Resumo:The reactional states seen in lepromatous form (erythema nodosum, leprosum, exacerbation) and i interpolar forms (tuberculoid exacerbation and tuberculoid reversal reaction) result in cutaneos, nervous, lymphatic, visceral and polyarthritic acute manifestations. The sucessif relapses result in articular, cutaneous and muscular sequelae of the hand but a paralysis of medial and ulnar nerves, often associated with leprosy, make them different from rheumatoid arthritis. They can be summarized thus: 1- Fixed claws hand associated with stiffness, erosions and sometimes subluxations of interphalangeal joints, mainly proximal and metacarpophalangeal joints...AU.

Resumo:A panel of 17 mouse monoclonal antibodies (MoAb) raised against Mycobacterium leprae (M. leprae) antigens was used to detect antigenic determinants in normal human skin. An indirect immunoperoxidase technique was used. Eight of the MoAb detected epidermal antigens similar to patterns well known for human sera. Five of these MoAb detected determinants in the dermis, too. These observations may indicate a certain degree of similarity between the antigenic determinants occurring in M. leprae and in the human host. We propose that such a similarity on the one hand may facilitate the survical of M. leprae in the human host when the antigens are not recognized as "non-self", a situation which seems to ocuur in lepromatous leprosy, when the patients' tissues are loaded with bacteria virtually without any immune response. On the other hand, M. leprae antigens which mimic host antigens may induce an auto-immune reaction against the host's own antigens, which could explain the immune reaction in tuberculod leprosy and during a "reversal reaction" when M. leprae is not observed in the host tissues, but extensive granuloma formation occurs (AU).

Resumo:Definitive surgery for leprosy includes plastic, orthopedic, and general procedures. They are performed to improve or correct cosmetic and functional deformities that primarily affect the face and extremities, also for other problems due to leprosy, as gynecomastia and orchitis, and for problems that are unrelated to leprosy. Details of surgical technique would hardly be of interest to dermatologists. More important are the problems that contribute in the selection of surgical candidates. Knowledge of the pathogenesis, pathology and the morbidity of the indications and the appropriate surgical procedures. Factors concerning these problems are included in today's subject: the dermatological aspects and the relationship of the skin to surgery.

Resumo:To determine the best molecular method for diagnosing leprosy, two sets of Mycobacterium leprae-specific primers were compared. Fresh biopsies and slit skin smear samples were obtained from 67 leprosy patients and examined by touchdown (TD) PCR using primers amplifying either a 129-bp fragment of the RLEP repetitive sequence or a 360-bp fragment of the 18-kDa protein gene of M. leprae. Seventeen of 30 (56.7 per cent) biopsy specimens and four of 37 (10.8 per cent) slit skin smear specimens were positive using the primer for the 18-kDa protein gene, whereas 24 of 30 (80 per cent) biopsy and 27 of 37 (73 per cent) slit skin smear samples showed detectable PCR products in the RLEP repetitive sequence. Twenty-one of 31 cases (67.7 per cent) with a bacterial index of zero were PCR positive for the primer RLEP repetitive sequence. These results demonstrate that detection of M. leprae using PCR with primers to a RLEP sequence is more sensitive and specific than PCR with the 18-kDa protein gene primers and also slit smears with acid fast staining. PCR of RLEP repetitive sequences is therefore a useful means of detecting M. leprae DNA even when it is present at very low levels. (AU).

Resumo:While attempting to grow M. leprae in an innovated culture medium, organisms similar to Propionibacterium, but not conforming to any of its documented types, were isolated from lepromatous nodules in 10 out of 15 leprosy cases. Subsequently, a similar organism could also be isolated from the depth of the skin of normal persons but not from their skin surface. The possible role of Propionibacterium in the pathogenesis of leprosy or other skin infections has been discussed.(AU).

Vasculitis, a dense inflammatory exudate, and necrosis of the skin characterized the Lucio type of leprosy; in erythema nosodum leprosum the vasculitis was not so severe and necrosis did not occur. Large numbers of organisms in the endothelial cells of the vessels in the skin could have given rise to bacteremia and development of the disease in the liver. The liver in the patient with Lucio's phrnomenon contained tuberculoid lesions, whereas necrosis of sinusoidal walls and many Virchow's cells were noted in the other patient. Lperosy bacilli in Kupffer cells were easily demonstrated with electron microscopy. The presence of the bacteria within membrane-bound phagocytic vacuoles and the fusion of lysosomes with the vacuoles were seen.(AU).

