Resumo:Activities of the brush-border enzymes, alkaline phosphatase, maltase, leucine aminopeptidase, and gamma-glutamyl transpeptidase, were measured in urine samples of 25 lepromatous leprosy patients and an equal number of age-matched healthy controls. None of the patients were shown to be suffering from any other systematic disease. The enzymatic activities were shown to be significantly elevated in leprosy patients when compared to controls^ien.

Resumo:Renal diseases unique to the tropics are those that occur in association with infectious diseases including dengue hemorrhagic fever, typhoid fever, shigellosis, leptospirosis, lepromatous leprosy, malaria, opisthorchiasis, and schistosomiasis. These renal complications can be classified on the basis of their clinical and pathologic characteristics into acute transient reversible glomerulonephritis, chronic progressive irreversible glomerulonephritis, amyloidosis, and acute renal failure (ARF) resulting from acute tubular necrosis, acute tubulointerstitial nephritis, and thrombotic microangiopathy. Certain primary glomerular diseases including immunoglobulin (Ig) M nephropathy and focal segmental and global glomerulosclerosis are prevalent in some tropical countries. Renal complications of venomous snakebites also are common in the tropics. This article discusses and summarizes important works in the literature in respect to the clinical syndromes, pathologic features, and pathogenesis of tropical renal diseases both in humans and experimental animal models. Resumo:Thirthy patients with lepromatous and nonlepromatous leprosy were studied in respect to their renal lesions and changes in kidney function. In none of the patients were specific lesions of the disease found, nor was Mycobacterium leprae demonstrable in any. The most frequent lesions met with included focal interstitial nephritis, glomerular hypercellularity, tubular degeneration, and hyalinization of smail-sized and medium-sized blood vessels. In addition, three patients showed circumscribed aggregates of mononuclear cells, and one showed chronic pyelonephritis. The affection of the kidney was independent of the duration and the type of leprosy.(AU).

Resumo:The genitourologic aspects of leprosy are reviewed. (1) Renal complications are widespread and clinically significant. Amyloid degeneration is common in leprosy, particularly in the West. (2) Adrenal involvement is also seen. Clinical consequences, although not thoroughly evaluated, do not seem significant. (3) Testicular and epididymal involvement, universal in lepromatous leprosy, is of diagnostic and therapeutic importance, particularly in association with lepromatous gynecomastia, a clinical model of acquired Klinefelter´s syndrome. (4) The existence of venereal leprosy, postulated through the ages in folklore, is not borne out by scientific investigation. (5) The external genitalia are generally spared, although occasional involvement has been reported. (6) The ureters, bladder, and urethra are spared as are the prostate and seminal vesicles. (7) Abnormal urinalyses, reflecting renal impairment, are frequently seen. The frequency and significance of mycobacteriuria remain unsettled.(AU).

Resumo:Sixty consecutive patients with leprosy were investigated for renal involvement. Clinically overt renal disease was present in 4 patients; 3 presented with a nephrotic state and one patient with progressive renal failure. Urinalysis showed daily protein loss ranging from 0.4 to 8.9 g in 8 patients and microscopic haematuria in 4 cases. Elevated levels of blood urea and creatinine were seen only in one patient with diffuse proliferative glomerulonephritis. Of the 36 patients in whom distal tubular functions were evaluated, concentration and/or acidification defects were detected in 9 patients (25%). Renal histology revealed no abnormality in any of these patients. Serum C3 levels were decreased in 5 patients with lepromatous leprosy and 3 patients with borderline leprosy. Histological evidence of renal involvement was detected in 9 patients (15%). Amyloid deposits were seen in 3 (5%) patients of whom 2 had lepromatous leprosy and one had tuberculoid leprosy with chronic trophic ulcers. Mesangial proliferative lesions were seen in 5 (8.3%) and diffuse proliferative lesions (with crescents in more than 70% of glomeruli) in one patient. All of them had lepromatous leprosy. Three of the 5 patients with mesangial proliferative glomerulonephritis had erythema nodosum leprosum at the time of biopsy. Immunofluorescence studies revealed granular deposits of IgA, IgM and C3 in one patient with mesangial proliferation and IgA/IgM with or without C3 in 3 more patients in whom renal histology was normal. Glomerulonephritis associated with leprosy appears to be immune mediated but confirmation requires identification of lepra antigen in the glomerular immune complex deposits.(AU).

Resumo:Three African patients with the nephrotic syndrome due to renal amyloidosis are discussed. Together they exemplify the two major categories of amyloidosis: the first patient developed reactive systemic amyloidosis as a complication of chronic tissue necrosis associated with trophic ulceration in tuberculoid leprosy; the second and third patients represent cases of amyloidosis associated with immunocyte dyscrasia.(AU).

Resumo:While leprosy does not primarily affect the kidneys, secondary involvement of kidneys has been reported in lepromatous leprosy and especially in lepra-reaction. In the present study the authors have investigated the functional status of the kidneys of lepromatous leprosy patients by focussing attention to parameters like acidification of urine, glomerular filtration rate and excretion of a foreign dye administered intramuscularly. This study revealed a decreased ability to acidify urine and to excrete the dye as well as a drop in glomerular filtration rate in untreated LL patients. The said disturbances were more pronounced in lepra reaction.(AU).

Resumo:Sequential renal functions have been studied in eight patients of lepromatous leprosy during and after subsidence of erythema nodosum leprosum. The renal functions which were impaired during reaction were observed to improve during quiescent phase unless there were associated complications like amyloidosis. The possible mechanisms for altered renal functions during reactive phase of the disease have been discussed.(AU).
