Título:Rate and time distribution of relapses in multibacillary leprosy.

Resumo:Of the 47,068 patients registered in the Polambakkam Leprosy Center between 1955 and 1982, we selected 1886 cases having shown bacteriological positivity at any time during this period, whatever their classifications at registration, and subsequently found bacteriologically negative. After an average follow-up period of 10 years, 243 relapses were observed, giving a crude relapse rate of 12.8 per person-years of observation and a cumulative probability of relapse of 18.9%. Relapse rates were found to be dependent on regularity during smear-positive and -negative periods; a regularity greater than 75% in the smear-positive period proved to be particularly important. The results show no evidence that relapses occurring after 3 years of negativity could be reinfections, and that the relapse rate was still affected by regularity 7 years after negativation. The median delay of relapses was found to be 4.4 years and was not affected by the regularity of treatment.

Título:Leprosy in hypertensive nude rats (SHR/NCrj-rnu).

Resumo:Since more than a decade ago, we have attempted to develop spontaneously hypertensive rats carrying the nude gene that permits high multiplication of Mycobacterium leprae. A congenic strain carrying nude (rnu) and hypertensive genes was produced using SHR/NCrj females and F344/NJcl-rnu males. Cross-intercross was carried out 12 times to establish the hypertensive nude rat congenic strain. As a result of the genetic monitoring test with NE12F2 generation rats, the genetic profile of the SHR/NCrj-rnu rats was the same as that of the SHR/NCrj rats except for the rnu gene. We have successfully developed a hypertensive congenic nude rat strain (SHR.F344Hfh11; SHR/NCrj-rnu). An increase in the blood pressure in nude rats was found to begin at a slightly delayed age when compared with their hairy litter mates. Both female and male rats showed the highest blood pressure at approximately 20 weeks of age--166 +/- 1.4 and 197 +/- 11 mm Hg in nude rats and 175 +/- 11 and 193 +/- 3.2 mm Hg in their hairy litter mates in female and male rats, respectively. In the present study, comparisons were made on the susceptibility to M. leprae in hypertensive SHR/NCrj-rnu and normotensive F344/NJcl-rnu rats. We have reconfirmed that hypertensive SHR/NCrj-rnu rats of the NE12F3 generation were highly susceptible to M. leprae. In the SHR/NCrj-rnu rats of both sexes excellent massive swelling due to multiplication of M. leprae was observed and, also, nodular lesions were produced in uninoculated fore feet and lips while those sites in the F344/NJcl-rnu rats showed only a slight swelling of the inoculated feet with mild nodular lesions. Although mild lymphocyte proliferation was seen only in the M. leprae-inoculated site with numerous bacilli and partial necrosis in the SHR/NCrj-rnu rats, at noninoculated sites, multiplication of M. leprae was only observed in the cells of the mononuclear phagocyte system. However, in F344/NJcl-rnu rats, lymphocyte proliferation with a few neutrophils was seen at the site of inoculated hind foot pads and everywhere at the site of multiplication of M. leprae. There was a wide difference in the susceptibility to M. leprae between the SHR/NCrj-rnu and the F344/NJcl-rnu rats. (AU)^ien.

