Abstract:Malignant lymphoma involves the gastrointestinal tract as a primary or secundary in the course of disseminated lymphoma. Although primary lymphoma has received the most attention in the literature, secundary lymphoma of the gastrointestinal tract is much more common. The gastrointestinal manifestations and complications are a common problem and there is a lack of information as to diagnosis, management and prognosis. Intensive application of currently-available diagnostic techniques including radiology, cytology, endoscopy, biopsy and gastric secretory studies should be pursued for the evaluation of patients with lymphoma. Abstract:In the past 5 years, there have been notable strides toward the earlier recognition and discovery of melanoma, including new technologies to complement and augment the clinical examination and new insights to help clinicians recognize early melanoma. However, incidence and mortality rates throughout most of the developed world have risen over the past 25 years, while education and screening, potentially the best means for reducing the disease, continue to be severely underutilized. Much progress needs to be made to reach middle-aged and older men and persons of lower socioeconomic status who suffer a disproportionate burden of death from melanoma. Worldwide melanoma control must also be a priority, and comprehensive educational and screening programs should be directed to Northern Ireland and a number of Eastern European nations, whose 5-year survival rates range between 53% and 60%, mirroring those of the United States and Australia more than 40 years ago. LEARNING OBJECTIVE: After completing this learning activity, participants should be aware of the most recent melanoma epidemiologic data, both in the United States and internationally; worldwide early detection and screening programs; clinical strategies to recognize and improve the detection of early melanoma; the latest technologies for early detection of melanoma; and public and professional education programs designed to enhance early detection.(AU).

Abstract:Certain skin cancers occur with increased frequency or altered course in patients infected with HIV. Malignant melanoma and squamous cell carcinoma are examples of cutaneous malignancies that have a more aggressive course in patients with HIV. Others, such as basal cell carcinoma, appear more frequently in this population but do not appear to be more aggressive. The incidence of HIV-associated Kapsosi's sarcoma has markedly decreased since the advent of HIV antiretroviral therapy. Our understanding of the pathogenesis of this malignancy and its unique management issues are fully reviewed. Cutaneous T-cell lymphoma (CTCL) is rare in this population. Other types of cutaneous lymphoma and HIV-associated pseudo-CTCL are discussed. This article addresses prevention, treatment, and follow-up strategies for this at-risk population. Abstract:Skin cancer is the most common cancer in human beings. The increased incidence of skin cancer has brought much attention to the process by which these tumors develop and how they can be prevented. Efforts have been made to educate the public about the importance of protecting skin from excessive ultraviolet light. Despite this work, the incidence of skin cancer continues to increase. Available compounds may be useful in the chemoprevention of skin cancer. Chemoprevention is defined as oral or topical use of dietary or pharmacologic agents to inhibit or reverse the development of cancer. Potential agents included are the retinoids; difluoromethylornithine; T4 endonuclease V; polyphenolic antioxidants, such as (-)-epigallocatechin gallate, found in green tea and grape seed extract; silymarin; isoflavone genestein; nonsteroidal anti-inflammatory drugs; curcumin; lycopene; vitamin E; beta-carotene; and selenium. Many of these agents are available over the counter as topical or oral preparations. Abstract:Cutaneous flushing-a common presenting complaint to dermatologists, allergists, internists, and family practitioners-results from changes in cutaneous blood flow triggered by multiple conditions. Most cases are caused by very common, benign diseases, such as rosacea or climacterum, that are readily apparent after a thorough taking of history and physical examination. However, in some cases, accurate diagnosis requires further laboratory, radiologic, or histopathologic studies to differentiate several important clinicopathologic entities. In particular, the serious diagnoses of carcinoid syndrome, pheochromocytoma, mastocytosis, and anaphylaxis need to be excluded by laboratory studies. If this work-up is unrevealing, rare causes, such as medullary carcinoma of the thyroid, pancreatic cell tumor, renal carcinoma, and others, should be considered. Abstract:Skin cancer is less common in persons with skin of color than in light-skinned Caucasians but is often associated with greater morbidity and mortality. Thus, it is crucial that physicians become familiar with skin cancer in persons of color so as to maximize the likelihood of early detection of these tumors. In dark-skinned ethnic groups, squamous cell carcinoma is most common; squamous cell carcinoma and melanoma usually occur on nonsun-exposed sites; and ultraviolet radiation is not an important etiologic factor for skin cancer with the exception of basal cell carcinoma. Races of intermediate pigmentation, such as Hispanics and Asians, share epidemiologic and clinical features of dark-skinned ethnic groups and Caucasians. Skin cancers pose a significant risk in skin of color and clinicians should focus on preventive measures in these groups such as regular skin exams, self-examination, public education, and screening programs. Abstract:Um paciente, portador de infecçao hansenica ha 20 anos, e considerado como virchowiano inativo, apresentou nos ultimos 4 anos de vida, dois surtos de reaçao de pseudo-exacerbaçao ( reaçao reversa), o ultimo deles coincidindo com internaçao onde foi diagnosticado um carcinoma bronquiolar dos pulmoes, cujo agravamento ocasionou o obito. Discutem-se as dificuldades do diagnostico clinico diferencial deste tipo de carcinoma pulmonar com outras patologias com caracteristicas clinicas e evolutivas semelhantes, bem como a dificuldade de se chegar a este diagnostico apenas pela analise de uma biopsia pulmonar. Em relaçao as reaçoes de pseudo-exacerbaçao, duas interpretacoes sao postas em confronto, e e salientada a hipotese destas reaçoes se desenvolverem como resposta a surtos de proliferaçao bacilar que poderm ocorrer em pacientes dimorfos em qualquer fase da evoluçao da infecçao, devido a tratamento inadequado ou resistencia medicamentosa...(AU).

