Abstract:Tissue expression of cyclooxygenase (COX)2, an inducible enzyme synthesizing eicosanoids in inflammation, was studied in reversal reaction (RR) leprosy in comparison with nonreactionary leprosy. COX2 was consistently expressed in cells of the mononuclear-macrophage lineage across the leprosy spectrum. Only in RR, the following two additional sites showed COX2 expression in the dermis and subcutis: 1) microvessels and 2) nerve bundles and isolated nerve fibers. The same sites also express vascular endothelial growth factor (VEGF). This is in keeping with experimental models relating VEGF to COX2 expression, with VEGF enhancing prostaglandin production through COX2 stimulation and prostaglandin synthase expression. We postulate that selective COX2 inhibitors, which are currently used in several inflammatory conditions, could be considered for RR treatment to reduce acute symptoms caused by tissue edema and possibly prevent long-term nerve damage, the main complication of RR. Abstract:Leprosy is an infectious disease with two polar forms, tuberculoid leprosy (TT) and lepromatous leprosy (LL), that are characterized by strong cell-mediated immunity (CMI) and CMI anergy, respectively. Transforming growth factor-beta (TGF-beta) belongs to a family of pleiotropic cytokines (TGF-beta1, TGF-beta2 and TGF-beta3) that participate in the control of cell differentiation and proliferation, as well as tissue repair. This cytokine family is unique because it suppresses CMI. In this study, we compared the expression of the three TGF-beta isoforms and their receptors in skin biopsies from LL and TT patients (LL = 20; TT = 20) using immunohistochemistry and automated morphometry. The percentage of cells immunostained for the three TGF-beta isoforms and cells positive for the three TGF-beta receptors in the inflammatory infiltrate located in the papillary dermis, reticular dermis and periadnexal tissue were significantly higher in LL than that in TT, with macrophages being the most common and strongest immunoreactive cells. Some lymphocytes, fibroblasts, keratinocytes and epithelial cells from sweat glands and hair roots were also positive. In situ reverse-transcription polymerase chain reaction corroborated the capacity of these cells to synthesize TGF-beta1 and TGF-beta receptor 2. This high expression of TGF-beta isoforms and their receptors could contribute to CMI anergy and other clinical characteristic features of leprosy, like skin atrophy. (AU).

Abstract:The sites of expression of vascular endothelial growth factor (VEGF) and of KDR, its endothelial cell receptor, were investigated in leprosy reaction Type 1, or reversal reaction (RR), by immunohistochemistry and in situ hybridization. In comparison with nonreactional leprosy, overexpression of both VEGF and KDR was seen in granuloma cells, especially epithelioid and foreign body-type giant cells, the epithelium and the vascular endothelium of RR specimens. In granuloma cells, hybridization for VEGF was stronger than immunostaining, a finding that may reflect the rapid turnover of VEGF in an immunologically dynamic situation such as RR. In the epidermis, double immunohistochemistry revealed VEGF overexpression in CDla-positive dendritic cells. The VEGF may not only be relevant for hyperpermeability and mononuclear cell differentiation (the key morphologic features in the acute, clinically evident phase of RR), but it could also be implicated in RR onset, when dendritic cells are activated in response to antigen stimulation. Abstract:One hundred and sixteen women with leprosy and 31 healthy controls were studied throughout 155 pregnancies, and their babies were observed for a period of up to two years. Babies of mothers with leprosy weighed less than those of healthy mothers; the placental weights and coefficients followed the same trend. The babies of the mothers with leprosy grew more slowly than those of the healthy mothers and these findings were most marked in the babies of mothers with lepromatous leprosy. The cause of the reduced feto-placental weight is thought to be related to the immune status of the mother.(AU).

Abstract:Investigou-se, no presente trabalho, os níveis de TGF-B1 em sobrenadantes de cultura de células aderentes, obtidas de células mononucleares do sangue periférico de voluntários sadios e pacientes com hanseníase, virgens de tratamento. Estas células foram separadas pelo Ficoll-Hypaque após remoçao das plaquetas por lavagem em tampao Versene e centrifugaçao diferencial. Os monócitos foram aderidos em placas de cultura em presença de meio RPMI 1640 suplementado com soro humano AB inativo a 2,5 por cento e cultivados, posteriormente, em meio livre de soro, na presença de LPS ou PGL-1. O TGF-B1 foi mensurado por Elisa nos sobrenadantes colhidos após 48 horas de cultura. Demonstrou-se que as células aderentes dos doentes e dos controles(AU).
