Title:Humoral and cellular immune reactivity to recombinant M. leprae antigens in HLA-Typed leprosy patients and healthy controls.

Abstract:In our search for Mycobacterium leprae antigens that might specifically induce immunity or immunopathology, we have tested both humoral and cellular immune reactivity against purified recombinant M. leprae antigens in 29 paucibacillary (PB), 26 multibacillary (MB) leprosy patients, and 47 matched healthy contacts. The following M. leprae antigens were tested: 2L-1 (65L-1, GroEl-1), 2L-2 (65L-2, GroEl-2), 4L (SoDA), 43L, 10L (B) and 25L (Sra). The individuals were also typed for HLAD-RB1 and DQB1 in order to see whether leprosy status and/or immune reactivity to these antigens might be associated with certain HLA types. We also tested sera from another 48 patients before, during and after multidrug therapy (MDT) to study the relationship between antibody reactivity to recombinant M. leprae antigens and MDT. Antibody titers to the four recombinant M. leprae antigens tested and to D-BSA were higher in MB patients compared to PB patients and healthy controls, and declined with treatment. From a diagnostic or monitoring point of view none of the recombinant antigens seemed to be an improvement over D-BSA, mainly due to the lower sensitivity. IgG subclasses were measured in positive sera of untreated patients. These were mainly of the IgG1 and IgG3 subclasses, but subclass diversity was also observed and antigen dependent: all four subclasses could be detected against 10L (B), only IgG1 and IgG3 against 43L and only IgG1 against 25L and 2L-1. Cellular immune reactivity against the purified recombinant M. leprae antigens was measured in a lymphocyte stimulation test (LST). As for M. leprae, there was an inverse correlation between antibody and T-cell reactivity. However, the number of LST responders to recombinant antigens was much lower than to M. leprae. The 43L antigen was recognized most often (19%-24% of individuals tested) and more often than the 10L (B) antigen (10%-12%). No clear correlation was observed with leprosy type or protection and, in general, M. leprae nonresponders were also negative with recombinant antigens. Finally, we confirmed that HLA-DRB1*02 is associated with leprosy in this population, and we observed an association between DQB1*0601 and lepromatous leprosy. The number of positive individuals was too small to allow a meaningful analysis of the relationship between HLA type and immune reactivity. Although these data do not allow a conclusion as to one of these purified recombinant antigens being either protection or disease related, the antigen-dependent IgG subclass diversity warrants further study on antigen-specific qualitative differences in immune reactivity that may be relevant for the outcome of an infection with M. leprae. Title:Low serum HDL-cholesterol is associated with raised tumor necrosis factor-alpha during ENL reactions.

Abstract:The concentrations of serum lipids and tumor necrosis factor (TNF) were measured in leprosy patients across the spectrum of the disease and in erythema nodosum leprosum (ENL) patients at the onset of the reaction and after the reaction had clinically subsided. Lepromatous/borderline lepromatous (LL/BL) patients had significantly higher serum triglyceride and lower HDL-cholesterol levels; there was no such change in the tuberculoid/borderline tuberculoid (TT/BT) patients. The household contacts (HC) of the LL/BL patients also had significantly lower serum HDL levels. ENL patients during the acute phase of the reaction had significantly lower total, LDL-, HDL-cholesterol levels compared to the stable LL/BL patients, and these changes were reversible to pre-ENL levels after the reaction had subsided. Serum TNF levels were significantly higher in household contacts and in LL/BL patients but were not statistically different in TT/BT patients. Serum TNF levels were also significantly higher during the acute phase of ENL, and declined after the clinical remission of the reaction to levels comparable with those of LL/BL patients. There was a significant negative correlation between serum TNF and HDL-cholesterol levels during and after ENL reaction. However, there was no such correlation between TNF and total or LDL-cholesterol levels in ENL patients. Our results suggest that the changes in HDL-cholesterol metabolism are a specific part of the host response to lepromatous leprosy and to the ENL reaction and may be mediated by increased TNF production. Title:Assessment of anti-PGL-I as a prognostic marker of leprosy reaction.

