Abstract: Deux observations d'arthrosynovite survenant au cours d'une << réaction lépreuse >> sont rapportées: l'une concerne un cas de lèpre nerveuse où une monoarthrite d'un genou succède à une intradermo-réaction de lépromine; l'autre un cas de lèpre lépromateuse où, quelques mois après l'administration de disulone, survient une atteinte polyarticulaire au cours d'un érythème noueux. Divers cas relevés dans la littérature sont analysés. Les désorders immunitaires observés au cours de la réaction lépreuse fournissent l'explication la plus satisfaisante. La rifampicine, utilisée avec succès dans l'un des cas rapportés, completè lers moyens thérapeutiques de la réaction lépresuse où corticoides, thalidomide, lamprène, sont doués d'une réelle efficacité.(AU). Abstract: Four patients are described who presented with congenital finger contractures and arthropathy. There was synovial cell hyperplasia and giant cells but no inflammatory process. Radiographs showed flattening of the metacarpal and metatarsal heads and the proximal femoral ossification centres. In the oldest patient the process had subsided leaving slight contractures but severe impairment of hip mobility. In another the arthropathy was still prominent in the early teens. In a third, finger contractures had failed to respond to conservative or surgical measures.(AU). Abstract: Characteristic bone lesions in leprosy affect the small bones of the face, hands, and feet. They are due directly to leprous infection imposed upon denervated tissues. The manifestations of leprous bone infection vary from the presence of bacilli in the bone to actual destruction. Distal absorption involves fingers and toes with the loss of digits. Combined absorption of lenght and width of bone often affects the metatarsophalangeal joints. Tarsal absorption patterns are determined by abnormal weight-bearing forces and infection.(AU). Abstract: Queixas reumáticas foram clinicamente estudadas em 32 doentes de hanseníase; 18 mulheres e 14 homens (idade entre 14 a 60 anos); formas clínicas: 16 virchowiana, 7 dimorfa, 4 tuberculóide, 3 indeterminada e 2 tuberculóide reacional. Resultados: Artralgia em 11 pacientes; foi poliarticular, bilateral, episódica e acometeu grandes articulações (joelhos, cotovelos e punhos); esteve comumente associada a febre e eritema nodoso. Artrite em 6 pacientes; predominou no sexo masculino; foi fixa, poliarticular e geralmente bilateral; acometeu grandes e pequenas articulações (IFP, MCF, joelhos, punhos e cotovelos) esteve comumente associada a lesões cutâneas da hanseníase e febre. Pseudo-artropatia em 23 casos; foram consideradas as manifestações neurológicas e mão e pé reacionais que simularam artropatia. Algumas lesões dermatológicas da hanseníase que se assemelhavam à doença difusa do tecido conjuntivo foram estudadas. (AU).
