Abstract: Histological and biology research of the liver in 4 patients affected by lepromatous leprosy is reported. It shows the constant presence of the characteristics lesions that make possible an exact diagnostic and aetiological definition. In one case, controlled after 6 months after admission into the leprosarium, the hepatic lesions were still present without any clinical manifestations of leprosy. In all cases the Hansen's bacilli were constantly present in the bioptic fragements even after a long period of treatment with the usual therapy employed for leprosy. (AU). Abstract: Se revisan las lesiones hepáticas encontradas en 40 enfermos autopsiados, afectos de lepra lepromatosa. Estas lesiones aparecieron en el 60 por 100 de los casos. La hepatitis lepromatosa no evoluciona a la cirrosis (aunque ésta aparece independiente en el 25 por 100 de los casos) ni está en relación con el depósito de amiloide. Este, a su vez, aparece en el 27,5 por 100 de los casos y tampoco provoca cirrosis. Tanto para la aparición de amiloidosis como de hepatitis lepromatosa es imprescindible una evolución larga de la enfermedad con leprorreacciones frecuentes e intensas. (AU). Abstract: An iron-containing blood pigment was found in the affected organs and tissues of all cases of leprosy that I examined. In the nodular form of the disease this pigment is deposited in strikingly large quantities, while in the maculo-neural and tuberculoid forms the quantity is very small. The cause of the pigment formation may be (a)erythrocyte destruction in the general blood stream, or (b) some process of local character. The generally accepted view that the leprosy bacillus does not excrete toxic substances that may act on the cells of the organisms seems to be against the first of these theories. However, this view is based entirely on clinical data and not on experimental immuno-biological studies. It seems quite possible that products of bacillary activity may be fixed by the lipoid membrane of the erythrocyte and destroy them. In favor of the second theory, that the pigment is of local origin, is my observation that erythrophagocytosis is found with some constancy in the lesions. It is true that in cases of the nodular form phagocytosis is demonstrated with great difficulty, though the pigment is deposited in large quantities. However, in some of the cases in which there were lesser quantities of pigment, that did not interfere with the clearness of the picture, phagocytosis could be observed. At any rate, the question must be regarded as an open one, requiring further study... (AU). Abstract: Liver leprosy and liver function tests were performed on 20 cases of leprosy, (4 tuberculoid, 7 borderline and 9 lepromatous types). The histopathological changes seem to appear in a chronological sequence, the first change in all types being a histiocytic response. Further evolution and development into epithelioid granulomata in tuberculoid and borderline cases, and foam cell collections in the lepromatous types seemed to depend mainly on the response of the host. Lowered efficiency of liver function was noticed in borderline and lepromatous types. There was no direct correlation between these tests, the duration of disease, and the histophatological changes observed.(AU). Abstract: Se estudió un grupo de 86 enfermos tuberculosos que recebieron esquemas terapèuticos con Isoniacida y Rifampicina y se evaluó la acción de esas drogas sobre el hígado. Para ello, antes de iniciar la quimioterapia, se praticó una biopsia hepática y se dosaron transaminasas, fosfatasa y bilirrubina séricas; al finalizar el tercer mes de tratamiento se repitió una biopsia hepática. Los tests funcionales se repitieron mensualmente durante todo el tratamiento. El grupo I (10 casos) recibió Isonacida, Ethambutol y Estreptomicina. En la biopsia inicial hubo 1 caso con transformación grasa y 1 con necrosis focal; al 3er. mes persistía la necrosis focal en 1 caso y apreció en otros 3. El grupo II (14 casos) recibió Rifampicina, Ethambutol y Capreomicina.(AU).
