Resumo: This paper deals with footwear for partially amputated feet of patients in tows and cities who require either surgical footwear made to measure or normal footwear which can be adapted surgically. It includes a problems associated with amputation of the great toe including balance, relief of pressure and satiffeling to prevent deformity. Unilateral amputation of all toes and associated complications, bilateral amputation of al toes, with similar complications, but with the added possibility of rocker soles to correct the gait, and amputatiuons through the midtarsalarea where major protheses are necessary.Descriptions are given of the use of sponge rubber cork and plastazote for weight distribution and relief and of the manufacture of suitable footwear by the vacuum forming method.(AU). Resumo: One hundred and fifty-six opponensplasties carried out on 115 patients at Anandaban Hospital between 1987 and 1997 were reviewed. In most cases a flexor digitalis superficialis opponensplasty was performed. The outcome was assessed by measuring the finger to which the thumb could obtain a pinch grip, the gap between the thumb and little metacarpophalangeal joints, and the satisfaction of the patient. The objective assessments demonstrated excellent or good results in 89%. Good or fair patient satisfaction was obtained in 93%. Early complications were seen in seven cases (4%). Objective measurements of outcome and patient satisfaction were not always in agreement, indicating that objective measures do not adequately assess the success of surgery from the patient's perspective. We thus conclude that subjective measurements of results are an important measure of success and should be included in the evaluation of surgical results. (AU). Resumo: In 12 patients, the extensor carpi radialis longus muscle tendon unit was elongated using the radial half of the parent tendon so that it could reach the site of new insertion, the A1-A2 pulley of flexor sheath or lateral bands, after routing the transfer through the carpal tunnel. The tendon was of appropriate thickness and could be split into two halves to be used as a graft. Further splitting of the tendon into four tails was possible. The transferred slips retained adequate strength to activate the fingers after the operation. It is suggested that splitting of the extensor carpi radialis longus tendon to use one half as a tendon graft be considered in patients in whom extensor carpi radialis longus transfer is planned to correct finger clawing. This technique is simple, needs minor modification in the sequence of operative steps, reduces operating time, and saves the patient from postoperative discomfort, muscle herniation, and scarring at the donor site (usually the thigh). Resumo: Dermatoglyphics patterns and total ridge count have been atudied in 25 cases of Tuberculoid leprosy and were compared with 25 controls. There was no statiscally significant variations in loops, whrols and total average ridge count. There were 4 (1.6%) arches in Tuberculoid leprosy as compared to 11 (4.4%) of controls and was not statiscally significant. There was more decrease of arches on the left hand of the patients as compared to the left hand of the controls.(AU). Resumo: Skin slit smears of 46 patients of lepromatous leprosy (16 untreated, 30 long treated) were studied from ear lobules, fingers, elbows and knees. In untreated patients ear lobules gave highest BI, as compared to other sites. MI from ear lobules was higher than elbows and knees but slightly lower than that from fingers. In treated patients sites other than ear lobules yielded solid staining bacilli more frequently. Multiple sites and especially peripheral sites are recommended for study of skin slit smears to discover persistent bacilli.(AU). Resumo: Four patients are described who presented with congenital finger contractures and arthropathy. There was synovial cell hyperplasia and giant cells but no inflammatory process. Radiographs showed flattening of the metacarpal and metatarsal heads and the proximal femoral ossification centres. In the oldest patient the process had subsided leaving slight contractures but severe impairment of hip mobility. In another the arthropathy was still prominent in the early teens. In a third, finger contractures had failed to respond to conservative or surgical measures.(AU). Resumo: Characteristic bone lesions in leprosy affect the small bones of the face, hands, and feet. They are due directly to leprous infection imposed upon denervated tissues. The manifestations of leprous bone infection vary from the presence of bacilli in the bone to actual destruction. Distal absorption involves fingers and toes with the loss of digits. Combined absorption of lenght and width of bone often affects the metatarsophalangeal joints. Tarsal absorption patterns are determined by abnormal weight-bearing forces and infection.(AU). Resumo: A total of six index finger branch of the radial cutaneous (IRC) nerves from three BL, two BB and one BT cases of leprosy, with less than 6 months history of disease were subjected for nerve conduction velocity (NCV) studies followed by biopsy. The biopsy was divided into three parts and subjected for light microscopy quantitative histology, electron-microscopy and fibre tease studies. The nerves revealed a slight reduction in NCV and a varying degree of segmental demyelination ranging from 10% to 35%. The earliest changes observed were the thickening and proliferation of Schwann cell processes of the unmyelinated fibres and degeneration of their axons regardless of the type of leprosy.(AU) A total of 35 contacts from 20 families of lepromatous index cases were screened clinically. All of them were subjected for nerve conduction velocity (NCV) studies of both left and right index branch of the radial cutaneous (IRC) nerve. Fifteen of these nerves were biopsied. Out of 15 IRC nerves biopsied 10 had slightly reduced average NCV values and 5 were normal. Four nerves out of 10 where average NCV was reduced and 1 nerve out of 5 where NCV was normal showed significant ultrastructural changes and higher percentage of segmental demyelination (10%--13%). There was a striking similarity between the changes seen in the IRC nerves of early cases of leprosy and of contacts.(AU).
