Abstract:In spit of the liberal regime instituted by the Belo Horizonte Seminar (1958), our sanatoria shall preceed for many years to come with their humanitarian and social purposes, taking in socially maladjusted patients, invalids (blind, paralytic, and mental), provinding medical and surgical care for needy patients with acute intercurrent diseases, or requiring physical rehabilitation; these institutions are indispensable for therapeutic investigations in the field of new drugs or trying to perfect the existing methods of treatment. The high fugure of 85% represents patients interned in our State Institutions for leprosy, poly-infectious and incurable form, of chronic evolution and low mortality, invading the organism slowly and progressively during decades, causing social maladjustment and physical inability; this irreversible residue of leprosy hides behing the weight of mystic and millenary popularbeliefs, making it thus a plumbeoum onus sanatorium. This lepromatous population, the result of deficient control of the endemic disease in 40 years, is even more serious in view of the influence of cumulative leprosy on the expansion of the endemic disease, acting through a contagious chain, perpetuating itself throughout the decades of intra-familiar contagion (occult cases). James A. Doull, the noted American epidemiologist, after amny years of study of leprosy in the Philippines (Cebu) thus concludes: "It is thus evident that a great proportion of lepromatous ulcers of the nasal septum and the skin are cured within 48 weeks of treatment. The inference that leprosy coud not subsist but for the infectivity, spread over long periods of time, of the lepromatous cases discharging large numbers of bacilli, seems to be justified". Since the present prophylactic program is based on the figures of undetermined cases and correlated breach of the chain of familiar contagion, the preventive medicine program for leprosy views the endemic control by preventing leprosy: this i...(AU)^ien.

Abstract:Relatam os AA. a observação de paciente do sexo masculino, de 27 anos, preto, portador de lepra tuberculóide reacional, com lesões localizadas na face, de aspecto nitidamente verrucoso. Nos antecedentes mórbidos há referência à tuberculose pulmonar ainda em tratamento e na vigência da qual ocorrem as lesões da lepra T.R. Fazem o diagnóstico diferencial com a tuberculose verrucosa que pode ser afastada cl¡nicamente e pela histopatologia que foi compatível com o diagnóstico de lepra tuberculóide reacional com lesões verrucosas. Na revisão de literatura encontraram apenas 6 trabalhos referentes à lesão verrucosa no mal de Hansen, sendo o presnte, o primeiro referido com o tipo tuberculóide reacional. (AU)^ipt.

Abstract:The presente study was carried out involving 25 patients with paucibacillary leprosy who attended the outpatient department of dermatology of Father Muller's Medical College Hospital during the period January 2001 to March 2002. All the patients were examined clinically and histopathologically at the beginning and at the end of six months of MDT and relevant data recorded. Clinicopathological correlation with histopathological classification before MDT was 72% and 68% at the end of MDT in our study. At the end of treatment 4 (16%) cases were clinically active and 8 (32%) were histopathologically active. The study showed that active cases were siginificantly reduced as a result of MDT, both clinically and histopatologically. The histopathological activity that outlasts MDT may be due to the bacillary fragments that persist; but clincial activity coupled with histopathological activity seen in 2 patients at the end of 6 months of MDT was possibly an indicator of relapse and these patients and similar others need to be follewed up for a longer duration. Is this study, resolution of granuloma and clinical activity after completion of MDT were assessed (AU).

