Abstract:A retrospective study was done at the Leprosy Control Unit (LCU) in Durgapur of Burdwan district, West Bengal, to determine the relapse rate following multidrug therapy (MDT). A total of 1581 patients (1276 PB and 305 MB) completed MDT regimens during a period of 5 years as per WHO recommendations and National Leprosy Eradication Programme (NLEP) guidelines. The treated patients were kept under surveillance as per NLEP guidelines and searched for relapses. The results of MDT were compared with those of pre-MDT (monotherapy) era at the same centre (total: 405 patients; PB-373, MB-32) andalso with those of the Leprosy Clinic in Gopalpur (only dapsone was given to a total of 189 patients, PB-167, MB-22) Following monotherapy, the relapse rate was 10.06% at the Gopalpur Leprosy Clinic and 12.4% at the Dargapur LCU during the 2 years (PB) and 5 years (MB) of surveillance, whereas following MDT no relapse case was encountered both in PB and MB cases during the surveillance periods recommended by WHO. The results of this study are comparable with those of ohter studies. Though a few studies showed relapses during long-term surveillance beyond the periods recommended by WHO, it is once again established that MDT can prevent relapse in leprosy (AU).

Abstract:The question of the prospect for absolute cure, oron the other hand that of transformation to the lepromatous type, of active major tuberculoid cases, especially those that at one time or another have been found bacteriologically positive, is discussed. Out of twelve such cases observed at Cebu for several years, no less than seven have relapsed at one time or another after apparently complete recession of the disease. In one instance there had been a period of five years without trouble. It is suggested that the feature of relapse in tuberculoid leprosy is worthy of more attention than it has received. Lepromatous transformations has not as yet occurred in any of these cases. Two of them, however, have persisted for three years and more in an unstable, borderline condition, the picture dominated by repeated papular eruptions, the lesions atypical clinically and histologically. Another borderline case of decidelly different aspect seen in China is cited; it presented at the same time apparently lepromatous, tuberculoid and intermediate lesions and was looked upon as transitional, yet later recession occurred as is common in ordinary tuberculoid leprosy...AU.

Abstract:Paraneoplastic dermatoses comprise a heterogeneous group of noninherited skin conditions that manifest internal malignancy. Abstract:Se comparan dos grupos de pacientes MB: uno tratado hasta negatividad en frotis (TSN) y el otro con terapia de duración fija (24 dosis MB OMS) (FDT), para determinar los índices de recidivas durante el tratamiento y el período post-tratamiento, encontrando 20 recidivas (2.04/100 pacientes/año) durante el seguimiento de 980.2 personas/año en 260 pacientes tratados en FDT. En el otro grupo , 301 pacientes recibieron tratamiento hasta negativización, detectándose 12 recidivas en 1085.46 personas/año (1.10/100 pacientes año). La comparación de los índices de supervivencia (sin recidivas) revela que aunque no hay diferencia significativa hasta los 4 años, el riesgo de recidiva es significativamente mayor en el grupo FDT durante un período mayor de seguimiento. Además, cuando se compara en los pacientes la carga bacilar inicial, se observa un índice de recidivas mayor en el grupo con IB≥4, significativamente mayor (p<0.01) que en el grupo FDT comparado con el grupo que recibe tratamiento hasta la completa negativización del frotis (4.29 años 1.27/100 pacientes/años). Todos los pacientes con recidivas respondieron al tratamiento con la misma combinación de medicamentos, indicando qu la exacerbación de su condición es por insuficiente tratamiento. Se sugiere para prevenir o reducir recidivas, el continuar cuando sea posible el tratamiento hasta negativización del frotis, al menos en pacientes con IB elevado. (AU).

Abstract:Areas of low endemicity of Hansen's disease, such as Texas, California, and Hawaii, exist due to immigration and rare autochthonous infections. Managing this disease in these areas of low endemicity is difficult, especially in observing for relapse. The accurate diagnosis of relapse is imperative so that appropriate therapy can be promptly reinstituted and unnecessary treatment can be avoided. To assess treatment failures in an area of low endemicity, we retrospectively evaluated 113 patients with Hansen's disease treated in southern Texas. Of 57 patients who completed therapy, 11 were later restarted on medications for this disease for presumed relapse. However, nine of the 11 were found not to have true relapses of Hansen's disease. The accurate diagnosis of relapse of this disease is essential not only in the individual patient but also for prospective treatment trials to establish best practices. (AU).

