Abstract:The region of conserved synteny on mouse chromosome 11/human 17q11-q21 is known to carry a susceptibility gene(s) for intramacrophage pathogens. The region is rich in candidates including NOS2A, CCL2/MCP-1, CCL3/MIP-1x, CCL4/mip-1b, CCL5/RANTES, CCR7, STAT3 and STAT5A/5B. To examine the region in man, we studied 92 multicase tuberculosis (627 individuals) and 72 multicase leprosy (372 individuals) families from Brazil. Multipoint nonparametric analysis (ALLEGRO) using 16 microsatallites shows two peaks of linkage for leprosy at D17S250 (Z score 2.34; P=0.01) and D17S1795 (Z 2.67; P=0.004) and a single peak for tuberculosis at D17S250 (Z 2.04; P=0.02). Combined analysis shows significant linkage genes contribute, 49 informative single nucleotide polymorphisms were typed in candidate genes. Family-based allelic associationtesting that was robust to family clustering demonstrated siginificant associations with tuberculosis susceptibility at four loci separated by intervals (NOS2A-8.4Mb-CCL18-32.3kb-CCL4-6.04 Mb-STAT5B) up to several Mb. Stepwise conditional logistic regression analysis using a case/pseudo-control data set showed that the four genes contributed separate main effects, consistent with a cluster of susceptibility genes across 17q11.2 (AU).

Family-based allelic association testing that was robust to family clustering demonstrated significant associations with tuberculosis susceptibility at four loci separated by intervals (NOS2A-8.4 Mb-CCL 18-32.3 kb-CCL4-6.04 Mb-STAT5B) up to several Mb. Stepwise conditional logistic regression analysis using a case/pseudo-control data set showed that the four genes contributed separate main effects, consistent with a cluster of susceptibilitty genes acros 17q11.2 (AU).

Abstract:Leprosy, a chronic infectious disease caused by Mycobacterium leprae, affects an estimated 700,000 persons each year. Clinically, leprosy can be categorized as paucibacillary or multibacillary disease. These clinical forms develop in persons that are intrinsically susceptible to leprosy per se, that is, leprosy independent of its specific clinical manifestation. We report here on a genome-wide search for loci controlling susceptibility to leprosy per se in a panel of 86 families including 205 siblings affected with leprosy from Southern Vietnam. Using model-free linkage analysis, we found significant evidence for a susceptibility gene on chromosome region 6q25 (maximum likelihood binomial (MLB) lod score 4.31; P = 5 x 10(-6)). We confirmed this by family-based association analysis in an independent panel of 208 Vietnamese leprosy simplex families. Of seven microsatellite markers underlying the linkage peak, alleles of two markers (D6S1035 and D6S305) showed strong evidence for association with leprosy (P = 6.7 x 10(-4) and P = 5.9 x 10(-5), respectively). Abstract:Each year an estimated 600000 new leprosy cases are diagnosed worldwide. The spectrum of the disease varies widely from limited tuberculoid forms to extensive lepromatous forms. A measure of the risk to develop lepromatous forms of leprosy is provided by the extent of skin reactivity to lepromin (Mitsuda reaction). To address a postulated oligogenic control of leprosy pathogenesis, we investigated in the present study linkage of leprosy susceptibility, leprosy clinical subtypes, and extent of the Mitsuda reaction to six chromosomal regions carrying known or suspected leprosy susceptibility loci. The only significant result obtained was linkage of leprosy clinical subtype to the HLA/TNF region on human chromosome 6p21 (P(corrected)=0.00126). In addition, we established that within the same family different HLA/TNF haplotypes segregate into patients with different leprosy subtypes directly demonstrating the importance of this genome region for the control of clinical leprosy presentation. Abstract:A major susceptibility locus for leprosy has recently been mapped on chromosome 10 (10p13) by genome-wide linkage analysis. Microsatellite markers from this genome screen that showed suggestive evidence of linkage to leprosy were evaluated in an additional 140 families with affected sib pairs. A second region of linkage has thus been identified on chromosome 20 (20p12). The peak of linkage lies at marker D20S115, which has a significant single-point maximum logarithm of odds score of 3.48 (P=.00003). Transmission disequilibrium testing of the microsatellite markers in 20p12 showed that the marker D20S835 is associated with protection against leprosy (P=.021), which suggests that a locus controlling susceptibility lies close to this marker. Abstract:O objeto deste estudo está delimitado pelo desenvolvimento histórico das políticas sanitárias em hanseníase e o processo de construção da cidadania dos indivíduos que se submeteram a esse controle sanitário. Três recortes teóricos nos orientaram: Saúde e sociedade - prática médica e políticas sociais; Cidadania, democracia e a constituição do sujeito histórico social; e Estigma como categoria entre hanseníase e cidadania. A tentativa de análise abrangeu 60 anos de história da Colônia Santa Izabel. Trata-se de um estudo do tipo exploratório, de natureza qualitativa, para o qual usamos dois instrumentos: história de vida e análise documental. O confronto entre políticas sanitárias e movimentos dos hansenianos nos permitiu estabelecer três cortes históricos. O primeiro, da institucionalização da Colônia Santa Izabel, é marcado pelo isolamento compulsório, que gerou exclusão social e cerceamento de liberdade. Os doentes resistiram ao controle coercitivo com fugas e modos próprios de viver. O segundo período caracteriza-se pela possibilidade de cura com a dapsona e a adoção do isolamento seletivo. Os doentes e seus familiares empreenderam a construção do Arraial de Limas, na periferia da Colônia, num processo de resistência e solidariedade. O terceiro período coincide com a desativação das colônias e a elaboração de um novo discurso: "doença igual as outras". No plano dos doentes surge o Movimento de Reintegração do Hanseniano, cuja matriz discursiva para a sociedade e o Estado é a luta contra discriminação e pela reintegração social. Os doentes internados na Colônia procuram incorporar aos direitos "adquiridos" através da tutela do Estado, o direito de ter direitos sobre o espaço da Colônia, seu novo "esquadrinhamento", incluindo a legislação dos lotes, a posse da terra, a preservação do patrimônio histórico e cultural, a convivência, a paz e a solidariedade. A possibilidade de transformar a Colônia num bairro comum, questões como espaço, tempo, possibilidades sociais, tutela, liberdade e cidadania, expõem uma contradição histórica desse grupo social representada por categorias tais como "antigo" e "novo", "dentro e fora", lepra e hanseníase. Concluimos que os fenômenos sociais se definem na articulação que se estabelece entre o campo socialmente estruturado e as determinações cotidianas das experiências singulares próprias dos sujeitos envolvidos nas tramas. Estudos dirigidos para grupos sociais excluídos e marginalizados, como os hansenianos, marcados(AU).
