Abstract:A anestesia de extremidades e uma caracteristica da hanseniase; os danos neural e vascular levam a deformidade. Uma das consequencias comuns e o comprometimento ungueal, apresentando-se principalmente sob a forma de distrofia, que pode ser colonizada por fungos. A infeccao micotica facilita o aparecimento de infeccoes bacterianas secundarias, que podem desencadear reacoes hansenicas, comprometendo a qualidade de vida do paciente. Neste trabalho, estudou-se a frequencia de agentes de onicomicose em 104 pacientes hansenianos com lesoes clinicamente compativeis. Foram utilizados exame micologico direto e cultura, que foi repetida por tes vezes. Encontrou-se frequencia de 91,3 por cento de onicomicose, com predominio de leveduras (60,0 por cento). Houve discordancia entre exame micologico direto e cultura em 27,6 por cento dos casos. Em 21,6 por cento dos exames diretos microscopicamente positivos nao foi observado crescimento fungico na respectiva cultura e em 6 por cento dos casos verificou-se exame micologico direto negativo e cultura positiva. Os dermatofitos represemtam 26, 7 por cento dos isolamentos, sendo Trichophyton rubrum o mais frequente (15, 3 por cento). Os fungos filamentosos nao dermatofitos foram 13,3 por cento das cepas isoladas, com maior frequencia do genero Scytalidium (10,5 por cento). Candida parapsilosis foi a especie de levedura mais frequente (21,53) por cento), seguida de C. tropicalis (7,69 por cento) e C. albicans (7,69 por cento). Os achados surgerm que as unhas destes pacientes sao suscetiveis as onicomicoses, principalmente com predisposicao a infeccao por agentes oportunistas...(AU).

Title:High oral prevalence of Candida krusei in leprosy patients in northern Thailand ..-

Abstract:Although Candida albicans is the most common human yeast pathogen, other Candida species such as C. krusei are now recognized as emerging agents, especially in patients with human immunodeficiency virus (HIV) disease. C. krusei is inherently resistant to the widely used triazole antifungal fluconazole and poses therapeutic problems, especially in systemic candidiasis. In a surveillance study of leprosy patients (with arrested or burnt-out disease) in a leprosarium in northern Thailand, we found a rate of oral carriage of C. krusei (36 per cent) significantly (P smaller 0.05) higher than that for a healthy control group (10 per cent). Among the Candida-positive patients, 16 of 35 (46 per cent) carried C. krusei, while C. albicans was the second most common isolate (12 of 35 patients; 34 per cent). The corresponding figures for the control group were 2 of 13 (15 per cent) and 6 of 13 (46 per cent), respectively. Studies of the antifungal resistance of the C. krusei isolates from patients indicated that all except one of the isolates were resistant to fluconazole, two isolates were resistant to ketoconazole, and all isolates were sensitive to amphotericin B. Evaluation of their genetic profiles by randomly amplified polymorphic DNA analysis with three different primers and subsequent analysis of the gel profiles by computerized cluster-derived dendrograms revealed that the C. krusei isolates from patients belonged to 10 disparate clusters, despite the origin from a single locale. These nascent findings indicate an alarmingly high prevalence of a Candida species resistant to a widely used antifungal in a part of the world where HIV disease is endemic. (AU).

Abstract:The response to soluble mycobacterial antigens of peripheral blood mononuclear cells, from most normal donors, tuberculosis patients or cases of tuberculoid leprosy (TT/BT) was enhanced by the addition of indomethacin. In contrast, indomethacin caused no enhancement of the response of cells from lepromatous leprosy (BL/LL) cases. Moreover the addition of 10-5M prostaglandin E2 (PGE2) failed to inhibit the proliferative responses of cells from the BL/LL patients, although it markedly inhibited the responses of peripheral blood mononuclear cells from the other groups. The addition of PGE2 or indomethacin to cells which had been precultured for 48 hours had no significant effect on the proliferative responses of cells from any of the groups of donors. These results suggest that a normal, prostaglandin-dependent, indomethacin-sensitive regulatory mechanism is absent from the peripheral blood mononuclear cells of BL/LL patients.(AU).

Abstract:Se estudio la fagocitosis y lisis de Candida albicans y Candida pseudotropicalis por polimorfonucleares neutrofilos, en 43 pacientes hansenianos. (19 virchowianos quiescentes, 12 virchowianos reaccionales y 12 tuberculoides quiescentes) y 15 testigos sanos. El proposito de este estudio fue determinar si los polimorfonucleares neutrofilos de pacientes hansenianos presentaban alteraciones de sus sistemas enzimaticos mieloperoxidasa-dependientes (investigado mediante lisis de Candida albicans) y mieloperoxidasa-independientes (investigado mediante lisis de Candida pseudotropicalis). Los resultados indican que los neutrofilos de los pacientes hansenianos tienen una actividad candidicida similar a la de los neutrofilos de testigos sanos, (p > 0.2) en sus sistemas mieloperoxidasa dependiente e independiente.La actividad de estos sistemas enzimaticos es tambien similar en todas las formas clinicas de hanseniasis (p > 0.3). Abstract:The in vitro effect of Mycobacterium leprae suspensions on the PMN hability to phagocyting and killing Candida albicans and Candida pseudotropicalis was studied in forty-five patients of Hansen's disease and in fifteen healthy controls. Our results show no significative differences between the diferent studied groups, both for the phagocytosis and for the lysis of yeasts. There was no significant changes in the mean values of these functions after previous or simultaneously incubation with Mycobacterium leprae suspensions. Those observations confirmed that there are not alterations in the enzimatic battery of PMN in Hansen's disease patients and that the Mycobacterium leprae presence does not exert stimulating effect on this in vitro model.