Abstract:The 1,659 non-leprous people in a Micronesian population experiencing an annual leprosy incidence rate of about 7/1,000 were offered 15 acedapsone (DADDS) injections during 1967--1970 for leprosy prevention purposes. Subsequent annual surveillance showed an initial cessation of new cases during the 3-year DADDS campaign, followed by a resumption of cases thereafter at a yearly level of about 2/1,000 with a longer pause and slower rise among those who received the full regimen. A secondary wave of cases that has occurred since 1973 among children born after 1968 shows that post-campaign transmission occurred, probably principally from relapsing multibacillary cases with onset before the campaign. Recommendations are made for a balanced, long-term control program with DADDS preventive treatment limited to contacts of multibacillary cases.(AU).

Abstract:Rectal biopsy in 190 inpatients at a leprosy hospital in the Highlands of Papua New Guinea disclosed 16 patients with secondary amyloidosis. This represented 20% of the patients who had had polar lepromatous leprosy (L.L.) for more than 2 years. Patients with amyloidosis characteristically had either a history of recurrent attacks of erythema nodosum leprosum (E.NH) reactions or chronic trophic ulcers. Levels of the serum component (protein SAA) antigenically related to the amyloid fibril protein AA were monitored, at varying intervals for three months, in lepromatous patients with E.N.L. reaction. The SAA levels rose during E.N.L. reactions in parallel with the neutrophil count. SAA occurred with greatest frequency among patients with LH, while most non-lepromatous patients with detectable SAA had chronic trophic ulcers. The correlation between raised neutrophil count and elevated SAA concentration, observed in this and other studies, suggests that neutrophils are associated with the production of SAA.(AU).

Abstract:Plantar ulcers present a serious problem in the management of leprosy. After studying the mechanism of ulceration and the causes of their indolence, it appears that the treatment of the disease by antileprotic drugs is not sufficient. The underlying causes, such as anaesthesia, bony deformities and paralysis, should usually be treated by surgical means. Anaesthesia can be reversed by decompression of the posterior tibial nerve if this is done early. Deformities should be corrected by procedures on the soft tissues or bones, and chronic infection eradicated to prevent recurrence of the ulceration. A combination of procedures may sometimes be needed.(AU).

Abstract:Uma variedade de manifestações dermatoneurológicas e psicossociais tem sido observadas em pacientes hansenianos, mesmo após o tratamento específico. É por esse motivo que, no presente trabalho, foram estudados pacientes hansenianos que já haviam recebido alta terapêutica no período de janeiro de 1987 a fevereiro de 1995. O estudo foi realizado no Ambulatório de Dermatologia do Hospital das Clínicas da Universidade Federal de Minas Gerais, Brasil. Avaliou-se o estado dos pacientes sob os aspectos clínicos e neurológicos, considerando as diferentes formas de tratamento e a classificação da hanseníase. Para tanto os pacientes foram avaliados em quatro períodos diferentes, antes do tratamento, momento da alta terapêutica, após o tratamento e exame proposto (realizado pela autora). Uma avaliação do aspecto psicossocial dos pacientes considerados neste estudo foi realizada no exame proposto. Foram analisados 243 prontuários e, embora tenham sido enviadas cartas de convocação para todos os pacientes, apenas 122 compareceram ao exame, sendo que, destes, 41 o fizeram de forma espontânea. Dos 122 pacientes examinados, 34,5 por cento eram paucibacilares e 65,5 por cento multibacilares, verificou-se também que 52,9 por cento haviam recebido esquema poliquimioterápico; outros 25,6 por cento esquema DNDS que, posteriormente, foi substituído por poliquimioterapia; e os restantes 21,5 por cento haviam recebido apenas esquema DDS. Foi observado somente um caso de recidiva. Observou-se, também, uma ocorrência maior de reação tipo I/II, de troncos nervosos espessados e de grau de incapacidade diferente de zero entre os pacientes multibacilares. Através da avaliação psicossocial, verificou-se que a maioria dos pacientes havia contado para a família sobre sua condição de portador de hanseníase. Pôde-se também constatar que um grande número de indivíduos que não se sentiam curados foi encontrado no grupo de pacientes que havia sofrido modificação terapêutica e naqueles que apresentaram reações. Quanto à auto-avaliação sobre cura da hanseníase, não se observaram diferanças significativas entre os pacientes com grau de incapacidade 0 ou 1 em relação aqueles com grau de incapacidade 2 ou 3. Observou-se ainda neste estudo que um grande número de pacientes se auto-discrimina. (AU).
