Abstract:A new rapid immuno-chromatographic test card for the detection of antibodies to the Mycobacterium leprae 35-kD protein is described. The new assay is compared in the same group of subjects with a direct enzyme ELISA method for 35-kD antibodies and with assays for anti-phenolic glycolipid-I (PGL-I) antibodies using a standard ELISA as well as the recently described [quot ]dipstick[quot ] method. Good concordance was found between the rapid methods and the corresponding ELISA methods. The detection of untreated paucibacillary leprosy by the 35-kD test card was 59% compared with 27% for the PGL-I dipstick; however, the specificity for the 35-kD test card was 90% compared with 100% for the PGL-I dipstick in an endemic population. The potential application of these new, rapid serologic methods for the diagnosis of leprosy under field conditions is discussed. (AU)^ien.

Title:A study on a possibility of predicting early relapse in leprosy using a ND-O-BSA based ELISA.

Abstract:Serological methods have been used for detecting infection with Mycobacterium leprae. We have applied a serological test to explore the possibility it could detect a bacterial relapse among patients who have been cured with chemotherapy. More specifically we used an indirect enzyme-linked immunosorbant assay (ELISA) using the natural disaccharide (ND) of the phenolic glycolipid antigen of M. leprae linked to bovine serum albumin as antigen. Antibody levels were measured in sera from normal controls, active leprosy cases, cured leprosy patients, and relapsing leprosy patients. We correlated antibody levels with the type of leprosy, the bacterial index, and with relapse among cured leprosy patients. In our hands, the ND-ELISA, when applied to screening for infection with M. leprae, had excellent sensitivity, specificity, positive and negative predictive values, and both a low false positive rate and a low false negative rate. Antibody levels gradually increased among active patients from the tuberculoid to the lepromatous end of the leprosy spectrum. There was a year-by-year fall in antibody levels in patients responding to chemotherapy. Antibody levels and the bacterial index were correlated using the Spearman's rank correlation method. Serial antibody levels were measured in 666 leprosy patients after being cured with dapsone monotherapy. Over a three year follow up, 95 multibacillary patients became antibody positive and 12 of them had bacterial relapses of their disease. In contrast, among 335 cases that remained antibody negative, only one relapse was seen. Among 44 paucibacillary cured patients who became antibody positive, there was one relapse. There were 192 such patients who remained antibody negative and one relapsed. The risk of relapse is 6.7 times higher among cured multibacillary patients compared to cured paucibacillary patients. Overall, the cumulative relapse rate among antibody positive cases was 13.7%, compared to 0.4% among antibody negative patients. We conclude that the ND-ELISA is a useful tool both for screening for early infection with M. leprae and for predicting a relapse in cured patients, particularly in cured multibacillary patients. Abstract:We comparaed the granuloma morphology and immune response of hamsters inoculated with Paracoccidioides brasiliensis (Pb) into the cheek pouch, which lacks lymphatic drainage, and the ffotpad, which is rich in lymphatics. Our objective was to better understand the modulation of Pb granuloma in an immunocompetent animal inoculated in an immunologically privileged site. The humoral immune response (ELISA) and cell mediated immunity (footpad test) became positive on days 7 and 14, respectively in animal inoculated into footpad and on days 35 and 60 in animals inoculated into the pouch. Typical epithelioid granulomas were observed at both sites on day 14. The number of fungi gradually decreased from the beginning of the experiment in footpad lesions, but only after day 35 in pouch granulomas, when cell mediated immunity was detectable. The results indicate that typical epithelioid paracoccidioidomycotic granulomas may develop in the absence of a detectable immune response; however, they are incapable of controlling fungal reproduction. Lack of lymphatic drainage delays the appearance of a detectable immune response, but with time fungi escape from the pouch, elicit an immune response and reach other organs. Our results further indicate the importance of the lymphatics in the pathogenesis of paracoccidioidomycosis. (AU).

Abstract:Foi realizada a deteccao sorologica dos anticorpos anti-HIV 1 em um grupo de 59 pacientes com hanseniase multibacilar (Grupo A), 20 dos quais apresentaram surto reacional tipo II. Foi incluida tambem a analise retrospectiva da mesma sorologia correspondente a 104 pacientes (Grupo B) com formas multi e paucibacilares de hanseniase. As amostras de ambos os grupos foram testadas utilizando dois tipos de ELISA para o HIV 1. A prevalencia da infeccao pelo HIV 1 foi significativamente superior entre os pacientes com hanseniase (1,84 por cento) que entre os doadores de sangue (Grupo Controle - 0,28 por cento). Resultados falsamente positivos nao foram detectados nos testes de ELISA utilizados(AU).

Abstract:Os testes sorológicos para diagnóstico de hanseníase, usando o glicolipídeo-fenólico, considerado antígeno específico do M. leprae têm aberto algumas possibilidades de estudo do comportamento epidemiológico desta doença. Foi realisado um estudo para medir a sensibilidade e especificidade de um teste de ELISA anti PGL-1, usando material e técnica proveniente de Cuba (UMELISA). Foram testados 84 doentes e 112 controles sadios. A sensibilidade do teste foi maior para o grupo de doentes multibacilares, sendo mais alto entre os casos classificados como virchovianos (V), seguido dos dimorfos (D). No grupo de multibacilares, considerando o limiar de reatividade de 0,200, observou-se que apenas 75,0(por cento) de pacientes V e 50,0(por cento) de D foram positivos ao teste e, no limiar de 0,300, apenas 64,3(por cento) dos V e 40,0(por cento) dos D ainda mostravam positividade. Os doentes indeterminados apresentaram maior proporçäo de soropositivos do que os tuberculóides e isto talvez traduza a polarizaçäo de alguns casos para formas multibacilares. A especificidade do teste foi de 87,5(por cento) no limiar de 0,200 e de 99,1(por cento) no de 0,300. Este teste, à semelhança de outros, apresentou alta especificidade e baixa sensibilidade, resultando em grande percentual de falso-negativos. Para uma doença em que os testes sorológicos säo escassos, apesar de näo se recomendar o uso indiscriminado do teste para triagem de casos na populaçäo geral, a comunidade de seu aprimoramento tecnológico deve ser estimulada. Conseguir-se-ia, assim, avançar na pesquisa de instrumentos cada vez mais sensíveis e específicos para melhorar o diagnóstico precoce e, desse modo intervir mais oportunamente na cadeia de transmissäo.
