Título:Induction of apoptosis in monocytes by Mycobacterium leprae in vitro: a possible role for tumour necrosis factor-alpha

Resumo:A diverse range of infectious organisms, including mycobacteria, have been reported to induce cell death in vivo and in vitro. Although morphological features of apoptosis have been identified in leprosy lesions, it has not yet been determined whether Mycobacterium leprae modulates programmed cell death. For that purpose, peripheral blood mononuclear cells obtained from leprosy patients were stimulated with different concentrations of this pathogen. Following analysis by flow cytometry on 7AAD/CD14+ cells, it was observed that M. leprae induced apoptosis of monocyte-derived macrophages in a dose-dependent manner in both leprosy patients and healthy individuals, but still with lower efficiency as compared to M. tuberculosis. Expression of tumour necrosis factor-alpha (TNF-alpha), Bax-alpha, Bak mRNA and TNF-alpha protein was also detected in these cultures; in addition, an enhancement in the rate of apoptotic cells (and of TNF-alpha release) was noted when interferon-gamma was added to the wells. On the other hand, incubation of the cells with pentoxifylline impaired mycobacterium-induced cell death, the secretion of TNF-alpha, and gene expression in vitro. In addition, diminished bacterial entry decreased both TNF-alpha levels and the death of CD14+ cells, albeit to a different extent. When investigating leprosy reactions, an enhanced rate of spontaneous apoptosis was detected as compared to the unreactive lepromatous patients. The results demonstrated that M. leprae can lead to apoptosis of macrophages through a mechanism that could be at least partially related to the expression of pro-apoptotic members of the Bcl-2 protein family and of TNF-alpha. Moreover, while phagocytosis may be necessary, it seems not to be crucial to the induction of cell death by the mycobacteria. (AU).

Resumo:Biopsies of the skin and liver were obtained from a patient with Lucio's type of leprosy and from one with erythema nodosum leprosum. Vasculitis, a dense inflammatory exudate, and necrosis of the skin characterized the Lucio type of leprosy; in erythema nosodum leprosum the vasculitis was not so severe and necrosis did not occur. Large numbers of organisms in the endothelial cells of the vessels in the skin could have given rise to bacteremia and development of the disease in the liver. The liver in the patient with Lucio's phrnomenon contained tuberculoid lesions, whereas necrosis of sinusoidal walls and many Virchow's cells were noted in the other patient. Lperosy bacilli in Kupffer cells were easily demonstrated with electron microscopy. The presence of the bacteria within membrane-bound phagocytic vacuoles and the fusion of lysosomes with the vacuoles were seen.(AU).

Resumo:A skin test has been developed to determine the degree of competency in clearing bacilli from the tissues of patients suffering from various forms of leprosy. The test involves the intradermal ijection of a suspension of killed Mycobacterium leprae. The response of leprosy patients to the injection of other mycobacterial antigens, one prepared from M. lepraemurium and another from an atypical mycobacterium from a hamster, was also investigated in roder to study the isopathic phenomenon. Since lepromatous patients react negatively in tests with standard Mitsuda antigen, a concentration of 640 x 10(6) M. leprae per ml was used to producce macroscopic responses. The results of the test can be applied to determine the duration of consolidation treatment for lepromatous and indeterminate bacteriologically negative patients after regular treatment has ended. The test can also be used to indicate which Mitsuda-negative contacts should be given preventive treatment, and might be used to identify a given mycobacterium as M. leprae.(AU).

Resumo:The purpose of this study was to investigate the presence of peroxidase (PO) in the histiocytes which are found in the lepromatous lesions of patients with nodular lepromatous leprosy (NLL). We studied dermoepidermal biopsy specimens from lepromstous lesions and blood smears of 10 patients with NLL, eight males and twoo females 28 to 63 years age (average 45 + or - 6.2), of which nine coursed with the stable form of the disease and one was in lepromatous reaction. Six had received treatment with diaminodiphenylsulphone for more than six months, and the other four, none. As controls we studied the blood smears of 10 healthy controls and 10 rat liver sections. PO was investigated in histiocytes, Kupffer cells and polymorphonuclears by dichlorhydrate oxydation, according to the technique of Kaplow. By means of Fite-Faraco´s stain, all ten cases proved to have abundant phagocytized M. leprae. PO was not found in histiocytes of lepromatous lesions in nine cases of stable NLL in lepromatous reaction. PO was present in Kupffer cells, in polymorphonuclears of patients with NLL and in controls. No difference was found either in the PO or M. leprae contents between treated and untreated patients. The PO deficiency in histiocytes of patients with NLL may be related to an incapacity of these cells to destroy M. leprae.(AU).