Abstract:The anti-phenolic glycolipid-I (PGL-I) assay as currently applied for leprosy is conceived as an early marker of asymptomatic infection, early disease diagnosis and cure monitoring. Its use as a prognostic marker of reaction is still a matter of controversy. We conducted a case-control study to investigate whether IgM and IgG anti-PGL-I antibodies could discriminate patients at increased risk of developing reactions. Eligible cases were untreated leprosy patients at the onset of type 1 and type 2 reactions recruited from among 600 concurrent, newly detected, untreated leprosy patients attending an outpatient clinic in central Brazil. For the patients with reaction, approximately the same number of leprosy cases without reaction matched as to bacterial index (BI), age and gender were randomly selected. Individuals without clinical leprosy were evaluated as healthy controls. Sera from type 1 reaction (N = 43) and type 2 reaction (N = 26) patients were tested by an ELISA using PGL-I synthetic disaccharide-BSA antigen and 1:300 sera dilution (cut-off point > or = 0.2 OD). Antibody profiles were evaluated by exploratory data analysis and reverse cumulative distribution curves. The IgG anti-PGL-I response did not have a defined pattern, being detected only at low levels. Our results indicate that leprosy patients, independently of their reactional status, produce high levels of IgM anti-PGL-I, demonstrating a strong correlation between the magnitude of antibody response and the BI. Patients with a higher BI were at least 3.4 times more prone to produce an antibody response compared to healthy controls. (AU)^ien.

Abstract:Após considerações gerais sobre a importância do problema do mal perfurante plantar como incapacidade f¡sica e sua gênese, o autor expõe as caracter¡sticas de um produto obtido do sangue de amimais jovens (Solcoseryl) e usado no tratamento desta complicação tardia de lepra. Em quatorze anos obtem doze curas com um m¡nimo de cinco e um máximo de 38 aplicações, e em dois casos obtem melhora evidente. Os resultados são ilustrados por fotografias anteriores e posteriores ao tratamento. Concluiu-se que, pela ação cicatrizante rápida, é de utilidade no tratamento, em regime ambulatorial, do mal perfurante plantar de origem leprótica. (AU)^ipt.

Abstract:1- No presente trabalho o autor analisou a distribuição dos grupos sangu¡neos do sistema clássico ABO e Rh(D-d), num grupo de 1.127 doentes de lepra (1.656 lepromatosos e 471 tuberculóides), classificados de acordo com o tronco (stock) racial (tabela 1). 2- Quando a amostra é considerada como um todo, a frequencia do grupo sangu¡neo O nos doentes da forma tuberculóide mostra um excesso significante em relação à encontrada nos lepromatosos (tabela 2). 3- A amostra de doentes de lepra lepromatosa, quando comparada aos dados da população não leprosa, não apresenta diferença quanto à frequencia do grupo O. O mesmo não acontece com os tuberculóides, que apresentam um excesso de indiv¡duos desse grupo sanguineo. 4- Todas as sub-amostras consideradas mostraram estar em equil¡brio de Hardy-Weinberg (tabela 3), o que significa não ser a seleção do grupo O tão eficiente, a ponto de destruir o equil¡brio genético. 5- As sub-amostras lepromatosas e tuberculóides, comparadas quanto às estimativas das frequencias gênicas do sistema ABO, não mostraram diferenças significantes (tabela 3). Essa diferença, entretanto, aparece quando as amostras aumentam de tamanho. 6- Não se assinalaram diferenças na frequencia de Rh negativos entre lepromatosos e tubercul¢ides (tabela 1). 7- Sabendo-se que existem indicações recentes de que os ant¡genos de certos organismos infecciosos são serológicamente semelhantes aos ant¡genos do sangue, parece interessante averiguar se não seria este também o caso do Mycobacterium leprae. Abstract:Rheumatic manifestations are a common feature of leprosy. The present study was performed to assess the incidence of rheumatic manifestations in leprosy. Methods: seventy cases of leprosy attending the indoor and outdoor departments were studied. Results: Out of the 70 cases studied, rheumatic manifestations were seen in 61.42% of cases: arthritis in 54.28% and soft tissue rheumatism in 17.14%. Enthesitis was seen in 2.84% of cases. The incidence of arthritis in lepra reactions was 57.57%. Conclusions: Musculoskeletal manifestations can occur at any time during the infection. Articular inflammation in leprosy usually occurs in reactive states, particularly erythema nodosum leprosum (ENL). Leprosy can present with arthritis as the first symptom.

