Resumo: Current literature rejects nerve release in leprous facial neuropathy and states that lesions are restricted to the peripheral zygomatic branches. Since there are approximately 500,000 patients with this disease throughout the world, we wanted to clarify the precise location of facial nerve's affection and the benefit of neurolysis. Our study showed that in patients with leprosy, the facial nerve's main trunk, the peripheral zygomatic branches, and all other branches were affected. Follow-up showed improvement in lagophthalmos and in misreinnervation, with no improvement in the control cohort. Nerve release improves muscle function in leprous facial neuropathy, provided surgery is performed on all affected segments. Intraoperative electroneurodiagnostics is an effective tool for detecting the most proximal site of lesion and ensuring effective surgery. (AU). Resumo: Investigation of the eyes of 388 Turkish patients suffering from leprosy revealed some rare and interesting ocular lesions, including Melkersson-Rosenthal syndrome, acute diffuse plastic iridocyclitis, military leproma of the iris, an unique type of iris strophy that produces iris holes and hypopigment patches in the fundus and choroiditis leprosa´praecox. Histopathologic studies of the orbicularis oculi muscle in patients with lagophthalmos suggested that myositis rather than facial nerve paralysis was responsible for the clinical picture. Tonometric examination of 38 eyes showed abnormally low pressures, and tonographic studies of 49 eyes demonstrated that both aqueous production in leprosy patients.(AU). Resumo: While temporalis transfer offers the patient with moderate or severe lagophthalmos the best possible opportunity to preserve his vision, his comfort, and his appearance, the average results in leprosy are not as good as a few isolated reports in the literature would suggest. In spite of all efforts, full closure may not always be achieved, and the training of the patient to activate the transfer frequently, by reflex and in isolation, may not succeed as well as we had expected. But even with the limited success, most of the patients are symptomatically improved asnd sight is saved that otherwise would be lost. Even when it fails to completely satisfy the patient or the surgeon, a temporalis transfer for lagophthalmos is worthwhile.(AU). Resumo: Complete and partial nerve paralysis can result from leprosy. The latter is more prevalent and results in lagophthalmos and corneal hypoesthesia. The former is characterized by loss of facial expression, deviation of the face to the nonparalyzed side, difficulty in chewing, and drooling, in addition to lagophthalmos and corneal hypoesthesia. Affected patients are at risk to develop blindness, as well as suffering social and economic deprivation because of the effects of facial nerve paralysis. Prevention of blindness and amelioration of the latter were successfully achieved by muscle transfer procedures, temporalis transfer to the eyelid, and masseter transfer to the mouth and nasolabial fold in small, minimally equipped hospitals in Africa.(AU). Resumo: The aim of this study was to detect early muscular changes in lepromatous leprosy using simultaneously electromyography and muscle biopsy. In 13 subjects a single clinically normal muscle innervated by the popliteal nerve was studied. Three were found to be normal. All the others were electromyographically denervated. Histopathologic findings included only 3 cases of fascicular atrophy. In 8 cases inflammatory nodules were observed in the connective tissue of the muscle and acid-fast bacilli were present in Virchow cells in 5. In only one patient were intact acid-fast bacilli found in muscle cells. It was concluded that electromyography was the better method of detecting early denervation, while muscle biopsy was the better examination to detect lepromatous myositis. In practice these techniques are complementary in the study of muscle data in lepromatous leprosy.(AU).
