Resumo:The author reduces the moulding facial time from 20-25 to minutes only. To achieve this significant reduction he has imagined an all perforated melt aluminum mold where he put the modelling material in the most possible creamy-like consistency. The technique he used for the molding and confection of the artificial nose as well as for the tiny openings imitating the pores of the patient's skin and teleangiectasis is also discussed. Dr. Oliveira also justifies the use od movable prothesis for all cases when plastic surgery fails in restoring the nasal appendix due to damage of the bone and to dystrophic tissues in lepers. at "Sanat¢rio Cocais" in Casa Branca, S„o Paulo, Brazil, says the author, there are several patients who presented no good results from surgical treatment because of their extensive facial damage. He also emphasizes that public curiosity is always worrying lepers what makes then a rather depressed ori introverted people, and concludes that movable nasal prothesis mitigates their behavior sometimes psychotic, leading then to a better and more pleasant life despite their misfortune. Resumo:Os autores referem a correção do nariz retraído na leprose pela restauração do estôfo nasal empregando o retalho frontal interciliar mediano introduzido à cavidade cutâneo nasal, isto é, por via posterior. A extremidade do retalho é suturada às bordas das narinas, substituindo a totalidade do fôrro do nariz externo. Vinte dias após, secciona-se o retalho, colocando-se o ped¡culo sôbre a região glabelar. Todavia, o objetivo principal do trabalho consiste na obtenção de um retalho frontal composto, isto é, cutâneo-periostal, pelo acréscimo de uma lâmina de periósteo frontal mantendo a sua aderência ao restante. O objetivo do mesmo é conferir maior rigidez ao retalho para evitar a necessidade posterior de enxertia óssea ou cartilaginosa do dorso nasal a ser corrigido. Aumenta-se assim a espessura do retalho, o que torna indicado nos casos de pacientes com pele fina da testa e nos casos mais graves do nariz retra¡do. (AU)^ipt.

Resumo:The author presents 98 cases of deformity of the nasal septum. In every case the repair was done with an osteoperiostal graft from the crest of the tibia. The grafts employed were always autoplastic. It appers that the technique employed in these cases is original. The percentage falture rate is 14,3 per cent.(AU).

Resumo:Destruction of the nasal septum and nasal bones by Mycobacterium leprae and subsequent infection is still seen regularly in leprosy endemic areas. The social stigma associated with this deformity is significant. Many different procedures have been developed to reconstruct the nose. Patients operated on at Anandaban Hospital and the Green Pastures Hospital and Rehabilitation Center between 1986 and 2001 were reviewed. There were 48 patients with an average age of 47 years. Five deformities were mild, 22 were moderate, 13 were severe, and eight were not graded. Bone grafting with nasolabial skin flaps was performed in 14 cases, bone grafiting alone was perfomed in 10 cases, flaps alone were perfomed in seven cases, and catilage grafiting was perfomed in 10 cases. In three patients a gull-wing forehead flap was performed. Overall, excellent or good cosmetic results were obtained in 83 percent of cases. Grafting with or without nasolabial flapes was associated with a 50 percent complication rate of infection or graft resorption. In mild to moderate deformities, cartilage grafiting with bony fixation and skin flaps is recommended. Perioperative antibiotics must be used, and these procedures should be perfoemed by an experienced surgeon. In very severe cases with skin deficiency, reconstruction with a forehead flap gives good results. (AU).

