Abstract:Partindo do conceito tradicional estabelecido por Armauer Hansen (1886), segundo o qual a lepra se propaga pelo cont gio no seio da fam¡lia, o A. aventa argumentos epidemiológicos sugerindo um sistema profilático de fundamentos ecológicos no campo da Medicina Preventiva, cujo êxito resultar  nas seguintes medidas: a) inspeção periódica dos domiciliares, atuando-se por Unidades Sanitárias Móveis; b) tratamento intensivo dos casos do grupo indeterminado, com o objetivo de interromper a cadeia de contágio, evitando a sua evolução para o tipo lepromatoso que, na maioria dos casos, decorre de precedente estádio na forma indeterminada, caracterizada por infiltrado inespec¡fico que é o processo dominante na lepra incipiens; c) terapêutica sulfônica intensiva da lepromatose incipiente, prevenindo-se a sua evolução para as lesões abertas poli-infectantes; d) abolição do isolamento compulsório que, sôbre ser extremamente oneroso como se pratica no atual regime sanatorial de segregação, determina a ocultação dos focos da moléstia; os sanatórios, existentes serão mantidos em regime de open-door, para assistência aos casos avançados de lepra, de resistente infecciosidade ou inválidos por cegueira e paralisia, oferecendo condições favoráveis à prática do isolamento seletivo, preconizado pelo VI Congresso Internacional da Lepra (Tóquio - 1958); c) transformação de preventórios em estabelecimentos polivalentes de educação e assistência para menores desamparados, com o objetivo de proteger os filhos de hansenianos do estigma de milenária tradição; f) integração da Profilaxia da Lepra nos Serviços de Saúde Pública, entrosando-a como setor espec¡fico nas organizações polivalentes de finalidades sanitárias, cient¡ficas e assistenciais oficiais, paraestatais e de iniciativa provada; deste modo se estabelece a mais ¡ntima colaboração entre médicos, patologistas e sanitaristas especializados no campo da leprologia, com diretores e técnicos dos diversos setores da Saúde Pública...(AU)^ien.

Abstract:(1) 4:4' diaminodiphenylsulphone (DDS) has been effective in leprosy in very small amounts. (2) In such is amounts it is not more toxic than its derivatives. (3) The effict is shown of vitamin B12 in controlling reaction and anaemia in cases intolerant to DDS. (4) In 58 cases there was an average improvement of over 20 por cent in the bacillary index, while in 12 cases in which the original bacillary index was 10 or less there was an average improvement of almost 50 per cent. (5) There is evidence that lepra reaction induced by DDS, or by another couse during treatment with DDS, favours the more rapid absorption of lepra bacilli. (6) On this account the advisability is discussed of inducing lepra reaction in cases in which it does not otherwise occur. (7) The question of combining local treatment with DDS therapy is discussed. (8) The oral and parenteral methods of giving DDS are compared (AU).

Abstract:The action of the oral administration of diasone has been studied in a group of advanced cases of leprosy of whom almost all had previously been treated with chaulmoogra oil. The drug was given for periods of 8 weeks with a daily dosage varying between 1 and 3 Grams according to individual tolerance; between each period or course of treatment there was a 4 weeks' interval. Anemia (82 per cent), asthenia and depression (88 per cent), headache (74 per cent) were noted as signs of intolerance. There were no grave consequences and intolerance was readily treated with liver extract, iron, and vitamin B complex. Activity of the drug was shown by softening, ulceration, and re-absorption of leprous nodules, healing of ulcers, and re-absorption of deep lepromatous infiltrations. Bacteriologically, granulation of the bacilli was observed but in no case had bacilli completely disappeared from the lesions. The improvement noted was greater the longer and more concentrated the treatment. The results found after 8 months trial were the following: in 5 cases treated for 3 periods, 100 per cent improved; in 11 cases treated for 2 periods, 63.6 per cent improved; in 26 cases after only 1 period of treatment, 50 per cent improved. Thus of 42 patients who had completed 1 to 3 periods of treatment, 59.5 per cent showed improvement. Finally it may be stated that diasone has proved to be effective in the treatment of leprosy in a series of 42 patients observed for 8 months. Further experimental treatment should be carried out so that definite conclusions based on greater experience can be obtained. (AU).

Abstract:A total of 275 determinations of the concentrations of free and hydrolized sulfones in the blood and urine were made in 137 patients over a period of three months. Analyses were made 24 hours after the last dose, and after a rest period of 14 days. In all of the 43 patients under promin treatment, the drug existed in the blood in the free form only, while in the urine it was found in both the free annnd conjugated forms after the 14-day rest period. The ratio between the free and conjugated drug in the urine varied considerably in different individuals. Of the 55 patients taking diasone, 26, or 47.3 per cent, showed conjugation of the drug in the blood 24 hours after the last dose, while in 29, or 52.7 per cent, there was no conjugation in the blood. No form of the drug was present in the blood of any of the patients after a rest period of 14 days. Conjugation in the urine occurred in 52 of the cases, 24 hour after the last oral dose and after the rest period. Concentrations of free and conjugated sulphetrone were noted in the blood and urine in the 15 cases studied, 24 hours after the last dose. This drug was not present in the blood after the rest period. Conjugation of sulphetrone in the urine occurred in 8 of the cases after the rest period, while free sulphetrone was eliminated in all of the cases. When the bloods and urines of 24 patients receiving promacetin were analyzed by directdiazotization and by acid hydrolysis the values for free and total sulfone were identical, which indicates that this drug was unaltered in the body of the patients studied.