Abstract:A autora realizou um estudo sobre o Carcinoma verrucoso, denominação abrangente para tres tipos de tumores malignos: papilomatose oral florida, Epitelioma cuniculatum e Condiloma gigante de Buschke-Lowenstein. Fez um levantamento bibliográfico da literatura nacional e estrangeira por periodo de 10 anos. As principais caracteristicas clinicas, histopatológicas, imunohistoquimicas, a controvertida etiopatogenese, o prognostico e os tratamentos preconizados foram revistos. Reuniu casuistica de cinco lesões representativas de cada uma das entidades que compôem o Carcinoma verrucoso. Foram analisados novos cortes histopatologicos corados pela Hematoxilina-Eosina e ressaltadas as principais caracteristicas microscopicas: hiperceratose, acantose, muitas vezes do tipo condilomatoso, presença de leucoedema, mitoses frequentes, atipias celulares em graus variaveis, preservação da membrana basal (durante longo periodo), e infiltrado dérmico, principalmente de células mononucleares. As variações que permitem o diagnostico diferencial entre as tres entidades representativas do Carcinoma verrucoso são enfatizadas. Complementou o exame histopatologico com analise imunohistoquímica (Imunoperoxidase. Método ABC. Substrato: Diaminobenzidina), usando cinco marcadores para as citoqueratinas: CK10, CK14, CK16, CK19 e CK20. Verificou os seguintes resultados imunohistoquimicos: retardo na expressão da CK10, identificada nas camadas suprabasais e mais superiores, liberação da CK14, principalmente no estrata suprabasal: positividade caracteristica da epiderme proliferante com a CK16; ausencia de reatividade com as CK19 e CK20 (habitualmente presente nas celulas de Merkel). A expressividade apreciada com estes anticorpos não aumentou a precisão diagnostica observada nos cortes corados pela Hematoxilina-Eosina, mas permitiu reconhecer as modificações intracitoplasmaticas que ocorrem nas células epidermicas no proceso de transformação maligna. Abstract:Se observan entre 1.332 histórias de enfermos de lepra de Fontilles, 23 casos de cáncer, que da una frecuencia del 1'72 por cento. En el 95 por cento eran enfermos lepromatosos. La localización cancerosa más frecuente fue la cutánea, 11 casos, un 47'82 por cento. Del número total de defunciones, hubo 12 por cáncer, es decir, un 4 por cento, siendo la distribución: cuatro de estómago, dos de recto, dos de piel, uno de mama, uno de esófago, uno de próstata y uno de lengua. subsisten un caso de mama a lso cuatro años de terapia radio-quirúrgica y uno de endoetrio de dos meses. La mortalidad menos fue la del cáncer cutáneo, con solo un 5'5 por cento. Los 11 casos cutáneos fueron epiteliomas, con una frecuencia entre los enfermos hansenianos estudiados del 0'82 por cento.Se observaron siete basocelulares y seis espinicelulares, pues en dos enfermos el número de epiteliomas fue de dos. Hubo clara preferencia por la cabeza y de las fromas espinicululares por las extremidades. Creemos que no hay ningún antagonismo entre lepra y cáncer, pero que la cifra no es superior a la población sin lepra. Destacamos la preferencia de la localización cutánea, siguiendo la gran frecuencia de epiteliomas observados en la región levantina en nuestra consulta dermatológicas. Se exponen las historias clínicas de siete nuevos casos de asociación lepra y cáncer, cuatro cutáneos, uno de mama, uno de endometrio y uno de lengua, haciendo hincapié en un caso interesante de aparición de cáncer sobre ulceración trófica de pierna y otro de etiología pós-traumática. Se observa el pronóstico benigno del cáncer cutáneo con la importancia del diagnóstico precoz y nos inclinamos claramente por la terapéutica quirúrgica. (AU).

