Title:La réaction de fixation du complément avec l'antigène de Witebsky, Klingenstein et Kuhn dans la lèpre.

Abstract:En considérant les résults obtenus en pratiquant la réaction de Witebsky, Klingenstein et Kuhn avec le sérum sanguin de 107 lépreux, nous voyons que la réaction a été positive sur 100 pour cent de 12 cas de lèpre cutanée et 35 de lèpre mixte, et sur 80 pour cent de 60 cas de lèpre nerveuse. Il se pourrait que ce dernier pourcentage soit supérieur si nous avions examiné sans distinction n'importe quel cas de lèpre nerveuse, et non comme cela a été fait, en recherchant soigneusement les cas des lésions extinctes dont nous disposions. Les réactions des malades de la lèpre nerveuse ont été moins intenses que celles de la lèpre cutanée mixte. Des 84 sérums de personnes ayant un contact continuel avec des lépreux, la plus grande partie représentée par des fils et conjoints de malades, 53 on donné une réaction negative, 4 douteuse, et 27 positive. Des cas négatifs, 9 ont présenté quelques symptômes qui pourraient être liés à la lèpre....(AU).

Abstract:Sequential estimates of the levels of circulating immune complexes (CIC), complement catabolic fragment C3d, complement-mediated immune complex solubilization (CMS) and immunoglobulins were made in 24 newly diagnosed with borderline tuberculoid leprosy over a 20 month period after initiation of chemotherapy. Fourteen of these patients had not suffered from reversal reactions either at the time of presentation or during the follow-up. The levels of CIC were evaluated in them from the third to the eleventh month after starting chemotherapy and immunoglobulin G (IgG) levels were evaluated up to eight months. The concentrations of C3d and immunoglobulins A (IgA) and M (IgM) were normal in these patients. The other ten patients had reversal reaction at the time of diagnosis which subsided by the third month after starting treatment. They did not have reversal reactions later. The levels of CIC and IgG were elevated and those of CMS were depressed throughout the study period. Serum C3d level was initially elevated but came down to normal by the third month while IgA and IgM levels were within normal limits. The relevance of these findings to the genesis of reversal reaction is discussed in this communication.(AU).

Abstract:The occurrence of immune complexes in the serum and the level of the C3 breakdown product C3d in the plasma from patients with leprosy were studied by quantitative methods and the results were compared in various forms of the disease. These studies were performed on sixty-two samples from twenty-six patients. The serum 125I-C1q binding activity was found to be increased by more than 2 s.d., as compared to the normal values, in most of the sera from patients with erythema nodosum leprosum (ENL) (80%) and uncomplicated lepromatous leprosy (82%), but also in the sera from patients with tuberculoid leprosy (58%). In vitro studies suggested that immune complexes involving mycobacterial antigens were present in leprosy sera. An increased C3d level (greater than 2s.d.) was also found in most of the plasma from patients with ENL (70%), but rarely in the plasma from patients with uncomplicated lepromatous leprosy (18%) and never in tuberculoid leprosy patients' plasma. The absence of a significant correlation between the 125I-C1q binding activity and the C3d level in leprosy patients may suggest that extravascular immune complexes are involved in the complement activation occurring in ENL. The quantitation of C3d in plasma may be of some practical interest in the early diagnosis of ENL complications of leprosy.(AU).

Abstract:Forty biopsies from skin lesions of untreated (24) and treated (16) leprosy patients, representing the whole leprosy spectrum, were examined by means of immunofluorescence (IF) methods. Only few or no cells staining with FTC-labelled antihuman immunoglobulin antisera were found in the lesional skin of all patients examined. Sparse IgM-deposits along the basement membrane of the epidermis were observed in a few borderline lepromatous and lepromatous patients. Complement deposits along the basement membrane of the epidermis and in the vessel walls were found in tuberculoïd as well as in lepromatous patients. Complement and in a lesser number IgG-deposits were observed around sweat glands and sometimes around sweat gland ducts and other skin appendages. Autofluorescing macrophages were noted in patients at the lepromatous side of the leprosy spectrum; approximately half of these patients showed complement deposits in an around these cells.(AU).

Abstract:Sera from patients with lepromatous leprosy react with the Clq component of complement in vitro. The incidence of sera that react in this way is higher in patients with active erythema nodosum leprosum (E.N.L.). This is further, although circumstantial, evidence for the immune complex aetiology of E.N.L. and opens a way for the more precise identification of the antigen involved.(AU).

Abstract:The sera of 87 Senegalese patients with various forms of leprosy were investigated. Two of the most reliable methods were used to detect circulating immune complexes : the radiolabelled C1q binding test and the Raji cell binding technique. Several fractions of complement, including C3, C4, factor B and the C3d product of C3 were also assayed. A material having the properties of immune complexes was detected in lepromatous and reactional leprosy. In tuberculous leprosy, only the Raji cell binding technique gave positive results. C3 and C4 were normal or slightly raised, but C3d was increased in all forms of the disease. There was no significant correlation between C3d values and the results of immune complexes detection tests.(AU).

Abstract:This study illustrates the consequences of smallpox revaccination in 45 lepromatous, 28 tuberculoid, and 47 normal individuals. Results obtained with leprosy were associated with a relative anergy against the vaccinia virus, the anergy being minimal in the tuberculoid leprosy but marked in the cases with lepromatous leprosy.(AU).

Abstract:Samples of serum from 45 patients with different clinical forms of leprosy and from 17 patients with systemic lupus erythematosus were studied in parallel for circulating immune complexes with use of two different in vitro tests adjusted to the same degree of sensitivity. The Clq deviation test relied upon the reaction of the complement component Clq with immune complexes. The Raji cell test detected complement-fixed immune complexes that bound to the complement receptors on cultured, bone marrow-derived lymphocyte-like Raji cells. Thirty (67%) of 45 patients with leprosy showed immune complexes according to the Clq deviation test; however, only two (7%) of the 30 samples of sera with positive Clq test results were positive by the Raji cell test. In contrast, 54% of 13 samples of sera from patients with systemic lupus erythematosus positive by the Clq test were positive according to the Raji cell test. Since Clq is known to react with DNA as well as with bacterial antigens, the Clq reaction may in fact be detecting antigenemia in many instances. Considerable caution is warranted in application of sensitive screening tests for assay of circulating immune complexes in various states of infectious diseases.(AU).

Abstract:Os niveis de C3 total (C3 nativo mais seus produtos de degradacao) e o grau de conversao do C3 nativo em seus produtos de degradacao foram estudados em soros de pacientes V, T, I e de V com ENH. Soros de individuos normais foram tambem analisados, enquanto os niveis de C3 total nao diferiram significativamente entre os grupos, a conversao de C3 em seus produtos de degradacao foi significativamente maior nos soros de pacientes V e naqueles com ENH. A ativacao do sistema complemento e o envolvimento dos IC neste processo sao discutidos. (AU).
