Abstract:Rheumatic manifestations are a common feature of leprosy. The present study was performed to assess the incidence of rheumatic manifestations in leprosy. Methods: seventy cases of leprosy attending the indoor and outdoor departments were studied. Results: Out of the 70 cases studied, rheumatic manifestations were seen in 61.42% of cases: arthritis in 54.28% and soft tissue rheumatism in 17.14%. Enthesitis was seen in 2.84% of cases. The incidence of arthritis in lepra reactions was 57.57%. Conclusions: Musculoskeletal manifestations can occur at any time during the infection. Articular inflammation in leprosy usually occurs in reactive states, particularly erythema nodosum leprosum (ENL). Leprosy can present with arthritis as the first symptom.

Title:The relationship of the latex fixation test to the clinical and serologic manifestations of leprosy ?-

Abstract:Forty-four positive reactions to the latex fixation tests were obtained in the serum of 101 leprosy patients, twenty-four using the euglobulin fraction. The result of the sheep cell agglutination test was positive in fourteen of ninety-five of the same serum samples. Serologic studies suggest that the agglutinating factors in serum from patients with leprosy are similar if not identical to the so-called rheumatoid factors. Duration, type of course and severity of leprosy appear to be unrelated to the presence of positive reactions to serologic tests. The incidence of other diseases found in association with leprosy has been recorded and their relationship to the latex fixation test and to the sheep cell agglutination test is discussed. From the results of this study, it is not possible to define the significance of rheumatoid factors in serum from patients with leprosy. It is concluded that they probably play a non-specific role with respect to the clinical course.

Abstract:É apresentado o caso de uma paciente que há 10 anos iniciou poliartrite simétrica evoluindo de maneira progressiva. Ela foi diagnosticada e tratada como artrite reumatóide durante todo esse período em vários serviços de Reumatologia. Em fevereiro de 2002 foi atendida em clínica particular por um dos autores que sugeriu a hipótese de hanseníase, pois apresentava, além do quadro articular, infiltraçäo difusa e discretos nódulos e pápulas nos membros. Encaminhada ao Instituto Lauro de Souza Lima o diagnóstico de hanseníase virchoviana foi confirmado e iniciado tratamento específico com poliquimioterapia para paciente multibacilar. Os autores discutem a possibilidade da paciente ter somente hanseníase ou também artrite reumatóide.(AU).

We report the first 2 cases of leprosy developing after treatment with infliximab. After discontinuation of infliximab, both patients developed type 1 ([quot ]reversal[quot ]) leprosy reactions. Abstract:Se comentan los estudios en convivientes íntimos de enfermos de Lepra, donde se observó una relación inversa entre grado y tiempo de exposición al Mycobacterium leprae (MI) y la magnitud de la respuesta inmune in vivo hacia el bacilo (reacción de Mitsuda); esta variable por otro lado, se halla influenciada por un importante grado de determinación genética. En un modelo murino experimental, la artritis por adyuvante, también se comprobó que el tratamiento con MI inducia una población de células esplénicas supresoras, con actividad regulatoria sobre la respuesta inmune celular. Se plantean además, las posible alteraciones inmunorregulatorias involucradas con la excesiva respuesta humoral que sirve de sustrato para la generación del episodio de eritema nudoso leproso (ENL) (AU).

Abstract:This overview is designed to introduce the reader to the broad spectrum of rheumatic syndromes associated with certain fungal and parasitic diseases, brucellosis and leprosy. Musculoskeletal disorders caused by fungi are uncommon and difficult to diagnose, particularly in the early stages. Deep fungal infections involve-in order of frequency-bone, soft tissues and joints. Rare but well-defined rheumatic syndromes occur in a variety of parasitic diseases. Brucellosis remains a major challenge to both human public health and clinical acumen in many countries today. The arthritis of leprosy deserves wider recognition and study as it contributes to disability and may hold clues to the mechanism of inflammation in other rheumatic disorders. All of the above may afflict residents in endemic areas and occasionally appear far from the source of origin in travellers or migrants, and usually cause diagnostic confusion. (AU).

Abstract:BACKGROUND: Eales disease (ED) is an idiopathic retinal vasculitis affecting young adult males. We have earlier reported the identification, purification and partial characterization of a novel 88 kDa protein found in the serum of patients with ED. The aim of the present study was to look for the 88 kDa protein in serum samples obtained from cases of retinal vasculitis mimicking ED and in other systemic inflammatory diseases. MATERIAL/METHODS: Serum samples from healthy volunteers and from patients with ED, uveitis, parsplanitis ocular sarcoidosis, toxoplasmosis, leprosy, diabetic retinopathy, viral hepatitis, and rheumatoid arthritis were analyzed for the presence of the 88 kDa protein by polyacralymide gel electrophoresis (PAGE). The immunological identity of the 88 kDa protein found in ED and in other diseases was investigated by Western blot. Immunohistochemistry was performed on epiretinal membranes (ERM) obtained from ED patients to localize the 88 kDa protein. RESULTS: 88 kDa protein were detected in serum samples obtained from patients with posterior uveitis, tuberculosis, leprosy and rheumatoid arthritis. The 88 kDa protein found in serum from patients with ED is immunologically identical to that found in other systemic inflammatory conditions. 88 kDa protein was localized in inflammatory cells and in nonvascular endothelium in ERMs obtained from patients with ED. CONCLUSIONS: We have identified a novel acute phase reactant, which is elaborated in ocular and systemic inflammatory conditions other than Eales disease. Further work is necessary to decipher the precise role of the 88 kDa protein in the pathophysiology of these inflammatory diseases. (AU).

Abstract:Deux observations d'arthrosynovite survenant au cours d'une << réaction lépreuse >> sont rapportées: l'une concerne un cas de lèpre nerveuse où une monoarthrite d'un genou succède à une intradermo-réaction de lépromine; l'autre un cas de lèpre lépromateuse où, quelques mois après l'administration de disulone, survient une atteinte polyarticulaire au cours d'un érythème noueux. Divers cas relevés dans la littérature sont analysés. Les désorders immunitaires observés au cours de la réaction lépreuse fournissent l'explication la plus satisfaisante. La rifampicine, utilisée avec succès dans l'un des cas rapportés, completè lers moyens thérapeutiques de la réaction lépresuse où corticoides, thalidomide, lamprène, sont doués d'une réelle efficacité.(AU).

Abstract:Articular manifestations occur in approximately 1% of cases of leprosy, sometimes at onset. They consist in a highly inflammatory polyarthritis, fairly similar to that seen in rheumatoid polyarthritis. They often herald a reactive leprous exacerbation and are dependent upon immunologic disturbances in brittle leprosy (mainly lepromatous). Joint pain should be differentiated from neurologic pain resulting from peripheral neuropathy which is often concomitant. Leprosy should be considered among the causes of polyarthritis, especially in immigrants, but also in residents who have travelled to areas where leprosy is endemic.(AU).

Abstract:Leprosy is a rare disorder in the United States, yet the rheumatic features of the disease appear to be common and frequently are the primary complaint. We observed rheumatic syndromes of erythema nodosum leprosum (ENL) occurring with and without arthritis, a swollen hands syndrome, cutaneous vasculitis, or myositis in the majority (15 of 21) of our patients. These syndromes were distinctive, sometimes dramatic, and appeared to mimic idiopathic rheumatic diseases, substantially delaying an accurate diagnosis of leprosy in some patients. These complications were the major cause of morbidity in our 21 patients and became dominant clinical problems requiring additional chemotherapy....(AU).
