Abstract: Between 1967 and 1987 in the Southern Marquesas, a remote archipelago in French Polynesia, the detection rate of leprosy was 48.9 per 100,000 when it was 8.6 per 100,000 for French Polynesia as a whole. In 1988, a program of chemoprophylaxis of leprosy with a single dose of 25 mg/kg rifampin was implemented, and 2751 persons (98.7% of the population) were treated in the Southern Marquesas. In addition, 678 South Marquesans and 2466 members of their families living in the Northern Marquesas and in the Society Archipelago, received the same chemoprophylaxis. Among 2676 persons studied in the Southern Marquesas (97.4% of the treated population), 130 had elevated IgM anti-phenolic glycolipid-I antibodies by ELISA without any evidence of leprosy. The onset of a skin lesion of borderline leprosy in a boy 3 months after chemoprophylaxis raises the question of the nature of such a skin lesion and, indirectly, of the effectiveness of the chemoprophylaxis. Lepr;68(4):452-455, Dec., 2000. tab. Abstract: Mycobacterium leprae were isolated from a Japanese patient, and susceptibility to antileprosy drugs was examined by the mouse foot pad method. The isolate was susceptible to clofazimine and clarithromycin, and resistant to dapsone, rifampin, ofloxacin and sparfloxacin. Mutations were identified in the genes associated with resistance to these drugs. The risk of the emergence of leprosy with multidrug resistance is emphasized. (AU)^ien. Abstract: Se hace un análises de los resultados obtenidos en 789 pacientes de lepra sometidos a un tratamiento con la combinación RMP e IPD que nos permite afirmar sobre su buena tolerancia, eficacia y aceptabilidad, ya que al cabo de 4 años tenemos a 59 por ciento de ellos en etapa de control post-tratamiento, sin señales de recaída ninguno. Efectos colaterales se presentaron en el 14 por ciento del total, principalmente en relación al área gástrica, epro de orden pasajero. El 33 por ciento de los casos L y B hizo reacción del tipo ENL, de los que el 94 por ciento lo fueron de carácter leve, manejable con éxito con talidomida. Hepatitis con ictericia se vio en el 2 por ciento, (16 casos) siendo que en todos ellos se superó dicha situación con la sola interrupción de la terapia, sin dejar secuelas. En quienes hubo necesidad de reanudarla, se tomaron de nuevo sin inconvenientes hasta su finalización. Fueron registrados 5 fallecidos por causas no relacionadas a la terapia combinada. Cabe señalar la rapidez de la mejoría clínico-bacterilógico, ya bien evidente hacia los seis primeros meses de tratamiento, lo cual reviste indudable trascendencia epidemiológica.
