Resumo: The interaction between pregnancy, leprosy and leprosy reactions was examined in a systematic literature review. Several retrospective case series and one retrospective cohort study but only one prospective cohort study were identified in the English literature. Type 1 (reversal) reactions were particularly likely to occur during the post partum. This temporal association was also present for both overt and silent neuritis. Type 2 (erythema nodosum leprosum) reactions occur throughout pregnancy and during lactation, and may be severe and recurrent. No prospective, controlled studies were found that documented the complications of pregnancy in women treated with multidrug therapy regimens. Our study highlights the need for such studies, with appropriate controls, on women throughout pregnancy and lactation so that risk factors for reaction and neuritis during pregnancy can be identified and quantified. Resumo: Os autores tecem consideracoes sobre as transmissoes hereditarias, germinativas e congenita ou por via placentaria na lepra, negando a possibilidade das duas primeiras, como soe acontecer, em geral, com todas as molestias infecciosas. Baseados nas poucas observacoes de lepra congenita publicadas na literatura e nos achados bacillares em recem-nascidos, admitem a possibildade, se bem que rara, da infeccao leprosa congenita ou por via placentaria nos fetos. A raridade da tranmissao congenita, confirmada pelas observacoes prolongadas, feitas em recem-nascidos filhos de hansenianos, que rarissimamente se tornam leprosos, quando afastados logo ao nascer do meio em que deveria viver, mostra a pouca importancia que tem na propagacao da molestia. Pelo estudo bacterioscopico e histo-pathologico de cinco placentas, sete cordoes umbilicaes, trez autopsias de recem-nascidos e uma de um feto de quatro meses, relatam ter encontrado bacillos alcool-acido resistentes de Hansen no chorion das villosidades placentarias num caso e no cordao umbilical de outro caso, nao observando nunca a presenca de lesoes histo-pathologicas de natureza leprosa. Alem de nao observarem nos recem-nascidos e no feto a presenca de lesoes macroscopicas e microscopicas que justifiquem a lepra congenita, nao encontraram a presenca de bacillos alcool-acido resitentes de Hansen quer no sangue peripherico de um recem-nascido, quer no sangues e nos orgaos do feto... Resumo: Of 112 leprous women at Aisei-en who have had children, 39 (34.8 per cent) developed the disease during pregnancy or shortly thereafter. In 100 pregnancies that occurred in wommen with leprosy, exacerbation of the disease occured 48 times, the remaining 52 being uneventfull in this respect. Though the initial symptom of leprosy is usually a simple macule, or anesthesia, the symptoms that appear in pregnancy or chilbbirth are usually acute lesions, the so-called "rash", often appearing as numerous active macules and frequently with edematous, erysipelas-like swellings of the face. In the case of abortion there usually is little advance of the disease, though occasionally a case becomes worse in spite of it. It is evident that for women who are in the incubation stage of leprosy, pregnancy and childbirth are liable to precipitate the development of the sisease, and for those who have leprosy childbearing is apt to lead to its exarcebation and extension. Resumo: Estrogen excretion was assayed in 64 women with leprosy and 15 healthy control women. The mean estrogen excretion was lower in women with leprosy than in controls and the incidence of subnormal estrogen values was higher in the leprosy patients than in the controls. There was an association between infant birth weight and frequency of subnormal estrogen excretion. These features were most marked in women with lepromatous leprosy and are further evidence of diminished fetoplacental function in women with leprosy.(AU). Resumo: One hundred and sixteen women with leprosy and 31 healthy controls were studied throughout 155 pregnancies, and their babies were observed for a period of up to two years. Babies of mothers with leprosy weighed less than those of healthy mothers; the placental weights and coefficients followed the same trend. The babies of the mothers with leprosy grew more slowly than those of the healthy mothers and these findings were most marked in the babies of mothers with lepromatous leprosy. The cause of the reduced feto-placental weight is thought to be related to the immune status of the mother.(AU).