Resumo:The ultrastructure of blood vessels in endoperi and epineurium was studied in peripheral cutaneous nerve biopsies of 16 borderline (BB), borderline-lepromatous (BL) or lepromatous (LL) leprosy patients some of whom were in reversal reaction. Comparable vessels in nerve biopsies of control cases and vessels in skin lesion biopsies of the leprosy patients were also studied. Vascular changes were found in nerves of all the leprosy patients. The changes were pronounced in endoneurial vessels and affected 1. endothelial continuity and surface structure, 2. basement membranes od endothelium and pericytes, and 3. the vessel lumen. In addition, intra-endothelial (IE) Mycobacterium leprae were a feature in some of the patients. Gaps occurring between endothelial cells and plasma insudation both noticed in vessels of fascicles with early to very early neuropathy suggested extensive leakage which, in all propability, causes early nerve fibre damage. Luminal and abluminal endothelial protusions, which were frequently observed, may enhance transendothelial transposrt. Fenestrations and endothelial attenuation, possibly, lead to an increase in vascular permeability. Endothelial phagocytotic activity, particularly in small (epineural) arteries, appeared to be stimulated, possibly, by circulating M. Leprae. Basement membrane multilayering (a hyaline zone) was found peripherally to pericytes, as is the case in tuberculoid leprosy (Boddingus, 1976). In a number of patients, multilayering occurred also periendothelially. Perivascular zones, which are thought to initiate or aggravate neuropathological changes by impairment of diffusion of oxygen and nutrients or metabolites, were very wide in endoneurial vessels of patients in reversal reaction and this suggested an immunological aetiology. Partial or total vessel lumen occlusion, seen in advanced lepromatous neuropathy, most likely contributes to final nerve fibre degeneration and endoneurial fibrosis. M. leprae were found intra-endothelially in endoneurial vessels, though only fascicles with advanced neuropathy whereas bacilli were not seen in vessel lumina. By contrast, infascicles with realtively early neuropathy, solid (viable) bacilli were frequently encountered intra-axonally in myelinated fibres. This suggested thast, inmany instances, primary infiltration of M. leprae into nerve facicles may arise from intraaxonal..(AU).

Resumo:The effects of dapsone on polymorphonuclear leukocyte functions and lymphocyte mitogen-induced transformation were assessed in vitro and in vivo in normal individuals and in newly diagnosed untreated patients with lepromatous leprosy. The effects of dapsone on the cell-free generation of superoxide by the xanthine: xanthine oxidase system and iodination of bovine serum albumin by horseradish peroxidase were also investigated. In normal individuals dapsone mediated stimulation of polymorphonuclear leukocyte migration in vitro and vivo. Dapsone had no effect on postphagocytic hexose monophosphate shunt activity in vivo. Similar effects were found in patients with lepromatous leprosy. Dapsone also decreased the inhibitory activity of serum from patients with lepromatous leprosy on normal polymorphonuclear leukocyte migration in vitro. Progressive loss of serum-mediated inhibition of migration was observed after ingestion of dapsone by the patients. Further experiments showed that stimulation of polymorphonuclear leukocyte motility was related to inhibition of lymphocyte transformation at high concentrations in vitro, but had slight stimulatory activity on phytohemagglutinin-induced transformation in controls and patients in vivo.(AU).

Resumo:The ultrastructural changes that develop in mouse peritoneal macrophages from 10 minutes up to 14 weeks after exposure to Mycobacterium leprae are presented. Phagocytosis occurred by a process of engulfment by cytoplasmic processes and incorporation into a phagosome, into which lysosomal enzymes were subsequently introduced. Electron transparent zones (E.T.Z.) were not observed around phagocytosed bacilli in this study, however discrete droplets of lipid-like material appeared in the cytoplasm of macrophages, between 2 and 4 weeks after ingestion of the micro-organisms. Phagosomes with double limiting membranes were observed in macrophages harvested as early as 40 minutes after exposure to M. leprae, contrary to the observations of Evans and Levy (1972).(AU).
