Abstract:The ultrastructure of radial cutaneous nerve biopsies from five untreated patients with lepromatous leprosy was studied. Mycobacterium leprae prenet in large numbers frequently in Schwann cells, macrophages, and endothelial cells, and occasionally in perineural cells. Proliferation of the bacilli inside these cells resulted in their foamy degeneration. It is pointed out that the Schawann cells is the target cell in the leprous infection of the nerves and that the destructive lesions of the nerves produced in lepromatous leprosy are esentially the result of Schwann-cell degeneration. Mycobacterium leprae was demonstrated in the lumen of the capillaries as well, and it is suggested that the infection may spread in the body even into the nerves through the blood-stream.(AU).

Abstract:Biopsies of the skin and liver were obtained from a patient with Lucio's type of leprosy and from one with erythema nodosum leprosum. Vasculitis, a dense inflammatory exudate, and necrosis of the skin characterized the Lucio type of leprosy; in erythema nosodum leprosum the vasculitis was not so severe and necrosis did not occur. Large numbers of organisms in the endothelial cells of the vessels in the skin could have given rise to bacteremia and development of the disease in the liver. The liver in the patient with Lucio's phrnomenon contained tuberculoid lesions, whereas necrosis of sinusoidal walls and many Virchow's cells were noted in the other patient. Lperosy bacilli in Kupffer cells were easily demonstrated with electron microscopy. The presence of the bacteria within membrane-bound phagocytic vacuoles and the fusion of lysosomes with the vacuoles were seen.(AU).

Abstract:The spleen of the armadillo is invested by a capsule composed predominantly of smooth muscle. Inward extensions from the capsule contribute to an extensive system of trabeculae of the non-vascular type. The white pulp presents series of venous sinsuses. The capillary segment of the penicillar artery is invested by an ellipsoid sheath. The spleen of the armadillo may be included in the class of mammalian spleens characterized as sinusal in nature and possessing ellipsoid sheaths. Electron micrographs of thin sections of the sheathed capillaries demonstrate a high-type of endothelium. A characteristic feature of this endothelium is the presence of a distinct intracytoplasmic fibrous component. The reticular cells comprising the sheath proper exhibit various sized vacuoles and inclusion bodies suggestive of their potentially phagocytic nature. In both light and electron microscopy, venous sinuses lie in close proximity to the paripheral cells of the ellipsoid sheaths. Within the wall of the venous sinuses, patent openings are apparent between the lining cells. If such an arrangement exists in the functional state of the organ, it could provide for an additional means of exposure of both plasma and cellular elements to the adjacent phagocytic cells of the sheath proper.(AU).

Abstract:While studying circulating immune complexes (CIC) in the sera of lepromatous patients by the polyethylene glycol (PEG) precipitation technique, we found (by light microscopy) abundant acid-fast bacilli (AFB), morphologically similar to those seen in slit skin smear preparations from these patients, precipitated with the PEG precipitated materials. Both solid and non-solid AFB could be readily identified. Ultrastructures of these AFB in the PEG aggregates showed some similarity with those detected in the PEG precipitates prepared from armadillo-derived lepromin under identical conditions. The most striking difference between the AFB in the test sera and that in the armadillo-derived lepromin was the absence of any transverse band in the former. This suggested that the AFB in the patients' circulation were not in division stage. Furthermore, electron-dense material were deposited on the AFB co-precipitated from the patients' sera, which were not found on the AFB co-precipitated from the armadillo-derived lepromin.(AU).

Abstract:Gross changes of myelinated fibres and Schwann cells at different degenerative stages were present in all five ENL nerve lesions. Besides these changes, infiltrating cells mainly macrophages, deposition of excessive collagen and perineurial vessel damage were also observed.(AU).

Abstract:The ultrastructural changes in radial cutaneous nerve biopsies from 3 tuberculoid and 14 tuberculoid-borderline patients were studied. In tuberculoid leprosy the nerve parenchyma was almost entirely replaced by tuberculoid granuloma. In nerves from borderline-tuberculoid leprosy many axons showed segmental demyelination and Wallerian degeneration. The nerve tissue was gradually replaced by proliferating collagen fibrils and inflammatory granulomata. M. leprae were rare and when present they were found in phagolysosomes of Schwann cells and macrophages. Schwann cells were surrounded and occasionally infiltrated by lymphocytes and macrophages. It was suggested that "sensitized Schwann cells" were the target of a hypersensivity reaction resulting in their degeneration or death which in turn produced segmental demyelination or Wallerian degeneration.(AU).