Title:Associação lepra-tuberculose num caso de tumor ganglionar gigante da região crutal direita.

Abstract:Os AA. referem um cso em que observaram ganglios cruraes direitos grandemente enfarctados, com o tamanho da cabeça de recem-nascido. Os ganglios eram adherentes entre si a á pele, apresentando um delles nitida fluctuação; por fistula escôava-se o conteudo ganglio, sendo um liquido amarellado, pouco denso. Com esses caracteristicos, os AA. consideram provavel tratar-se de uma adenopathia causada pela lepra e tuberculose associadas. O diagnostico é confirmado pela necropsia, pelo exame hsitopatologico e pela inoculação em cobaio da materia caseosa de um ganglio, cujo parenchyma fôra totalmente destruido, ficando reduzido apenas a capsula, um tando espessada. Referem-se á frequencia das adhenopatias cruraes determinadas pela associação lepra e tuberculose, considerando-as raras segundo a observação feita entre os doentes de Cocaes e de accordo com os dados dos outros AA. Estudam, finalmente, a maneira por que se manifestou de uma propagação pela via lymphatica e admittindo que os bacillos attingiram-nos pela via hematogenetica. (AU).

Abstract:La question de la concomittance de lèpre-tumeurs malignes se montre encore plu éclaircie, étent donée le petit nombre de cas publiés. Ainsi donc, elle est très diversement apreciée par les auteurs, léprologistes ou cancénologistes. Leurs opinions sont les plus différentes, jugeant la majorité que la concomittance est d'une grande rareté. Ayant observé, dans une période de 3 ans environ, 44 cas l'incidence de néoplasies malignes chez des lépreux de léproseries de l'État de São Paulo (Brésil), les auteurs ne sont pas da l1avis de ceux qui jugent telle incidence très rares, et moins encore de ceux qui croient à une imconpatibilité entre les deux maladies, ou à une petite sensibilité des lépreux vis-àvis du cancer. Tous les 44 cas onde été au point de vue clinique at anatomo-opathologique, para biopsie ou nécroscopie. (AU).
