Title:Histological assessment of dermal nerve damage occurring during multidrug therapy for leprosy.

Abstract:This is a prospective histomorphological assessment of dermal innervation in biopsies taken before and after multidrug therapy (MDT) from 41 leprosy patients: 35 borderline tuberculoid (BT), 3 borderline lepromatous (BL), 3 lepromatous (LL). Biopsies of the same lesions taken before commencement (diagnostic therapy) and at the end of therapy (check biopsy) were compared. Hematoxylin and eosin, immunoperoxidase stain for S-100 protein, and the Holmes' silver impregnation method for nerve cells and fibers were used. Skin biopsies were classified as having detectable or undetectable nerves. Of 35 patients with BT leprosy, 17 had no detectable nerves in their diagnostic biopsies; in the check biopsies of 13 of these 17, dermal nerves remained undetectable, in 2 they were S-100 positive but were Holmes negative. Identifiable dermal nerves were present in diagnostic biopsies from 18 patients; in the check biopsies 5 of these 18 had no detectable nerves while in the remaining 13 nerve branches could be detected. The study provides histological documentation of complete damage to dermal innervation in 62.85% (22/35) of patients with BT leprosy, of which 14.28% (5/35) occurred during MDT. Of the patients with detectable dermal innervation at the onset of MDT, 27.7% (5/18) suffered continuing damage during MDT. (AU)^ien.

Abstract:The authors researchd fat substancies in 231 cutaneous lesions of lepers, of various forms and stages, using frozen sections stained with the Scharlach R-hematein. Is was established that in leprosy of lepromatous form fat is found in a large percentage (63,5 per cente); in the tuberculoid form fat was found only in the reactionary from, and among these only in 16,6 per cente, and with special characteristics. Furthermore, in 33 per cente of the incharacteristic leprosy cases fat could be disclosed; for this reason and for other motives of cytological, bacteriological and clinical order, the authors believe that such lesions ought to be considered as regressive lepromatous lesions. It is the authors opinion that the research of the may be helpful, in some cases, in the differential diagnosis between certain types of lepromatous lesions. (AU)^ien.

Abstract:The lesions suspensions by intracutaneous injections of M. tuberculosis and M. leprosy suspensions, in BCG vaccinated guinea pigs, are cytologically alike; the cells of the inflmmatory tissue are morphologically and physiologically indicals and constitute lesions of a same type, with a central abscess surrounded by the tuberculosis inflammatory tissue. This cytological and histological identity makes easy the comparative study of the lesions by these two mycobacteria in vaccinated guinea pigs. In opposition in normal guinea pigs the same comparison is difficulted by the presence of the abscess observed only animals injected with M. tuberculosis. Ninety eight normal guinea pigs were innoculated with 33.0 mg of BCG, by peritoneal or intramuscular route. Fourt days after 58 guineas pigs were injected intracutaneously with 0,05 ml of a M. tuberculosis (BCG) suspension, containing 0.165 mg of bacilli; 22 guinea pigs were injected by this same via with 0, 1 ml of lepromin contaning nearly 0.074 mg of M. leprae; the other 20 guinea pigs were injected by this same route with 0.165 mg of BCG on a side and with 0,1 ml of lepromin containing 0.170 mg of M. leprae on the other side. The macroscopic and microscopic lesions suscitated by the intracutaneous injection of these two mycobacteria were observed during 50 days. The macroscopic examination shows: a) the reaction provoked by 0.165 mg of BCG is stronger but disappears at the 30th day after innoculation; b) the reaction suscitated by approximately 0.074 mg of M. leprae is weaker but vanishes only 40days after innoculation. The histological study of the lesions shows: a) the injection of 0.165 mg of BCG induces stronger reactions which have shorter evolutive course, with complete cicatrization of the 50th day; b) the injection of nearly 0.170 mg of M. leprae produces weaker lesions which have longer evolutive course, without complete cicatrization at the 50th day after innoculation. Que diante das provas pharmaco-dynamicas e biológicas applicadas como tests demonstrativos da acção do systema grande sympathico, apezar da desigualdade dos resultados, julgados que o paciente reage mais no sentido vago-tonico que no sympathicotonico e em rigor, talves, seja um amphotonico. Abstract:The reactional states seen in lepromatous form (erythema nodosum, leprosum, exacerbation) and i interpolar forms (tuberculoid exacerbation and tuberculoid reversal reaction) result in cutaneos, nervous, lymphatic, visceral and polyarthritic acute manifestations. The sucessif relapses result in articular, cutaneous and muscular sequelae of the hand but a paralysis of medial and ulnar nerves, often associated with leprosy, make them different from rheumatoid arthritis. They can be summarized thus: 1- Fixed claws hand associated with stiffness, erosions and sometimes subluxations of interphalangeal joints, mainly proximal and metacarpophalangeal joints...AU.

Abstract:A panel of 17 mouse monoclonal antibodies (MoAb) raised against Mycobacterium leprae (M. leprae) antigens was used to detect antigenic determinants in normal human skin. An indirect immunoperoxidase technique was used. Eight of the MoAb detected epidermal antigens similar to patterns well known for human sera. Five of these MoAb detected determinants in the dermis, too. These observations may indicate a certain degree of similarity between the antigenic determinants occurring in M. leprae and in the human host. We propose that such a similarity on the one hand may facilitate the survical of M. leprae in the human host when the antigens are not recognized as "non-self", a situation which seems to ocuur in lepromatous leprosy, when the patients' tissues are loaded with bacteria virtually without any immune response. On the other hand, M. leprae antigens which mimic host antigens may induce an auto-immune reaction against the host's own antigens, which could explain the immune reaction in tuberculod leprosy and during a "reversal reaction" when M. leprae is not observed in the host tissues, but extensive granuloma formation occurs (AU).
