Abstract:Um homem de 31 anos de idade procurou o serviço de saúde especializado devido a alterações de sensibilidade em braço esquerdo. O exame clínico deu suporte a esta queixa. Os esfregaços obtidos a partir da linfa colhida nesta área não desmontraram bacilos álcool-ácido-resistentes (BAAR). A reação de Mitsuda foi negativa e uma biópsia do local não mostrou bacilos ou reação inflamatória. Mesmo assim optou-se pelo diagnóstico de hanseníase indeterminada, e foi instituída a poliquimioterapia para paucibacilar. Nove anos após, o paciente retorna apresentando várias lesões de padrão dimorfo, com quadro histopatológico característico e baciloscopia do esfregaço e da biópsia positiva. Os autores consideram que este indivíduo já era um multibacilar por ocasião do primeiro diagnóstico em 1997 e o tratamento insuficiente não conseguiu destruir todos os bacilos. Nesta situação, os bacilos remanescentes poderiam ter proliferado lenta e progressivamente, reproduzindo o longo período de incubação próprio dos indivíduos Mitsuda negativos. Discutem-se as limitações do diagnóstico exclusivamente clínico e baseado no número de lesões específicas.(AU).

Abstract:Um indivíduo com 49 anos de idade recebe o diagnóstico de Hanseníase Virchoviana sub-polar. Ele referiu, durante os prévios 11 anos, sintomas, principalmente neurológicos, característicos de hanseníase, mas este diagnóstico não foi definido nos serviços médicos que freqüentou. Após 6 meses de poliquimioterapia passou a apresentar neurites que foram tratadas como reação tipo 2. Em um último episódio apresentou febre, mal estar, mialgias e lesões necróticas e ulcerativas nos membros, nádegas, dorso, pavilhão auricular esquerdo. O aspecto clínico destas lesões era semelhante as lesões observadas no fenômeno de Lúcio e no Eritema nodoso necrosante. As biópsias mostram alterações da microvasculatura superficial, necrose epidérmica e dérmica, similares a estas reações, mas as alterações básicas são vasculites granulomatosas no derma profundo e tecido celular sub-cutâneo. Estes aspectos clínicos e histopatológicos são interpretados e discutidos como reação tipo 1 (reação reversa) com envolvimento vascular predominante.(AU).

Title:Tuberculoid changes in leprosy. III. The pathology of a nerve abscess.

Abstract:A description is given of the histopathology of a specimen of the peculiar chronic leprous nerve abscess of India, the frequency of wich in that country - if not its occurrence at all - constitutes one of the regional peculiarities of the disease. The tissue is a highly organized tuberculoid granuloma, with outer capsular and inner pseudo-villous layers. The latter consists of isolated, vascular lymphoid follicles clothed top and sides with a layer of proliferating tuberculoid tissue; necrosis of this occuring beyond a given distance produces villosities in a strikingly regular manner. The cpsule is of comparatively finely-knit structure, well supplied with vessels; it apparently derived from the perineurium, indicating that the process arose within a nerve cord. This finding, in view of the evident frequence of tuberculoid affection of the nerve in India, and on the other hand the non occurrence in the Philippines of abscess of this kind and the rarity and unimportance there of tuberculoid lesions of the nerve, indicates that the latter forms the basis of the former. In view of the clinical features, and especially of Lowe´s opinion that this absces formation is a reaction condition and greatly increased in frequency by the use of potassium iodite, it is accepted as entirely probable that the condition is a manifestation of lepra reaction and, more specifically, of chronic, persistent lepra reaction in a tuberculoid lesion. The extensive epithelioid production is in keeping with findings in chronic reaction of tuberculoid skin lesions and is a further indication that that feature is characteristic of chronic reaction in lesions of that nature. Leprosy bacilli were not found in this specimen, though Lowe found a few, apparently degenerated, in about half of his cases. However, though there is need of further control work, the causative agent is in all probability that of leprosy. It is believed that these abscesses are worthy of further attention. It remains to be determined whether they are regularly tuberculoid and regularly associated with reaction. The fact that they are apparently limited to males involves an intriguing question. Finally, material from these lesions might prove of special value in an investigation of the hypothetical submicroscopic form or phase of the leprosy bacillus. (AU).

Abstract:Biopsies of the skin and liver were obtained from a patient with Lucio's type of leprosy and from one with erythema nodosum leprosum. Vasculitis, a dense inflammatory exudate, and necrosis of the skin characterized the Lucio type of leprosy; in erythema nosodum leprosum the vasculitis was not so severe and necrosis did not occur. Large numbers of organisms in the endothelial cells of the vessels in the skin could have given rise to bacteremia and development of the disease in the liver. The liver in the patient with Lucio's phrnomenon contained tuberculoid lesions, whereas necrosis of sinusoidal walls and many Virchow's cells were noted in the other patient. Lperosy bacilli in Kupffer cells were easily demonstrated with electron microscopy. The presence of the bacteria within membrane-bound phagocytic vacuoles and the fusion of lysosomes with the vacuoles were seen.(AU).

Abstract:Cryostat sections of dermal lesions from 30 untreated leprosy patients were studied by indirect immunofluorescence using monoclonal antibodies defining T cell subsets and Ia like antigens. Most lymphocytes in leprosy lesions were positive for OKT3 and Ia like antigens indicating thereby the presence of activated T cells. Maximal numbers of these cells were seen in localized paucibacillary tuberculoid leprosy lesions in close association with epithelioid cells. A decline in their numbers was observed over the leprosy spectrum with a marked reduction in disseminated, multi-bacillary, lepromatous leprosy where only scattered OKT3+ cells were visualized. OKT4 and OKT8 positive cells defining T cell subsets, were frequently found within the OKT3+ lymphocytes throughout the leprosy spectrum. The ratio of OKT4+/OKT8+ cells ranged from 1.2 to 5.0 in tuberculoid and from 0.2 to 1.0 in lepromatous lesions. Macrophages in the granulomas stained intensely with anti-Ia antisera. Ia like antigens were expressed to the same degree on macrophages with or without intracellular acid fast bacilli.(AU).

Abstract:A clinical and histhopatological review of thirteen cases of cauliflower growths occurring in neurophatic plantar ulcers in leprosy patients is presented. The majority of these lesions were found to be psuedo-epitheliomatous hyperplasias although a considerable number showed atypical histological features. Conservative surgical treatment by deep but limited local excision is advocated.(AU).