Resumo:On the basis of clinical, histological and bacteriological assessments, 31 patients in Central India were selected and classified as having active but early lepromatous leprosy and 4 patients as having early borderline leprosy. From the nose of each patient an average of 4 biopsies were taken from particular sites of the septum and turbinates either by punch biopsy or dissection with a scalpel. The nasal tissues from all the lepromatous patients contained many acid-fast bacilli; no bacilli or abnormalities were seen in the nasal tissues from the borderline patients. The histopathology of these highly bacilliferous tissues is described. Bacilli were universally seen in macrophages , but they were also seen in blood monocytes and polymorphs, fibroblasts, squamous and pseudo-columnar epithelium, kerati, peri- and endo-neurial cells of tiny nerve bundles, erectile tissue, vascular plain muscle, perivascular histiocytes and frequently and abundantly in the cytoplasm of endothelial lining cells of lymphatics and of small blood vessels and free within the lumina of these vessels. Five basic mechanisms of scape of bacilli from the sunbmucosa on to the surface, and thus into the external environment, are described. Secondary infection, in the presence of an expansile lepromatous infiltrate, together with simple trauma to the surface epithelium, are the main factors in the discharge of bacilli. These histopathological observations are consistent with the findings from other recent studies on the nose in leprosy regarding (1) the large mumbers of morphologically intact and viable Myco. leprae excreted in the nasal mucus of lepromatous patients; (2) the clinical changes observed in the nose of such patients, and (3) the similar nasal involvement and excretion of Myco. leprae from the nose of mice inoculated with leprosy bacilli of human origen. On particular interest was the frequency and intensity of bacilli within the endothelial lining cells of small blood and lymph vessels and the presence of bacilli free within the lumina of these vessels or within monocytes and polymorphs. The possible dynamic significance of these observations in the pathogenesis of leprosy is discussed. The significance of all these observations in relation to (1) the spresd of leprosy; (2) local factors in the nose which might favour the growth of Mico. leprae, and (3) the nose as a portal of entry, are discussed. Resumo:The number of registered leprosy patients world-wide has decreased dramatically after extensive application of WHO recommended Multiple Drug Therapy (MDT). The annual number of new cases has, however, been almost unchanged in several populations, indicating that the infection is still present at community level. Nasal carriage of Mycobacterium leprae DNA was studied in Lega Robi village in Ethiopia. MDT had been applied for more than ten years, and 718 residents over 5 years old were eligible for the study. During the first survey nasal swab samples were collected from 664 (92.5%) individuals. The results of a Peptide Nucleic Acid-ELISA test for M. leprae DNA interpreted by stringent statistical criteria were available for 589 (88.7%) subjects. Thirty-five (5.9%) individuals without clinical signs of leprosy were positive for M. leprae DNA. Seven PCR positive individuals lived in a household where one or two other members were also positive for M. leprae DNA. During a second survey 8 (46%) of 175 interpretable PNA-ELISA tests were positive. Of 137 individuals tested twice, only two were positive on both occasions whereas 10 were PCR positive only once. The study confirms the widespread distribution of M. leprae DNA in healthy individuals. The feasibility of curbing possible transmission of subclinical infection needs further consideration. (AU).

Resumo:332 nose-blow specimens have been examined from 73 untreated multibacillary patients of leprosy before and periodically after they were put on a maximal, minimal or an intermediate multi-drug regimen. 80% of these specimens were found to be positive for acid fast bacilli initially. Bacillary positivity rate was more in samples containing pus or blood. Bacilli were seen in LL, LI as well as BL patients. Nearly half of the cases became negative for AFB in their nose-blow specimens within 3 months of initiation of treatment whereas none of these patients has become negative in skin smears. However, a few cases have continued to discharge bacilli in their nasal secretions even after 12 months of multi-drug regimen therapy.(AU).

Resumo:The communication is based on the experience with 380 reconstructive operations in the face fo patients at the residual stage of leprosy. A classification of the sequelae of leprosy in the face, which includes all types of disfigurement and facilitates selection of the most rational method of surgical treatment for each group, has been elaborated. in most patients who have overcome leprosy, a combination of all kinds of deformations and defects in the facecan be observed, which requires a whole complex of plastic operations in order to take off the mask of leprosy. Reconstructive operations in the face are indicated at the stage of deep regression or in residual leprosy. The paper deals with the successful repair of deformations of the nose by means of dividing the subcutaneous scar contractures and of constructing a supporting cartilage scaffolding, of reconstructing the nose and lips with a Filatov pedicle flap, of repairing facila-muscle palsy with a tape of fascia and lagophthalmos with a myofascial-band plasty, of reconstructing the eye browns with arterialized flaps on subcutaneous pedicles, and of other reconstructive and cosmetic operations. The skin wounds have healed well in leprosy patients, and no hypertrophic or kelloid scar formation has been observed.(AU).
