Abstract: Histioid leprosy is a special nodular variety of lepromatous leprosy characterized histologically by spindle-shaped histiocytes arranged in intertwining patterns. These spindle-shaped cells are a marked contrast to the typical lepromatous histiocytes which exhibit a foamyvacuolated cytoplasm. The term histioid was introduced because it resembles a tumor developing from spindle-shaped histiocytic elements. It must be differentiated from certain histiocytic and fibrocytic skin tumors which closely resembles. Such dermal tumors may also be found in patients with lepromatous leprosy. Histioid lesions must not be considered to be entirely confined to patients in leprosy hospitals, and such rare lesions may confront the pathologist unexpectedly. Acid-fast stains for Mycobacterium leprae reveal numerous bacilli within the histioid lesions that conform to the spindle-shape of the cell and the absence of bacilli from the dermal tumors which it resembles.(AU). Abstract: Exophytic neoplasms arise rarely in neurotrophic ulcers of leprosy. When such a lesion does develop, it has clinical resemblance to an indolent and locally invasive epithelioma cuniculatum, may invade deeply, and may metastasize. The case described showed deep invasion and extensive involvement of bone. Exophytic neoplasms in neurotrophic ulcers of leprosy should be considered to be squamous-cell carcinomas, not epitheliomata cuniculata, because of their potential for rapid, deep invasion and metastases.(AU).
