Abstract:Seventy-five leprosy patients and an equal number of age- and sex-matched controls were examined for tear functions, using Schirmer's test and tear break up time (BUT). There was no statistically significant difference in the Schirmer's test, but the tear BUT showed a statistically significant lower value of < 10 seconds in multibacillary patients compared to paucibacillary patients. Leprosy patients with lagophthalmos and decreased corneal sensation showed a lower value of tear BUT which was also statistically significant. This study shows that even though the quantity of tears is not affected, proper and prolonged wetting of the cornea is deficient in many leprosy patients. (AU)^ien.

Abstract:The ocular complications of leprosy are the most serous lesions in the disease. Leprosy has been one of the great causes of blindness and partial loss of sight. With the advent of sulphone and cortisone therapy, most of this could be prevented if only patients suffering from this disease received the benefits of established treatment in the early stages. There are thousands and thousands of people in backward countries suffering from leprosy who receive no care.

Abstract:Objetico: Determinar a prevalencia de olho seco em portadores de hanseniase do Hospital de Dermatologia Sanitaria de Goiania, comparando-se a um grupo controle. Desenho: Estudo de prevalencia. Material e metodos: A amostra do presente estudo inclui 70 portadores de hanseniase, do Hospital de Dermatologia Sanitaria de Goiania, e 30 individuos no grupo controle, da Fundacao Banco de Olhos de Goias, ambos localizados em Goiania-GO. Foram realizados exeme oftalmologico e testes de Schirmer I, break-up time (BUT) e rosa bengala em todos estes individuos em uma unica avaliacao. Resultados: Quarenta e quatro (63,0%) portadores de hanseniase eram do sexo masculino e 22 73,3%) individuos do grupo controle, do sexo feminino (p=0,001). A idade medica dos hansenianos foi de 61,1+-12,5 anos e no grupo controle, 55,7+-9,6 anos. Quinze (21,4%) hansenianos e quatro (13,3%) individuos deo grupo controle apresentaram diagnostico de olho seco (p=0,429) A forma virchowiana (74,2%) da hanseniase foi a mais prevalente e o olho seco (66,7%) foi mais frequente nesta forma clinica da doenca. Conclusao: A prevalencia de olho seco nos portadores de hanseniase foi semelhante a encontrada nos individuos do grupo controle (AU).

Title:Epidemiological surveillance of the ocular impairments in Hansen's disease.

Abstract:An evaluation of data on ocular disabilities of leprosy registered cases in 1989, in São Paulo State, pointed that it was needed to change the criteria of diagnosis to improve sensibility at the time of detection. Since 1990, the Leprosy Program together with the Ophthalmology Service of São Paulo State, intensified training in detection, prevention and treatment of the ocular disabilities. A group of experts designed a new standardization of procedures and criteria for grading of the ocular disabilities in accordance with the WHO guidelines. The Leprosy notification forms were analyzed for the years 1989, 1993 and 1996 with the objective to evaluate the changes of criteria. In 1989, 3210 leprosy cases were notified, the frequency of ocular disabilities in those cases was 72 (2.2%). During 1993, of the 2927 leprosy cases notified, the frequency of ocular disabilities was 130 (5.3%). In 1996, 2915 cases were notified and the frequency of ocular disabilities was 164 (6.9%). The MB cases showed a greater proportion of ocular disabilities than the PB (p < 0.05). The changes in the forms for the diagnosis of the ocular disabilities, detected at the time of the notification, and the training of personnel, contributed to the increase of the number of cases with ocular disabilities, mainly in grade 1 (corneal hypostesia) and grade 2 (lagophthalmos, trichiasis, corneal opacification). This increase does not indicate worsening of the endemic disease neither later diagnosis, just operational improvement (AU).

Abstract:Após avaliação dos dados de incapacidades oculares disponíveis nas fichas epidemiológicas de hanseníase do ano de 1989, no Estado de São Paulo, constatou-se a necessidade de mudanças de critérios do grau de incapacidade ocular, para aumentar a sensibilidade. Desde 1990, o Programa de Controle da Hanseníase em conjunto com o Serviço de Oftalmologia Sanitária da Secretaria de Estado da Saúde de São Paulo intensificou os treinamentos na detecção, prevenção e tratamento das incapacidades oculares. Um grupo de técnicos foi designado para modificar a padronização dos procedimentos e os critérios para a classificação do grau de incapacidade, de acordo com as orientações da Organização Mundial da Saúde. Com o objetivo de avaliar as modificações da padronização dos critérios foram analisadas as fichas de notificação epidemiológica dos anos de 1989, 1993 e 1996. No ano de 1989, foram detectados 3210 casos de hanseníase, dos quais 72 (2,2%) apresentaram incapacidades oculares. Em 1993, 2927 casos de hanseníase foram notificados, sendo que 130 (5,3%) tinham incapacidades oculares. Em 1996, dos 2915 casos detectados, 164 (6,9%) apresentaram algum grau de incapacidade ocular Os casos multibacilares mostraram uma proporção maior de incapacidades oculares do que os paucibacilares (p < 0,05). As modificações adotadas para o diagnóstico das alterações oculares e os treinamentos realizados contribuíram para um aumento significativo da notificação do número de casos com alterações oculares, principalmente grau I (hipoestesia corneana) e grau II (lagoftalmo, triquíase e opacidade corneana). Este aumento não indica piora da endemia ou mesmo diagnóstico mais tardio, e sim melhora operacional (AU).

Abstract:At varying periods of time following the successful establishment of systemic infections with Mycobacterium leprae or M. lepraemurium in the mouse and the nine-banded armadillo eyes were examined by light microscopy. Inoculation of bacilli was by the intravenous or intraperitoneal route or directly into the hind footpads; eyes were not directly inoculated in this study. During periods of up to 3 years under laboratory conditions no animal showed evidence of impaired vision or blindness, and the external appearance of both eyes was normal. The ocular histopathology and the sites of accumulation of bacilli are described. In immunologically normal mice infected with M. lepraemurium bacilli were much commoner in extraorbital tissues, but they were, nevertheless, found in various tissues within the orbit, including the ciliary body and sclera. In immunologically normal mice (and one rat) injected with M. leprae of human origin no bacilli were found in the eye, but in mice immunologically depressed by thymectomy and total body irradiation considerable numbers of bacilli were present in the iris and ciliary body and also in the limbal cornea. In the armadillo bacilli were found in large numbers in virtually all tissues except the lens, retina, optic nerve, and aqueous and vitreous humours, but the uveal tract was heavily involved. Findings are discussed in relation to the great frequency of ocular involvement and the importance of immune-complex disease in patients with lepromatous leprosy, and to factors which may favour the localisation and multiplication of Mycobacterium leprae in the eye.
