepithelial deposits were seen in five biopsies. Of the patients with glomerulonephritis, 3 had a reduced total serum complement level, 5 had erythema nodosum leprosum, 5 had evidence of recent streptococcal infection, and 2 had microfilariae in the peripheral blood. The significance of these findings is discussed.(AU).

Resumo:Although Mycobacterium leprae does not invade the reanl parenchyma, there are many reports of nephritis in patients leprosy. Glomerulonephritis has been observed in autopsy studies, but the relationship to leprosy has never been clearly estabilished. In the present study, we found urinary abnormalities consistent with glomerulonephritis in 11 of 636 leprosarium inhabitants; there was a strong correlation with a history aof erythema nodosum leprosum (ENL). Seven of 8 noninstitutionalizes patients with active ENL aldo had urinary findings suggestive of glomerunephritis. In 2 patients with ENL, 1 of whom had alterations of serum complement suggestive of complement consumption, renal biopsies revealed proliferative glomerulonephritis. ENL has been considered to reflect, in part, the deposition of circulating immune complexes throught the body. These studies suggest that patients with lepromatous leprosy may expreience glomerulonephritis as one manifestation of ENL, and are consistent with recent observations of immne complex deposition in the glomerular capillary walls of the kidneys of patients with leprosy.(AU).

Resumo:A autora faz a introducäo do trabalho uma abordagem sobre o tabagismo como importante fator associado a doencas respiratórias, cardiovasculares e a alguns cânceres. Lembra também sobre as implicacöes do tabagismo na gravidez e descreve alguns estudos sobre fumante passivo e refere algumas informacöes epidemiológicas. Propöe-se a: 1- Estudar a prevalência de tabagismo entre estudantes da Universidade de Säo Paulo, campus de Ribeiräo Preto, em 1988. 2- Analisar a tendência desta prevalência no perído de 1980-1988. 3. Analisar a relacäo entre escolhas das marcas e o custo das mesmas. Foi feito um levantamento em 1988, com questionário padronizado e com a mesma metodologia utilizada em 1980, 1981 e 1986. As principais conclusöes foram: 1- A prevalência do tabagismo nas diferentes unidades da USP, em dezembro de 1988 foi: FMRP, 11,9% EERP, 21,2%, FFRP, 8,7; FORP, 8,9%; e prevalência de extabagista foi respectimente: 6,8%; 9,3%; 7,5% e 6,5%. 2- As mulheres fumam tanto quanto os homens em 1988 entre os alunos inqueridos. 3- Até 1986, há tendência geral de declínio em todas as escolas; em 1988, esta tendência näo tem significado estatístico. 4- A tendência segundo o gênero alterou-se no período estudado, em 1980 e 1981 os homens fumam mais que as mulheres e em 1986 e 1988, homens e mulheres fumam igualmente. Foram levantadas e discutidas várias hipóteses no sentido de tentar explicar esta tendência geral de declínio e as mudancas ocorridas neste período quanto a prevalência nos dois gêneros (AU). .
