Title:Leydig cell hyperplasia and the maintenance of bone volume: bone histomorphometry and testicular histopathology in 29 male leprosy autopsy cases.

Abstract:This study was conducted to determine if osteoporosis in male leprosy patients is caused by testicular atrophy. Bone volume (BV/TV), trabecular number (TbN), trabecular thickness (TbTh), and trabecular separation (TbSp) were measured in two areas in decalcified paraffin sections of lumbar bones from 29 male leprosy and 6 male nonleprosy autopsy cases. We found significant differences in the average BV/TV measurements among the 7 patients with nodular Leydig cell hyperplasia (BV/TV 12.24%) and the 22 patients without hyperplasia (BV/TV 7.35%) and 6 patients without leprosy (BV/TV 12.98%). Bone volume was maintained in patients with nodular Leydig cell hyperplasia, and we determined no clinical factor other than the Leydig cell hyperplasia that reflected the bone volume. The osteoporosis of male leprosy patients was attributed to secondary gonadal dysfunction due to testicular atrophy, and Leydig cell hyperplasia appears to preserve bone volume. (AU)^ien.

Abstract:OBJECTIVE: To study the changes in testicular aspirates and semen of patients with leprosy. STUDY DESIGN: A prospective study of 56 patients in the reproductive-age group, with no record of treatment for leprosy. Both Ridley-Jopling and WHO classification systems were used. Skin and/or nerve biopsies were performed for documentation of the diagnosis. Semen analysis and fine needle aspirates of the testes were performed. Smears from the testicular aspirates were stained with May-Grünwald-Giemsa and Ziehl-Neelsen stain. RESULTS: Five patients were unable to produce an ejaculate. Abnormal semen analysis and/or testicular aspirates were seen in 24 (42.8 per cent) patients. Eleven had oligospermia and eight azoospermia. Abnormalities in testicular aspirates ranged from hypospermatogenesis (4) through maturation arrest (1) and atrophy (11). Two patients had hydrocoele, and two had associated microfilariae. Three patients with multibacillary leprosy had type 2 reaction. Mycobacterium lepre was demonstrable in testicular aspirates from all patients with multibacillary and in three with paucibacillary leprosy. CONCLUSION: Abnormal semen analysis and/or testicular aspirates occur in a very high percentage of patients with leprosy. While this is expected for multibacillary disease, the high incidence in the paucibacillary form was surprising. With the rapid elimination of leprosy, fertility-related disability might emerge as a major problem in these people. (AU).

Abstract:Testicular germinal cell antibodies were found in forty-four out of the fifty-nine patients with lepromatous leprosy and in four out of ten patients with tuberculoid disease. A similar pattern was found in twelve out of 262 control patients and normal subjects. The antibody was found to be of the IgC class and forty out of forty-nine of these antibodies were shown to be complement fixing. Spermatozoal antibodies were detected in twelve patients, but no ovarian antibodies were found in any specimen. There was no close correlation between erythema nodosum leprosum (ENL) and testicular antibodies. it was found that the characteristic of the testicular antibody in leprosy was its ability to be absorbed by Mycobacterium BCG suspension suggesting that this is another antibody induced by infection. A similar fluorescent pattern was seen in some patients who did not have leprosy, but in these cases it could not be abolished with BCG. It is concluded that autoimmunity may be one of the factors involved in the pathogenesis of orchitis in leprosy.(AU).

Abstract:Neonatal thymectomy and neonatal thymectomy plus ATS resulted in a merked increase in susceptibility of both Buffalo and Lewis rats to infection with M. leprae. In Buffalo rats, increase in susceptibility was limited to footpad infection. In Lewis rats, this was extended to include testis infection where the organisms in thymectomized-ATS-treated rats were found to be still in the logarithmic phase of growth 15 months after inoculation.(AU).

Abstract:Twenty five patients of lepromatous leprosy were studied clinically and histopathologically for testicular involvement. Testicular pain or swelling was the commonest complaint (68%) followed by sterility (28%) and impotence (4%). Reduced testicular size associated with soft feel was observed in 76% patients. Gynaecomastia was present in 36% and altered sexual hair pattern in 24%. Eleven out of sixteen (69%) patients had oligo/azoospermia. Out of the twenty testicular biopsies 15 (75%) had definite histological evidence of leprous pathology, irrespective of testicular size, semen picture and clinical signs and symptoms. One out of three epididymal biopsies showed minimal changes. Histopathological changes varied markedly, it was not possible to categorize these into vascular, interstitial and obliterative phases.(AU).