Abstract:Forty-four positive reactions to the latex fixation tests were obtained in the serum of 101 leprosy patients, twenty-four using the euglobulin fraction. The result of the sheep cell agglutination test was positive in fourteen of ninety-five of the same serum samples. Serologic studies suggest that the agglutinating factors in serum from patients with leprosy are similar if not identical to the so-called rheumatoid factors. Duration, type of course and severity of leprosy appear to be unrelated to the presence of positive reactions to serologic tests. The incidence of other diseases found in association with leprosy has been recorded and their relationship to the latex fixation test and to the sheep cell agglutination test is discussed. From the results of this study, it is not possible to define the significance of rheumatoid factors in serum from patients with leprosy. It is concluded that they probably play a non-specific role with respect to the clinical course.

Abstract:Numerous clinical and serological similarities exist between lepromatous and dimorphous leprosy and diseases od collagen, particularly lupus erythematosus and rheumatoid arthritis. The clinical manifestations include spontaneous skin-ulcerations, ischaemic necrosis, petechial and purpuric eruptions, vesicle and bulla formation, subcutaneous nodules, hepatosplenomegaly, migratory arthralgia and polyarthritis, bizarre skin lesions including butterfly facial rashes, and a general tendency toward exacerbations and remissions. Haematological findings include anaemia and the ocasional appearance of lupus cells.Positive serological findings include rheumatoid factors, circulating thyroglobulin antibodies, false positive serological tests for syphilis, and cold precipitable proteins (a consistent finding). Because of these similarities, the authors believe that leprosy is a disease which should be considered as a model for the study of hyperglobulinaemic states, especially the diseases of collagen. An important consideration is that, unlike these other diseases, the agent causing leprosy is known.

Abstract:Secondary amyloidosis is the leading cause of death in lepromatous leprosy in this country. The mortality is directly caused by amyloid involvement of the kidneys, which occurred in 100% of this series. The diagnosis of this complication depends largely on the development of a persistent proteinuria, Congo red absorption, hepatosplenomegaly and, to a lesser degree, altered serum proteins and perhaps liver biopsy in selected cases. the correlation between the Congo red test and the degree of amyloidosis is poor, and unless this test shows 80 to 100% dye absorption in one hour the diagnosis cannot be made from this finding alone. Serum protein and lipoid changes occur in lepromatous leprosy per se, and in a study of the various protein fractions no consistent alteration occurred in cases complicated by amyloidosis. The average duration of life after the onset of proteinuria in amyloidosis was 38.33 months, with a fairly wide range above and below this figure. The development of anemia and a progressive elevation of the nonprotein nitrogen are unfavorable prognostic signs.

Title:Association of leprosy with HLA-DR2 in a Southern Brazilian population ..-

Abstract:The association between HLA specificities and lweprosy was investigated in a Southern Brazilian population. One hundred and twenty-one patients and 147 controls were typed for HLA-A, B, Cw, DR and DQ. Patients were subdivided into the following subgroups, according to clinical, histological and immunological criteria: lepromatous (N=55), tuberculoid (N=32), dimorphous (N=20), and indeterminate (N=14). The frequencies of HLA specificities were compared between the total group of patients and controls, and between the same controls and each subgroup of patients. After correction of the probabilities, deviations were not significant, except for the DR2 specificity, which presented a frequency of 44.2% in the total group of patients and 56.3% in the subgroup of invididuals with the tuberculoid from of the disease, compared to 23.3% in the controls. Stratified analysis showed that the increased DR2 frequency in the total group of patients was due to the subgroups with the tuberculoid and dimorphus forms. The relative risk of tuberculoid leprosy for DR2-positive individuals was 4.2 and etiologic fraction of DR2 was 0,429. In conclusion, a positive association of the DR2 specificity with the tuberculoid form of leprosy, but not with the lepromatous, dimorphous, or indeterminate forms, was demonstrated in this Southern Brazilian population (AU).