Resumo:Histopathologic studies were conducted on tissues from necropsies on 41 nine-banded armadillos from Louisiana with a natural disease resembling lepromatous leprosy, hereafter often referred to as the "natural disease". The lesions were composed of macrophages (histiocytes) containing numerous acid-fast bacilli and were similar those seen in armadillos experimentally inoculated with Mycobacterium leprae. Invasion of small and large nerves by phagocytes containing acid-fast bacilli was a characteristic feature of the natural disease. The Mycobacterium presumed to cause the disease was not cultivable on standard mycobacterial media; however, mycobacteria belonging to the M. avium-intracellulare group were cultivated from lymph nodes of 8 and the spleen of 1 of 32 of the diseased armadillos but not from other organs. Cultures of lymph node specimens and other organs from each of the remaining 24 diseased armadillos were negative. Acid-fastness of the bacilli in all tissues was abolished on exposure to pyridine. The bacilli were DOPA oxidase positive, but interpretation of this finding is difficult because some tissues from normal armadillos also gave positive reactions in the spot test employed. Histopathologic studies of tissue from autopsies on the 41 armadillos and microbiologic studies on tissues from 32 of the animals provided evidence indicating that the cause of the natural disease in armadillos may be M. leprae.(AU).

Resumo:Fifty armadillos from 12 locations have been found with a naturally acquired disease caused by an agent that is indistinguishable from Mycobacterium leprae. Forty-seven of the animals were from 459 examined from 11 locations in Louisiana. Two were from unknown locations in Louisiana and one animal was from eastern Texas. Most of the infected animals were initially diagnosed by the examination of ear specimens.(AU).

Resumo:The effect of increased temperature during transportation of clinical dermatophyte specimens was investigated. Recovery rates from untransported specimens cultured at dermatologists' offices and from duplicate transported specimens were compared. During the months of hot weather specimens could be exposed intermittently to temperatures as high as 60 degrees C during transportation from Tucson area clinics to the University laboratory. The rates of recovery from known positive specimens were found not to be significantly different at these places regardless whether specimens were transported during the hot months or cooler months of the year. In a controlled experimental approach to the effect of this elevated temperature on clinical specimens weighed amounts of skin scales collected from guinea pigs artificially infected with Trichophyton mentagrophytes were exposed to 60 degrees C for up to four hours and then digested with 0.5% (:300) trypsin for one hour. Analysis of plate counts done from the digestion mixture showed no significant difference between counts obtained from specimens exposed to elevated temperature and unexposed controls.(AU).

Resumo:Skin slit smears of 46 patients of lepromatous leprosy (16 untreated, 30 long treated) were studied from ear lobules, fingers, elbows and knees. In untreated patients ear lobules gave highest BI, as compared to other sites. MI from ear lobules was higher than elbows and knees but slightly lower than that from fingers. In treated patients sites other than ear lobules yielded solid staining bacilli more frequently. Multiple sites and especially peripheral sites are recommended for study of skin slit smears to discover persistent bacilli.(AU).

Resumo:An E.M. study was carried out to investigate whether Mycobacterium leprae occur intracellularly in epidermal melanocytes. As this could not be confirmed, the selective killing of melanocytes by cytotoxic lymphocytes could not explain the hypopigmentation in types of leprosy with a relative good immune response. There were indications that these hypopigmented lesions resulted from a disturbed transfer of melanosomes from melanocytes to keratinocytes. Further research is in progress.(AU).

Resumo:Histopathologic studies were conducted on tissues from necropsies on 41 nine-banded armadillos from Louisiana with a natural disease resembling lepromatous leprosy, hereafter often referred to as the natural disease. The lesions were composed of macrophages (histiocytes) containing numerous acid-fast bacilli and were similar to those seen in armadillos experimentally inoculated with Mycobacterium leprae. Invasion of small and large nerves by phagocytes containing acid-fast bacilli was characteristic feature of the standard mycobacterial media; however, mycobacteria belonging to the M. avium-intracellulare group were cultivated from lymph nodes of 8 and the spleen of 1 of 32 of the diseased armadillos but not from other organs. Cultures of lymph node specimens and other organs from each of the remaining 24 diseased armadillos were negative. Acid-fastness of the bacilli in all tissues was abolished on exposure to pyridine. The bacilli were DOPA oxidase positive, but interpretation of this finding is difficult because some tissues from normal armadillos also gave positive reactions in the spot test employed. Histopathologic studies from 32 of the animals provided evidence indicating that the cause of the natural disease in armadillos may be M. leprae.(AU).

Título:Rapid bactericidal effect of rifampin on Mycobacterium leprae

Resumo:Rifampin was rapidly bactericial for Mycobacterium leprae. Leprosy responds very slowly to current therapy, so there is a special need for more rapidly effective drugs. In mice rifampin exerted a bactericidal-type effect with single administration by gavage; the effect increased with dosage in the range 10 to 40 mg/kg of body weight. Five patients with lepromatous disease were treated with 600 mg rifampin daily, and the viability of the bacilli in their skin lesions was tested by inoculation of mice. Infectivity for mice had completely disappeared in the 1st specimen collected after the start of therapy - at 7 days in 4 patients and 14 days in 1. In 4 control patients treated with dapsone, infectivity for mice was lost much more slowly and in 1 was still present, though decreased, 112 days after the start of treatment. The slower loss of infectivity with dapsone is in accord with our previous experience in which the same methods were used.(AU).