Abstract:Basal LH, FSH, 17 beta-oestradiol and testosterone and the gonadotrophin responses to luteinizing hormone releasing hormone (LHRH) were studied in male patients with leprosy (twenty-four with lepromatous and six with tuberculoid leprosy). The mean basal LH and FSH was significantly elevated in the lepromatous group and was associated with an excessive response of both gonadotrophins following LHRH administration. The mean basal testosterone and 17 beta-oestradiol values in the lepromatous group were significantly lower than those of the tuberculoid and control groups. The abnormal gonadotrophin and sex steroid values in the lepromatous group are in keeping with the testicular atrophy and gynaecomastia accompanying this form of leprosy. However, the lack of a significant correlation between basal FSH and testicular atrophy should be noted. In addition, no correlation between any of these hormonal values and gynaecomastia could be demonstrated. The patients with tuberculoid leprosy had essentially normal hormonal profiles (except for two who had raised 17 beta-oestradiol values). This is compatible with the lack of gonadal involvement in these patients.(AU).

Abstract:Following inoculation either locally or intravenously with Mycobact. leprae of human origin, the histopathology and bacteriology of the testis in experimental mice is described. Normal mice, and mice rendered immunologically deficient by thymectomy and whole-body irradiation, were studied. Attention is drawn to a heavy bacillation of the testis in mice from both groups. Bacilli were found in and beneath the tunica albuginea, but mainly in interstitial cells and in macrophages surrounding the tubules. The percentage of solidly staining bacilli was high, and globi were frequent. The study showed that the testis in mice is particularly favourable for the lodgement and multiplication of Mycobact. laprae following either local or intravenous inoculation. The significance of this in relation to the metabolism of the leprosy bacillus and to the frequent occurrence of testicular damage in the lepromatous male patient is discussed. Title:Gonadal involvement in leprosy - study of gynaecomastia, testicular and epidedymal involvement and therapeutic efficacy of indegenous drugs.

Abstract:Sessenta pacientes masculinos de hanseniase (idade media = 27,2+ ou - 5,04 anos), selecionados aleatoriamente, foram estudados quanto ao envolvimento gonadal e a eficacia terapeutica de duas drogas nativas. Destes pacientes 34 eram casados, com duraçao media da doença de 4,17 + ou - 3,27 anos. Somente os pacientes com hanseniase virchowiana e dimorfa desenvolveram alteraçoes testiculares e de epididimos. Dor e/ou sensibilidade testicular (virchowianos = 62,5 por cento dimorfos = 30,0 por cento) foi a sintomatologia mais comum. Funçao sexual alterada foi observada em 34 (56,6 por cento) casos, enquanto 11 pacientes apresentaram padrao sexual de distribuiçao pilosa alterado e ginecomastia. O espermograna revelou azoospermia em 19 (35 por cento) dos casos e oligospermia em 16 (26,6 por cento) pacientes. Os achados histopatologicos das biopsia testicular revelaram evidencia de lesoes especificas nao relacionadas ao tamanho do testiculo, aos achados do semen e as manifestaçoes clinicas. As alteraçoes histopatologicas mostraram grande variabilidade, nao tendo sido possivel classifica-las em vasculares, intersticiais ou obstrutivas: acredita-se que as alteraçoes encontradas sejam devidas ao estado alterado das gonadas na hanseniase. A eficacia das preparaçoes terapeuticas nativas Speman e Tentex forte (Himalava) foi avaliada de um ponto de vista objetivo em 34 pacientes. Melhora subjetiva foi observada em 82,3 por cento dos casos, enquanto objetivamente o espermograma melhorou em 87,5 por cento dos casos com oligospermia. As drogas nao apresentaram efeitos colaterais e foram bem toleradas. (AU).

Abstract:Estudaram-se 58 fragmentos testiculares e o espermograma de 10 indivíduos normais e 24 hansenianos. Verificaram-se as alteraçöes gerais e precoces na histologia, a freqüência de espermátides jovens no lumem tubular e a medida dos diâmetros de 1.450 túbulos seminíferos. De acordo com os resultados obtidos, näo foi possível estabelecer uma correlaçäo qualitativa entre as descriçöes histológicas e ou quantitativa entre a morfometria, e um grupo clínico específico de hansenianos. Assim, também, a simultaneidade e a variabilidade dos achados observados em um mesmo testículo, se opöe às fases evolutivas, descritas por H. Grabstald & L.L. Swan e à classificaçäo proposta por B. Kumar et al. Os autores sugerem que as alteraçöes testiculares sejam conseqüência de uma auto-agressäo à glândula. (AU).