Abstract:O presente trabalho comparou dois esquemas terapêuticos em pacientes com hanseniase multibacilar. O grupo l, que recebeu o tratamento convencional (PQT-MB), foi denominado grupo de controle. O grupo 2, denominado de grupo teste, recebeu a associação de rifampicina 600mg, mais ofloxacina 400mg, mais minociclina 100mg (ROM), administrado sob supervisão, uma vez por mês. A duração total do tratamento nos dois grupos foi de dois anos. Na avaliação inicial foram realizados: exame clinico, baciloscopio e histologico. A baciloscopia e a biopsia foram repetidas no final do primeiro ano e novamente no final do segundo ano de tratamento. A avaliação clinica foi realizada mensalmente por ocasião da administração da dose supervisionada. No grupo 1 foram avaliados 14 pacientes. O indice baciloscopio (IB) antes do tratamento variou de 2 a 4,8. No grupo 2 foram estudados 12 pacientes. O IB antes do tratamento nesse grupo variou de m1,6 a 4,8. Ambos os grupos apresentavam lesões cutâneas que os caracterizavam como pertencentes ao pólo vichoviano. Histologicamente apresentavam quadro de hanseniase virchoviana ativa, exceto um paciente do grupo 2. Ao final do primeiro ano de tratamento estavam todos clinicamente melhorados, o indice baciloscopio diminuido e com quadro histologico em regressão. Essa tendencia de melhora se mantinha e na avaliação do finanl do segundo ano todos estavam clinicamente, baciloscopidamente e histologicamente ainda melhores. A análise estatistica dos parametros, baciloscopio e histologico, mostrou que não houve diferença estatisticamente significativa entre os grupos estudados, sendo, portanto os dois esquemas equivalentes. A ocorrência de reação tipo 2(eritema nodoso hansenico-ENH) foi igual nos dois grupos. no primeiro grupo 1, todos apresentaram pigmentação cutânea devido a clofazimina. Os resultados, portanto, demonstraram que o esquema com administração mensal de rifampicina, mais ofloxacina, mais minociclina tem eficácia e segurança equivalenteao esquema convencional. Além disso, tem a vantagem de não acarretar pigmentação cutânes, ser totalmente supervisionado, podendo ser utilizado como esquema alternativo.(AU).

Abstract:While from the nature of things it is impossible to prove that patients whose lesions are of the bacteriologically negative, resistant type are under no circumstances able to trasmit leprosy, available evidence is against their being a source of danger to children, who are usually highly susceptible. With regard to the juvenile type, the frequency with which such cases suddenly develop widespread, bacteriologically positive lesions makes clear the necessity of keeping children suffering from this type of the disease under close observation, remembering the danger of their suddenly becoming actively infectious cases. THere is a tendency for advanced cutaneous-type cases to develop nerve lesions in the later stages of the disease. Patients seen at this stage are often classified as neural (properly, " secondary neural"). Routine bacteriological examination may at first fail to show Mycobacterium leprae, but repeated examination will often show clumps of acid-fast bacilli in the skin, nose or gums. Patients who have at one time suffered from widespread sutaneous leprosy with bacteriologically positive lesions should, even after the disease has become quiescent and arrested, be kept from close contact with healthy people and especially with children. Such contact should be permitted only when the disease has remained arrested for several years, when the patients have shown no signs of the disease on being subjected to the iodice test, and when all parts of the skin, nasal mucosa, gums, etc., have been carefully examined for bacteria and found negative. Reference is made to the use of the leprolin test in determining resistance to invasion by Myco. leprae and in judging the degree of danger of transmission of disease. (AU).

Abstract:O Reconhecimento do grande número de pacientes com ressecamento de córnea e a inexistência de publicações neste ramo, motivou os autores a elaborar o presente trabalho. Forma examinados um total de 150 pacientes de Fevereiro a Maio/86, sendo 66% da forma Virchoviana, 16,6% forma Borderline, 16,6% foram Tuberculóide. Em 62% dos pacientes examinados apresentam ressecamento de córnea, tendo como principais causas baixa produção da lágrima, triquíase, lagoftalmo I (inicial) e avançado ectrópio. O tratamento dos pacientes durante o período foi dividido conforme a gravidade do problema em duas categorias: lubrificação da córnea com lubrificante artificial simples e lubrificação completa e em casos de triquíase usou-se no tratamento a retirada dos cílios. Pelos resultados obtidos (100% de melhora do ressecamento de córnea) observou-se o êxito do trabalho.(AU).