Abstract:T cell production of IFN-gamma contributes to host defense against infections by intracellular pathogens, including mycobacteria. Lepromatous leprosy, the dissminated from of infection caused by Mycobacterium leprae, is characterized by loss of cellular response against the pathogen and diminished Th1 cytokine production. Relieving bacterial burden in Ag-unresponsive patients might be achieved through alternative receptors that stimulate IFN-gamma production. We have previously shown that ligation of signaling lymphocytic activation molecule (SLAM) enhances IFN-gamma in mycobacterial infection; therefore, we investigated molecular pathways leading from SLAM activation to IFN-gamma production in human leprosy. The expression of the SLAM-associated protein (an inhibitory factor for IFN-gamma induction) on M. leprae-stimulated cells from leprosy patients was inversely correlated to IFN-gamma production. Howevwe, SLAM ligation or exposure of cell from lepromatous patients to a proinflammatory microenvironment down-regulated SLAM-associated protein expression. Moreover. SALAM activation induced a sequence of signaling proteins, including activation of the NF-kB complex, phosphorylation of Stat1, and induction of T-bet expression, resulting in the promotion a cascade of molecular events during signaling through SLAM in leprosy that cooperate to induce INF-gamma production and strongly suggest that SLAM might be a focal point for therapeutic modulation of the cell cytokine responses in diseases characterized by dysfunctional Th2 responses (AU).

Abstract:Although the prevalence of leprosy has decline over the years, there is no evidence that incidence rates are falling. A method of early detection of those people prone to develop the mosth infectious form of leprosy would contribute to breaking the chain of transmission. Prophylactic treatment of serologically idenfified high-risk contacts of incidend patients should be an operationally feasible approach for routine control programs. In addition, classification of high-risk household contacts will allow control program resources to be more focused. Is this prospective study, we examined the ability of serology used for the detection of antibodies to phenolic glycolipid I of Mycobacterium leprae to identify those household contacts of multibacillary leprosy patients who had the highest risk of developing leprosy. After the start of multidrug therapy for the index case, a new case of leprosy developed in one in seven of the 178 households studied. In households where new cases appeared, the seropositivity rates were significantly higher (P<0.001) than those in households without new cases. Seropositive household contacts had a significantly higher risk of developing leprosy (relative hazard adjusted for age and sex [aRH], 7.2), notably multibacillary leprosy (aRH=24), than seronegative contacts (AU).

Abstract:Toll-like receptor 2 (TLR2) is a key mediator of the immune response to mycobacterial infections, and mutations in TLR2 have been shown to confer susceptibility to infections with mycobacteria. This study investigated the profiles of cytokines, such as interferon (IFN)-gamma, interleukin (IL)-10, IL-12 and tumour necrosis factor (TNF)-alpha in response to Mycobacterium leprae in peripheral blood mononuclear cells (PBMC) with the TLR2 mutation Argo677Trp, a recently reported polymorphism that is associated with lepromatous leprosy. In leprosy patients with the TLR2 mutation, production of IL-2, IL-12, IFN-gamma, and TNF-alpha by M. leprae-stimulated PBMC were significantly decreased compared with that in groups with wild-type TLR2. However, the cells from patients with the TLR2 mutation showed significantly increased production of IL-10. There was no significant difference in Il-4 production between the mutant and wild-type during stimulation. Yhus, these results suggest that the TLR2 signal pathway plays a critical role in the alteration of cytokine profiles in PBMC from leprosy pateints and the TLR2 mutation Arg677Trp provides a mechanism for the poor cellular immune response associated with lepromatous leprosy (AU).

Abstract:A case report is presented of a patient in whom clinical evidence of hemolytic anemia with methemoglobinemia developed upon increasing the dosage of sulfoxone which he was receiving for Hansen's disease. Administration of the drug was withdrawn for six weeks, after which studies were made with the use of radioactive iron to demosntrate the hemolytic effect of sulfoxone. By means of tagging cells of a single age group, it was shown that the suceptibility to hemolysis was related to cell age, and it was only after the red cells had been in the circulation for approximately fifty days that they were subject to the hemolytic effects of this drug. No significant biochemical defect could be demonstrated in the erythrocytes of this patient. However, the decline in activity of glucose-6-phosphate dehydrogenase and glutathione reductase in the aging red cell and the reported sensitivity to this drug of patients with red cell deficiency of glucose-6-phosphate dehydrogenase suggest that the hemolytic mechanism is related to this enzyme system. The case demonstrates that drug-induced hemolytic anemia of this type may occur without demonstrable enzymatic defect of the erythrocytes. The observation that patients appear to compensate for the hemolytic anemia and methemoglobinemia if maintained on this drug may be explained by the fact that the older cells containing less adequate enzyme systems are removed from the circulation and an equilibrium is maintained by increased production of red cells with a shorter age span.
