Abstract:A pilot study has been undertaken to compare the efficacy of small dose pulsed betamethasone therapy with need based oral steroids in chronic recurrent erythema nodosum leprosum (ENL) patients. Though this mode of therapy was well tolerated, no advantage with intermettnt steroid administration was observed. This coul have been on account of small dose of steroid given monthly. Treatment of chronic recurrent erythema nodosum leprosum (ENL) patients continues to be unsatisfactory, particularly, because of non-availability of thalidomide. Though corticosteroids are effective in suppressing all the manifestations and even restoring partially or fully the functional impairment, their side effects and dependence are equally troublesome. Based on (a) the reported efficacy and safety of intermittent use of corticosteroids in several immune complex mediated disorders (Cathcart et al 1976, Kimberly et al 1979), Lieblin et al 1981 and Pasricah & Gupta 1984) and (b) ENL (type II) reactions having similar pathology, a pilot study has been undertaken to see the efficacy and the tolerance of pulsed steroids in chronic ENL patients (AU).

Abstract:despite the high frequency of renal involvement associated with Hansen's disease and the persistent occurrence of leprosy, especially in the underveloped countries, reference to kidney transplantation in these patients is rare. Secondary amyloidosis and a nephritic process possibly mediated by immune complexes are present in one-half of the leprosy pateints studied either by autopsy or by kidney biopsy.(AU).

Abstract:Membranoproliferative glomerulonephritis (MPGN) is a common form of glomerulonephritis and frequently is associated with chronic infections. Leprosy, one of the most common infections worldwide, was found in conjunction with MPGN, type I, in a patient. Serological abnormalities typical of MPGN, improvement in renal function with therapy of acute complications of leprosy, and long-term renal improvement with antileprosy therapy all occurred in this patient. Others have found that MPGN is found in 11% to 43% of leprosy patients undergoing renal biopsy. Serological abnormalities typical of MPGN frequently are found in patients with lepromatous leprosy. The associations of MPGN and leprosy, and the susceptibility of the glomerulonephritis to therapy, should be emphasized.(AU).

Abstract:A renal biopsy from a patient with lepromatous leprosy but no history of erythrema nodosum leprosum (ENL) showed histologic, immunofluorescent, and ultrastructural features typical of immune-complex glomerulonephritis. A literature review found reports of 187 renal biopsies from leprosy patients, with glomerulonephritis in 31% of the cases, and a variety of other renal lesions. This accumulated evidence demonstrates that, contrary to widely held opinion, there is no association between glomerulonephritis and a history of ENL. Furthermore, it suggests that glomerulonephritis in leprosy patients has a similar incidence in lepromatous and non-lepromatous cases. These observations have an important bearing on concepts regarding the pathogenesis of immune complex glomerulonephritis in leprosy and the humoral immune mechanisms in patients with leprosy.(AU).

Abstract:The clinico-biochemical features of nephrotic syndrome developing as a terminal event in five cases of lepromatous leprosy who were subjects of recurrent lepra reaction along with the post-morterm findings in two of them are presented. The various aetiological factors that might have played a part in the causation of this condition in this group of cases are discussed. It is the suggested that the recurrent episodes of lepra reaction, which in the acute phases were associated with abnormal urinary findings indicative of a focal glomerulo nephritis, could be due to the occurrence of lesions in the kidneys structurally similar to erythema nodosum leprosum or precipitation of antigen-antibody complexes in the glomeruli. Amyloidosis may be a more frequent accompaniment of recurrent lepra reaction than hitherto known. properly designed studies for the assessment of renal functions supported by renal biopsies will serve to gauge the type of renal involvement.(AU).

Abstract:Twenty one patients of lepromatous leprosy were investigated for evidence of renal diseases. Histological lesions were observed in 76 percent cases. Sixty two percent of the cases showed a proliferative type of glomerulonephritis. Amyloidosis was present in 14 percent. The remaining 24 percent had either non-specific changes confined to renal tubules or were normal.(AU).

Abstract:Renal involvement in 13 non-lepromatous and 17 lepromatous leprosy patients were assessed by routine urinalysis, detailed biochemical analysis of blood and urine and by renal histopathological studies and compared with 10 normal healthy controls. The presence of RBC and pus cells were detected in the urinary deposit of only one lepromatous leprosy patient in reactional phase. A reversal of albumin/globulin ratio was observed in 17.6% of non-lepromatous and 15.3% of lepromatous patients. 24 hours urinary excretion of sodium, potassium, chloride and aminonitrogen of the patients were within the normal range. Forth seven percent of the non-lepromatous and 46% of the lepromatous patients had proteinuria. Even though the mean serum creatinine values of the patients showed no difference from that of the normals, the creatinine clearance was low in 82.3% of the non-lepromatous and in all of the lepromatous patients. Serum phosphorus, serum uric acid, urinary phosphate excretion and the renal tubular reabsorption of phosphorus of the patients were normal. Twenty one percutaneous renal biopsy specimens showed nonspecific pathological changes such as nephritis of various varieties in 71.4% of the specimens. Among the lepromatous group renal involvement was observed in 5 out of 9 cases (55.6%) and in the non-lepromatous group 10 out of 12 cases (83.3%). No acid fast bacilli, amyloid and granuloma were seen in any of the renal tissues studied. There was no definite correlation between the type of renal pathology and biochemical changes. None of the patient showed any clinical evidence of renal involvement.(AU).
