Abstract:1) Se han tratado 29 casos de atrofia muscular de las manos en enfermos de lepra y se ha observado que 18 casos mejoraron clinicamente por el tratamiento con Vitamina E. 2) Se observo que las atrofias presentadas por los casos de lepromatosos difusos son los mas dificiles de modificar, al contrario de lo que pasa con los casos de forma indiferenciada. Se supone que las atrofias son mas rebeldes en la forma lepromatosa difusa porque muchos de estos casos son la resultante de una forma indiferenciada de evolucion prolongada. 3) La duracion de la enfermedad influye mucho en la reversibilidad de la atrofia. La mejoria es tanto mayor cuanto mas reciente es la enfermedad. (AU).

Abstract:Using a specific antibody (SMI 31), the state of phosphorylation of high and medium molecular weight neurofilaments (NF-H and NF-M) was studied in 22 leprous and four nonleprous human peripheral nerves by means of immunohistochemistry, sodium dodecyl sulfate-poly acrylamide gel electrophoresis (SDS-PAGE) and Western immunoblot (WB). The results thus obtained were compared with morphological changes in the respective nerves studied through light and electron microscopy. Many of the leprous nerves showing minimal pathology revealed lack of or weak staining with SMI 31, denoting dephosphorylation. Remyelinated fibres stained intensely with SMI 31 antibody. The WB analysis of Triton X-100 insoluble cytoskeletal preparation showed absence of regular SMI 31 reactive bands corresponding to 200 and 150 kDa molecular weight (NF-H and NF-M, respectively) in 10 nerves. Three of the 10 nerves revealed presence of NF protein bands in SDS-PAGE but not in WB. Presence of additional protein band (following NF-M) was seen in four nerves. Two nerves revealed NF-H band but not NF-M band and one nerve showed trace positivity. In the remaining five nerves presence of all the three NF bands was seen. Thus, 77.3% (17/22) of human leprous nerves studied showed abnormal phosphorylation of NF protein(s). The ultrastructural study showed abnormal compaction and arraying of NF at the periphery of the axons in the fibres with altered axon to myelin thickness ratio (atrophied fibres) as well as at the Schmidt-Lantermann (S-L) cleft region. Such NF changes were more pronounced in the severely atrophied axons suggesting a direct correlation. The observed well-spaced NF in the remyelinated fibres under ultrastructural study was in keeping with both intense SMI 31 staining and presence of NF triplet bands seen in WBs in four of leprous nerves that showed a large number of regenerating fibres suggesting reversal of changes with regeneration. Findings in the present study suggest that atrophy, that is, the reduction in axonal calibre and paranodal demyelination, seen in leprous nerves may result from dephosphorylation of NF-H and NF-M proteins. Abstract:O autor apresenta á Sociedade Paulista de Leprologia uma doente atacada de lepra, forma mixta C2 N1, em a qual verificou a apparição de um quadro clinico muito semelhante ao da Molestia de Recklinghausen, mas não passava de uma anetodermia em tumores, ou seja de uma anetodermia de Schweniger e Buzzi, já pelo numero de seus elementos que attingiam a 184 já pelos caracteres proprios desta forma de anetodermia que são os seguintes: atrophia com protrusão da pelle, annel fibroso peripherico, presença de tecido adiposo dentro das cavidades, infiltração sanguinea temporaria de varios elementos, localisação das lesões na face externa dos membros e alto thorax, côr das mesmas variando do amarello ao ardosio e violaceo, dôr expontanea e provocada, exacerbada pelos periodos menstruaes. Responsabiliza a leprose na produção deste quadro clinico apoiado nos exames de laboratorio, em esfregaços e cortes histologicos. (AU).

