Abstract:Leprosy is an infectious disease for which humans are considered the only source of infection. The major hindrance in leprosy control and thus in reaching the elimination goal is that numerous leprosy cases remain undetected for a long time. Many of these patients are a continuous source of infection and, and hence perpetuate transmission. The goal of the World Health Organization (WHO) is to eliminate leprosy as a public problem by the year 2000; that is, to reach as a global prevalence of <1 per 10,000 people. The epidemiological data generated routinely by health services are greatly influenced by their policies and activities. The data do not, however necessarily reflect the true situation in the field. Information on the magnitude of the leprosy problem in any one area is important for the health services with regard to their planning, monitoring and evaluation of leprosy control activities. Our studies have suggested that the high prevalence of antibodies in children may be indicative of the active transmission of M. leprae in their surroundings. The prevalence of these antibodies may also be important for leprosy control programs in order to detect new patients as early as possible and in an effective and sustainable manner. Based on PCR data, it seems that the environment also plays an important role in the transmission of leprosy in endemic areas. The results of our study show that contact with a leprosy patient is the major determinant in the incidence of leprosy and that this concept shows similarities with the "stone-in-the-pond" principle of tuberculosis transmission in concentric circle around patients. Title:Incidencia de lepra en el municipio de Nuevitas, Cuba: estudio de 25 años (1968-1992).

Abstract:Se estudian 141 casos de lepra que representan el 100% de la incidencia de esta enfermedad en el municipio de Nuevitas, provincia de Camagüey, Cuba, durante los últimos 25 años (1968-1992). Se observó que el promedio anual de casos nuevos en los períodos estudiados, que corresponden a los programas de lepra implantados en el país con diferentes tratamientos, disminuyó gradualmente de acuerdo con el medicamento recibido, según el programa de control vigente, en un 50% (8 a 4); igualmente las tasas de incidencia disminuyeron de 46'3 a 8'8 x 100.000 habitantes. La forma clínica más frecuente fue la LI (41'1%) y la menos notificada la LD (11'4%). El menor porcentaje de casos diagnosticados se encontró en los menores de 15 años (2'8%), mientras que el más afectado fue el de 45 a 59 años (28'4%). La LL predominó en el sexo masculino y la DL en el femenino. (AU).

Title:Recherche systematique de parasites chez les lepreux au Mali Systematic search for parasites among patients with leprosy in Mali-

Abstract:Practice of multidrug therapy in leprosy (combination Dapsone + Rifampicine + Clofazimine) established since 1981, has significantly reduced the incidence of the disease. However, immunosuppression due to treatment of multi-drugs therapy induced adverse reactions with glucocorticoid and the change in host immune response due to the leprosy itself, might increase the risk of parasitic infections. To test this hypothesis, we carried out a case-control study at the "Institut Marchoux" in Bamako. Stool and urine samples from all patients included in the study were examined for parasites identification. In addition, we performed thick and thin blood film to identify malaria infection and skin biopsy (snip) to detect onchocerciasis. A total of 121 cases of leprosy and 219 controls aged 10-84 years old were included in the study from March 1999 to February 2000. Sixty two percent (n = 121) of cases were treated with glucocorticoid. The prevalence of infection due to Entamoeba coli and Entamoeba histolytica were higher in cases than in controls (p = 0.02). The prevalence of infection due to hookworms was higher in cases than in controls. There was no difference of the infections to the other intestinal parasites. Three cases of cryptosporidiosis and one case of isosporosis were observed in leprosy group vs none in the control group. There was no significant difference between cases and controls with regard to prevalence of Schistosoma haematobium, Trichomonas vaginalis and Onchocera volvulus. The prevalence of Plasmodium falciparum was 4.9% (6/121) in the leprosy case and 7.8% (17/219) in the control group. In conclusion, despite the corticotherapy and immunosuppression due to leprosy, there was no difference in prevalence of pathogenic parasites. Entomoeba coli, Entamoeba histolytica which have significantly higher prevalence among the cases were not pathogen therefore there was no higher risk of severe intestinal parasitosis among the cases of leprosy. Treatment with glycocorticoid in patients with leprosy did not suggest any impact on the prevalence of this parasitic infections. In addition, multidrug therapy did not show any effect on the carriage of Plasmodium falciparum. (AU).

Abstract:Vanuatu, which formerly was known as the New Hebrides, is an archipelago in the southwest Pacific Ocean with a tropical humid climate. The national health system and the state of health of the population generally are satisfactory. The main diseases of public-health importance are malaria and tuberculosis. Their epidemiology and control are discussed. An epidemic of Plasmodium falciparum malaria, which commenced in 1980, is described; this possibly can be explained by an increase in chloroquine-resistant P. falciparum over the same period. Other diseases also are reviewed. Many tropical diseases of great significance elsewhere are not of much significance in Vanuatu. Medical advice for visiting travellers is provided. (AU).

Abstract:A total of 415 wild 9-banded armadillos from the East Atchafalaya River Levee (Point Coupee, LA) were collected over 4 years to estimate the incidence and prevalence of Mycobacterium leprae and Trypanosoma cruzi and to discern any relationship between the 2 agents. M. leprae infections were maintained at a high steady prevalence rate year to year averaging 19 per cent. T. cruzi antibody prevalence remained relatively low, averaging 3.9 per cent, and varied markedly between years. Prevalence rates were independent, with only 3 armadillos coinfected with both agents. M. leprae incidence density ranged from 0.47 to 3.5 cases per 1,000 animal-days, depending on case definition, confirming active intense transmission of M. leprae among armadillos. No incident T. cruzi cases were found. These infections seem to occur independently and may be used in comparisons to understand better factors that may influence transmission of these agents. (AU).

Abstract:Leprosy is a chronic infection of the skin and nerves caused by Mycobacterium leprae. Erythema nodosum leprosum (ENL) is a reactive state in lepromatous leprosy. Thalidomide has been used to treat ENL since the 1960s. One of its mechanisms of action is anti-inflammatory through selective inhibition of the pro-inflammatory cytokine TNF-alpha produced by monocytes. (AU).

Abstract:Leprosy has yet a great impact on public health in Haiti. A study was carried out in Haiti from 1977 to 1999. On 2,160 registered cases, mostly are paucibacillaries: 412 cases are under 15 year old patients and 1,306 more than 15 year old. Multibacillaries cases are 85 in under 15 year old patients and 357 in more than 15 year old. By the improvement of sanitary conditions, detecting cases and multidrug therapy, leprosy had decreased during these last years, with an incidence from 0.1 to 0.2 per cent. However, with the increasing poverty and political disorders, the control of leprosy remains difficult in Haiti. (AU).