Resumo:The pathology of the murine leprosy was studied detailedly in the more advanced stages of the infection; the same having occured initially, or in the consecutive phases. excepting the works of Fite and Henderson who, using subcutaneous innoculation, made a detailled and systematic studies in the lesions of the innoculation point; they give attention also to the pathological process in the internal organs, but without intermission for the knowledge about the complete evolution of the infection. Conscious of the absence of minute characteristics as to the different evolutive stages of the disease when intraperitoneal experimental innoculations is made in rats, it was decided to carry on the progressive study of the lesions in the various organs of the animal. We made use of 265 young rats, ages varying between 30 and 50 days, belonging to the Mc Clollum and Wistar breeds, and some others crossbreedings with Rattus nervergicus. The material used to inoculations proceeds from experimentally injected rats, with several passages; the subcutaneous nodules were removed, and the internal organs rich in bacilli were triturated and diluted in physiological solution-glycerin at 40 per cente in the proportion of 1:20; being kept in ice-box at 4.øC. The turbidity of bacilli suspension corresponding approximated to that of test-tube n.ø 5 of Mac Farland scale. The work was done under sterility conditions. Before innoculation was carried forth richness of gacilli suspension was checked. Innoculation was effected through intraperitoneal route and single dosed at 0,5 cc. per animal. This dose was varied with some groups. Animals were sacrificed daily, from the first to the twetieth day, every five days to the hundred and eightieth day after innoculation and in continuation, every fifteen days. Resumo:In sumary, it may well be conculuded that the nude mouse with thymus cell grafting as a severe lepromatous leprosy model was developed in C57BL/6-nu/nu as well as CF#1-nu/nu, comparing with untreated nude mice. In addtion, comparison of sex in treated group, susceptibility to M. leprae in the some males were higher than in foot swelling of the females, also, difference of individuality occured in both sex at a latter stage after infection, therefore, further investigation is under way to look for preferable nude mouse and hairless-athymic mouse which they are established by ourselves.

Resumo:BACKGROUND: Tumour Necrosis Factor alpha (TNFalpha) has been shown to contribute to heart failure (CHF) progression. AIMS: We have tried to antagonise the detrimental effects of TNFalpha on skeletal muscle apoptosis, by using thalidomide, a drug that inhibits its biosynthesis. METHODS: CHF was induced in 20 rats by injecting monocrotaline, which determines right ventricle (RV) failure. After 2 weeks, when CHF developed, 12 rats were treated with thalidomide 3.5.mg/kg per day for 2 weeks. Eight had saline and served as CHF controls. RESULTS: Thalidomide failed to decrease TNFalpha and its second messenger sphingosine (SPH), but was able to prevent the shift toward the fast myosin heavy chains. In the Tibialis Anterior muscle of the thalidomide group, the degree of atrophy, the number of apoptotic nuclei and the levels of caspases, were similar to those of the CHF controls. CONCLUSIONS: Thalidomide, at the doses used in this study, which are the same employed for the treatment of tubercolosis, leprosy, AIDS and cancer in humans, did not lower either TNFalpha or SPH and only marginally influenced the apoptosis-induced muscle atrophy. Since other TNFalpha blockers are under investigation for improving the clinical status of patients with CHF, the present data could be relevant in the design of randomised clinical trials in humans. Resumo:Neonatal thymectomy and neonatal thymectomy plus ATS resulted in a merked increase in susceptibility of both Buffalo and Lewis rats to infection with M. leprae. In Buffalo rats, increase in susceptibility was limited to footpad infection. In Lewis rats, this was extended to include testis infection where the organisms in thymectomized-ATS-treated rats were found to be still in the logarithmic phase of growth 15 months after inoculation.(AU).

Resumo:In order to learn whether the neonatally thymectomized Lewis rat (NTLR) infected with Mycobacterium leprae could serve as a model for chemotherapeutic studies in a situation resembling that found in human lepromatous leprosy, NTLR inoculated with M. leprae either locally or intravenously 9 to 16 months earlier were treated for from 1.5 to 8.5 months with dapsone (4,4´-diaminodiphenylsulfone, DDS) incorporated in the rat chow in the concentration providing the minimal inhibitory concentration of the drug for M. leprae and in the 100-fold larger concentration. NTLR were killed at intervals; the M. leprae were counted and passed to mice. Treatment with the smaller dosage of dapsone neither killed M. leprae nor reduced the number of organisms in the bacterial populations, whereas treatment with the larger dosage both killed M. leprae and reduced their numbers. The rate at which the organisms were killed (i.e., rendered noninfective for mice) was much the same as that in patients treated with dapsone in comparable dosage. The dead organisms were removed from the rat tissues at a faster rate than encountered in patients. The NTLR may indeed be suitable for chemotherapeutic studies relevant to man. In addition, the more rapid diappearance of dead M. leprae from the rat tissues may facilitate the study of treatment regimens designed to eradicate persisting viable organisms.(AU).