Title:Effect of thalidomide on the skeletal muscle in experimental heart failure ..-

Abstract:BACKGROUND: Tumour Necrosis Factor alpha (TNFalpha) has been shown to contribute to heart failure (CHF) progression. AIMS: We have tried to antagonise the detrimental effects of TNFalpha on skeletal muscle apoptosis, by using thalidomide, a drug that inhibits its biosynthesis. METHODS: CHF was induced in 20 rats by injecting monocrotaline, which determines right ventricle (RV) failure. After 2 weeks, when CHF developed, 12 rats were treated with thalidomide 3.5.mg/kg per day for 2 weeks. Eight had saline and served as CHF controls. RESULTS: Thalidomide failed to decrease TNFalpha and its second messenger sphingosine (SPH), but was able to prevent the shift toward the fast myosin heavy chains. In the Tibialis Anterior muscle of the thalidomide group, the degree of atrophy, the number of apoptotic nuclei and the levels of caspases, were similar to those of the CHF controls. CONCLUSIONS: Thalidomide, at the doses used in this study, which are the same employed for the treatment of tubercolosis, leprosy, AIDS and cancer in humans, did not lower either TNFalpha or SPH and only marginally influenced the apoptosis-induced muscle atrophy. Since other TNFalpha blockers are under investigation for improving the clinical status of patients with CHF, the present data could be relevant in the design of randomised clinical trials in humans. Abstract:A survey of 121 leprosy patients in the Netherlands showed eye involvement in 20% of the patients, rising to 27% if madarosis and the adnexae are included. Iris atrophy was the main lesion encountered in BL and LL patients. One typical lepromatous fundus lesion was seen. No bilateral blindness occurred. Two eyes had a visual acuity of 3/60 due to intractable scleritis and acute iritis in LL with ENL. Seven eyes of 5 patients had a visual acuity of 0.2 to 3/60, mainly due to iris atrophy in lepromatous leprosy. Prevalence of blindness in our series was low as compared with the literature.(AU).

Abstract:Clinical and pathological investigations in a group of 113 patients with leprosy of long duration demonstrated the importance of iris changes as a cause of blindness. In lepromatous leprosy the so called chronic iritis produces iris atrophy with small nonreacting pupils which exaggerate the visual impairment created by developing lens changes and corneal opacities. The cause of this chronic iritis is believed to be neuroparalytic from early involvement of the small nerves of the iris, particularly the autonomic supply. Clinical and pharmacological evidence for this theory is supported by the histological changes observed in 18 specimens of iris removed during the course of cataract surgery, with progressive atrophy of the iris preferentially affecting the dilator muscle and leading to a nonreacting miosed pupil. Further pharmacological and histological studies are to be undertaken on lepromatous patients with iris involvement with a view to establishing possible methods of prevention.(AU).

Abstract:Twenty five patients of lepromatous leprosy were studied clinically and histopathologically for testicular involvement. Testicular pain or swelling was the commonest complaint (68%) followed by sterility (28%) and impotence (4%). Reduced testicular size associated with soft feel was observed in 76% patients. Gynaecomastia was present in 36% and altered sexual hair pattern in 24%. Eleven out of sixteen (69%) patients had oligo/azoospermia. Out of the twenty testicular biopsies 15 (75%) had definite histological evidence of leprous pathology, irrespective of testicular size, semen picture and clinical signs and symptoms. One out of three epididymal biopsies showed minimal changes. Histopathological changes varied markedly, it was not possible to categorize these into vascular, interstitial and obliterative phases.(AU).

Abstract:Foram examinados 332 doentes de hanseníase no Hospital Arnaldo Pezzutti Cavalcanti. Dentre estes, 272 apresentavam o tipo virchowiano, 35 o tuberculóide e 25 eram do grupo dimorfo. Entre eles foram encontrados 6 virchowianos e um dimorfo com lesoes anetodérmicas. Dentre estes, 5 apresentavam características clínicas semelhantes à A.S.B., com protusoes de pele, atróficas, apergaminhadas, arredondadas ou ovaladas. Os outros dois mostravam quadro clínico com grandes placas de lesoes salientes, atróficas e arredondadas, sem confluir. A palpaçao, em ambas, sentia-se, na periferia, um anel herniário. Pressionando-se a pele com o dedo indicador, ela era facilmente deprimida, retornando imediatamente à posiçao inicial logo que se deixava de fazê-lo. Os diâmetros variavam de 0,5 a 4,5 cm, seguindo as linhas de clivagem da pele. Como nao se obteve informaçao, sobre a fase inicial da doença, nao foi possível afirmar a presença de sinais inflamatórios prévios. O exame histopatológico à MO e à ME sustenta o diagnóstico de anetodermia. As reaçoes sorológicas para sífilis, especificas e inespecíficas, foram negativas(AU).
