Resumo:Se presentan 3 casos de lepra que acusaban una anemia pronunciada. Durante 2 meses se le sometio a tratamiento con Diasone y se noto una franca mejoria de la anemia. Esto indica que a pesar de que las ulfonas son anemizantes se puede y se debe usarlas despues de haber estudiado e investigado la causa de la anemia. (AU).

Resumo:A case report is presented of a patient in whom clinical evidence of hemolytic anemia with methemoglobinemia developed upon increasing the dosage of sulfoxone which he was receiving for Hansen's disease. Administration of the drug was withdrawn for six weeks, after which studies were made with the use of radioactive iron to demosntrate the hemolytic effect of sulfoxone. By means of tagging cells of a single age group, it was shown that the suceptibility to hemolysis was related to cell age, and it was only after the red cells had been in the circulation for approximately fifty days that they were subject to the hemolytic effects of this drug. No significant biochemical defect could be demonstrated in the erythrocytes of this patient. However, the decline in activity of glucose-6-phosphate dehydrogenase and glutathione reductase in the aging red cell and the reported sensitivity to this drug of patients with red cell deficiency of glucose-6-phosphate dehydrogenase suggest that the hemolytic mechanism is related to this enzyme system. The case demonstrates that drug-induced hemolytic anemia of this type may occur without demonstrable enzymatic defect of the erythrocytes. The observation that patients appear to compensate for the hemolytic anemia and methemoglobinemia if maintained on this drug may be explained by the fact that the older cells containing less adequate enzyme systems are removed from the circulation and an equilibrium is maintained by increased production of red cells with a shorter age span.

Resumo:This study analyses retrospectively some of the risks associated with the use of WHO-multidrug therapy (MDT) in Sri Lanka. Case records of 3,333 new cases of leprosy attending the Central Leprosy Clinic in Colombo during 1991-1995, were analysed for adverse drug reactions involving the liver and blood. There were 81 reports of suspected hepatic or haematological adverse reactions associated with the use of MDT, of which 39 were classified as haemolytic anaemia, 25 as toxic hepatitis, 2 as methaemoglobinaemia and 15 as anaemia. Dapsone, was incriminated in the majority of adverse reactions (72 per cent). Adverse drug reactions were more common in female than male subjects (55 per cent vs 45 per cent; P < 0.5), but there was no significant differences between the age groups. Majority of adverse reactions was seen within the first three months of initiation of MDT. This study in no way undermines the benefits of MDT but highlights the risks and suggests measures to minimize these risks. (AU).

Resumo:Vanuatu, which formerly was known as the New Hebrides, is an archipelago in the southwest Pacific Ocean with a tropical humid climate. The national health system and the state of health of the population generally are satisfactory. The main diseases of public-health importance are malaria and tuberculosis. Their epidemiology and control are discussed. An epidemic of Plasmodium falciparum malaria, which commenced in 1980, is described; this possibly can be explained by an increase in chloroquine-resistant P. falciparum over the same period. Other diseases also are reviewed. Many tropical diseases of great significance elsewhere are not of much significance in Vanuatu. Medical advice for visiting travellers is provided. (AU).

Resumo:Fifty patients of lepromatous leprosy including those of dimorphic type were studied to determine whether anaemia in leprosy is due to chronic infection. Two groups of 25 each, one with no treatment and the other on Dapsone treatment, were subjected to serum iron studies, tissue storage iron and UIBC estimations. Hypoferraemia was observed in 58 per cent of cases of both the groups with low tissue storage ison, normal or high UIBC associated with low MCHC, characteristic of iron deficiency anaemia. Anaemia of chronic infections was observed in only one case. No abnormal sideroblasts coud be seen. Anaemia is tributted to dietary factor.(AU).

Resumo:A massive dose of 7.5 g of 4,4'-diamino, diphenyl sulphone (dapsone) taken as a suicide attempt in a patient on long-term therapy for tuberculoid leprosy resulted in permanent bilateral retinal necrosis, previously unreported side effect of this drug. The patient developed a severe haemolytic anaemia, methaemoglobinaemia, and acute renal failure requiring peritoneal dialysis. It is proposed that the retinal damage was due to a combination of severe hypoxaemia and the physical effects of red cell fragmentation producing vascular occlusion in the macular and perimacular region, with consequent ischaemic necrosis...(AU).

Resumo:Dapsona, farmaco bacteriostatico utilizado no tratamento da hanseniase, vem sendo associado a intercorrencias clinicas, principalmente devido a sua hemotoxicidade. A N-hidroxilacao, uma das principais vias de biotransformacao da dapsona vem sendo associada constantemente a quadros de metemoglobinemia decorrentes do uso da dapsona. Com objetivo de se verificar a inibicao reversivel desta via de bioativacao toxicologica, sem alterar os caminhos de detoxificacao do farmaco, a aceltilacao citosolica, a cimetidina, ranitidina e famotidina foram administradas concomitantemente a dapsona em ratos machos Wistar, com peso variando entre 200 e 250g divididos em 30 grupos (n=6), em estudo de doses unicas e multiplas. Apos as administracoes, os ratos foram submetidos a coleta de sangue(AU).
