Abstract: Motor and sensory nerve conduction studies were performed in the distal part of the ulnar, median and radial nerves of 12 tuberculoid and 12 lepromatous leprosy patients, compared with 15 normal subjects. Slowing of sensory conduction velocity (SCV) was shown in all nerves with no difference between tuberculoid and lepromatous patients. The radial SCV slowing is correlated (P less than 0.001) with the clinical findings. Impairment of motor distal latencies was observed only in tuberculpoid patients. It is concluded that the radial SCV is the most reliable conduction test and is proposed as an early diagnostic test for leprosy.(AU). Abstract: The internal anatoly of peripheral nerve trunks has been studied and discussed in relation to some features of the neural lesions of leprosy. Special significance is attached to the perineurium, funicular structure and the intraneural funicular plexuses.The perineurium controls the spread of the micro-organisms confining them, for some time, to the affected funiculi and promoting their movement centrally along the bundles. By the time the outlines of the perineurium have been lost and the epineurium involved at any level, the micro-organisms are likley to have advanced considerable distances up the bundles.Because of the elastic resistance of the perineurium, the pressure inside the bundle builds up as the products of the inflammatory reaction slowly accumulate. Compression and ischemia of the contents of the bundle introduced in this way add to the involvement and destruction of nerve fibres due directly to the activity of the M. leprae.(AU). Abstract: The two groups of leprosy patients were compared with normal for the evaluation of the functional status of the autonomic nervous system, with the aid of several established procedures. Our findings reveal that in the leprosy patients the adrenergic system is hypo-functional as compared to the cholinergic. Abstract: A handy device for testing the thermal sensibility of skin lesions has been developed and field tested in various centres in Africa and India. The instrument performed satisfactorily and its use made testing for thermal sensibility in the field practicable and straightforward. Analysis of the results of testing 260 persons, most of whom exhibited a few lesions that were characteristic of early leprosy, showed that the rate of diagnosis of sensory impairment of such skin lesions, and hence the diagnosis of leprosy, would be about 15-25% more if thermal sensibility testing using this device were added to the other tests of sensibility routinely carried out in the field. Regular use of the device in the field would help to bring more leprosy patients under treatment than at present.(AU).
