Title:Sistema sanguíneo Lewis em pacientes hansenianos / Lewis blood group system in leprosy patients

Abstract:No presente estudo avaliamos a distribuiçäo fenotípica do grupo sanguíneo Lewis na saliva e nos eritrócitos de pacientes hansenianos. Foram analisados 47 pacientes e 40 indivíduos controles quanto a presença de antígenos ABH bem como dos antígenos Lewis nos eritrócitos e na saliva, pela reaçäo de aglutinaçäo e inibiçäo de aglutinaçäo. As frequências dos fenótipos Lewis e secretor/näo nesses pacientes e no grupo controle foram: Le(a- b+) = 29,8(por cento) 62,5(por cento); Le(a+ b-) = 25,5(por cento) 7,5(por cento); Le(a- b-) = 44,7(por cento) 30,0(por cento); secretor = 59,5(por cento) e näo secretor = 40,5(por cento) 15(por cento). Em análise de X2 encontramos diferença significativa tanto para o fenótipo Lewis como para o fenótipo secretor, em relaçäo ao grupo controle respectivamente: X2 = 10,46; GL = 2; p=0,005 e X2 = 5,64; GL = 1; p = 0,018. Da mesma forma, o teste Z de proporçöes foi realizado obtendo resultados significativos para os fenótipos Le (a + b-) e Le (a- b+), que foram respectivamente: Z = 2,219; p = 0,027 e Z = 3,058; p = 0,002. Os resultados indicam que o fenótipo Le (a+ b-) parece estar associado à suscetibilidade à hanseníase e fenótipo secretor com proteçäo. Discute-se que a presença de Le na saliva pode inibir a aderência do M. Leprae às células da mucosa, protegendo assim contra infecçäo.(AU).

Abstract:The existence of a genetic component in mycobacterial disease susceptibility is no longer in doubt and the investigations now being conducted aim to determine which genes are involved, to what extent, and in which disease phenotype they are relevant. In certain rare instances of susceptibility to poorly pathogenic mycobacteria, the genetic component is clear. The approaches employed to elucidate common disease susceptibility include linkage studies, particularly genome-wide linkage analysis of both tuberculosis and leprosy, and association studies. A number of candidate genes have shown association with tuberculosis, and in many cases, on replication of the study, association has been confirmed in a disparate population, indicating the wider importance of the gene in the disease process. In other instances, associations appear to be particular to a population or a subtype of disease. (AU).

Abstract:Each year an estimated 600000 new leprosy cases are diagnosed worldwide. The spectrum of the disease varies widely from limited tuberculoid forms to extensive lepromatous forms. A measure of the risk to develop lepromatous forms of leprosy is provided by the extent of skin reactivity to lepromin (Mitsuda reaction). To address a postulated oligogenic control of leprosy pathogenesis, we investigated in the present study linkage of leprosy susceptibility, leprosy clinical subtypes, and extent of the Mitsuda reaction to six chromosomal regions carrying known or suspected leprosy susceptibility loci. The only significant result obtained was linkage of leprosy clinical subtype to the HLA/TNF region on human chromosome 6p21 (P(corrected)=0.00126). In addition, we established that within the same family different HLA/TNF haplotypes segregate into patients with different leprosy subtypes directly demonstrating the importance of this genome region for the control of clinical leprosy presentation. Abstract:The Mitsuda test, which measures the specific immune response against intradermally injected lepromin, has a high prognostic value for susceptibility or resistance to the lepromatous form of leprosy. A sib-pair linkage analysis between the Mitsuda response and the NRAMP1 gene was done among 20 nuclear families with leprosy (totaling 118 sibs) from Ho Chi Minh City, Vietnam. All family subjects were genotyped for several intragenic and flanking NRAMP1 markers, leading to the definition of a fully informative NRAMP1 haplotype. Significant linkage was observed between NRAMP1 and Mitsuda reaction when considered either as a quantitative (P<.002) or as a categorical (P=.001) trait. Separate analyses among healthy and affected sibs showed evidence for linkage in both subsamples, indicating that linkage between the Mitsuda reaction and NRAMP1 is independent of leprosy status. These results support the view that NRAMP1 plays a regulatory role for the development of acquired antimycobacterial immune responses as determined by in vivo Mitsuda test reaction. (AU).

Abstract:Os poucos estudos já publicados sobre a determinaçäo de fenótipo secretor dos antígenos ABH na saliva de hansenianos têm demonstrado näo haver uma correlaçäo significativa entre estas substâncias e a suscetibilidade à doença. No presente estudo avaliamos 74 pacientes, sendo 27 virchovianos, 23 tuberculóides e 24 dimorfos, quanto à presença de antíngenos ABH nos eritrócitos e na saliva, pela reaçäo de aglutinaçäo em tubo e inibiçäo de aglutinaçäo. A frequência dos grupos sanguineos ABO e do fenótipo secretor e näo secretor nestes pacientes e no grupo controle foram: O= 40,5(por cento) 49(por cento); AB= 6,8(por cento) 3,6(por cento) e näo secretor= 31,1(por cento) 17,5(por cento). Analisando-se os resultados apresentados nos hansenianos, observamos que näo houve antígenos ABH pesquisados nos eritrócitos e na saliva.

Abstract:Considerando-se que a hanseníase pode ser associada com distintos padroes de resposta imunológica, foram investigados 51 indivíduos; 36 doentes, subdivididos em 12 lepromatosos, 12 tuberculóides e 12 reacionais, e 15 controles normais. A partir de separaçao de células mononucleares do sangue periférico dos indivíduos dos diferentes grupos de estudo, foi realizado o teste de linfoproliferaçao, por 72 horas, com mitógenos policlonais, fitohemaglutinina (PHA) e concavanalina-A (ConA), e com proteína C-reativa (CRP), paralelamente ao estudo de fenótipo das células em cultura. A resposta linfoproliferativa foi medida por incorporaçao de timidina, a quantificaçao das porcentagens de células CD4 e CD8 foi obtida por citometria de fluxo(AU).
